This paper examines the uses of evidence-based medicine (EBM) in post-Soviet Russia through the use of semi-structured interviews. It asks what it means to talk about practicing evidence-based medicine in a setting where the context of practice presents considerable barriers to the implementation of EBM principles. Drawing on interviews with Russian physicians, medical students and users of the healthcare system, the paper argues that in post-Soviet Russia EBM serves as a strategic discourse for segments of the medical profession. With the collapse of the U.S.S.R. the healthcare system has been going through a period of crisis, and Russian physicians are finding that they have to redefine their professional identity with respect to the domestic and the international context and have to seek new sources for legitimating their professional position. The western origins of EBM endow this rhetoric with considerable power in the Russian context and render it a very useful tool in the project of redefinition.  相似文献

目的探索一种适合ICU护理工作特点的管理模式,提高管理效应。方法用护理工作量、ICU护理质量标准奖惩考核、职称系数三部分综合考评护理工时,与护士的绩效工资分配直接挂钩。结果护理工时量化法实施后,护理质量考核评分,患者?医生对护理质量满意率显著高于实施前(P〈0.05,P〈0.01)。结论护理工时量化绩效分配克服了经验式管理的弊端,建立了有效的激励机制,体现了多劳多得、优劳优得的分配原则,挖掘了护理人员的自身潜力,提高了护理质量。  相似文献

This study of interprofessional work relations in a Canadian mental health team examines how nursing deployed different forms of power in order to alter the mental health division of labour, to gain administrative, organizational and content control over its own work, expand its jurisdictional boundaries by expropriating the work of other professionals, and exclude others from encroaching on its old and newly acquired jurisdictions. This is set against the context of nursing's long-standing professional project to consolidate and expand its professional jurisdiction. Using an ethnographic study of a single interprofessional mental health team in a psychiatric hospital in Canada, the paper attempts to understand the politics and paradoxes involved in realizing nursing's professional project and how the politics of professional autonomy and professional dominance are actually conducted through micro-political struggles. The data demonstrates the effects of the political struggles at the organizational and work process levels, particularly in the forms of collaboration that result. Nurses gained substantial autonomy from medical domination and secured practical dominion over the work of non-medical professionals. New forms of interprofessional collaboration were accomplished through both simultaneous and sequential micro-political struggles with psychiatrists and non-medical professionals, and the formation of political alliances and informal agreements. Nursing solidarity at the elite level and substantial effort by the elite nurses and their committed colleagues to mobilize their less enthused members were fundamental to their success. The nurses deployed political (power) strategies and tactics to organize and reorganize themselves and other professionals on multiple levels (politically, organizationally, ideologically, socially and culturally). This study reveals the complexity and robustness of micro-political dynamics in the constitution of professional and collaborative interprofessional work relations.  相似文献

目的 探讨宁波市普通高中的教师心理压力,社会支持与亚健康状态的相关关系.方法 采用职业压力问卷、社会支持量表与亚健康躯体症状自评量表对随机抽取的128名宁波市普通高中教师进行相关关系分析.结果 当存在社会支持变量时,2组变量的典型相关系数达0.563,0.471( P <0.01).职业压力的增大与社会支持水平的减少都将导致教师亚健康躯体症状的增多.当撤去社会支持变量后,典型相关系数为0.497( P <0.01).典型变量ξ中"考试压力","工作负荷"与"职业期望"的因子负载分别增大为0.801,0.855,0.726.典型变量η中的"疲劳感觉"与"睡眠障碍"的因子负载分别增大为0.774,0.859.结论 社会支持水平在职业压力与亚健康躯体症状之间可能具有中介调节作用.  相似文献

目的探讨如何对新调入肿瘤科的护士进行专科教育的有关问题。方法帮助新护士尽快适应工作环境,进行必要的专科知识教育和专科技术传授,熟练掌握肿瘤病人的心理特点及健康教育技巧。结果新护士在调入1个月后能单独完成本职工作,未发生差错和事故,同事对新调入护士评价好,病人满意。结论护士长对新调入护士有目的地进行专科理论知识教育和专科技术操作传授,言传身教,对尽快提高新调入护士的工作能力,确保护理质量起着重要的作用。  相似文献

Training needs analysis is considered essential to professional and organizational development, for continuing education and for incorporation into professional practice. Increasingly, nurses are encouraged to develop their health promotion role. Despite this there has been relatively little research in Scotland into the health promotion role of the hospital nurse. This study describes the results of a health promotion training needs analysis undertaken on qualified, hospital-based nurses in Lanarkshire, Scotland. Information on the nurses' current health promotion practices, their attitudes and beliefs, their views on role development and priorities for further training were collated, analysed and discussed within the framework of the Health Action Model. This model was originally designed to assist understanding of the gap that exists between an individual intention to act and the eventual health action. However, in this study the ways in which ‘cognitive factors’ (knowledge and beliefs) and ‘motivational factors’ (considering values, attitudes and drives) and pressures from social norms and significant others assisted in the understanding of the nurse's orientation to, and beliefs about, health promotion practice. Fifty-seven per cent of nurses in the study considered health promotion activities to be emerging in clinical care and 4% considered them to be advanced. The interest respondents have in role development and the further integration of health promotion activities into nursing practice was explored by ascertaining the nurses' attitudes and beliefs about their own health promotion role. Of 107 respondents, a majority 72 (67%) agreed health promotion interventions to be an important function of the nurse with 29 (27%) strongly agreeing (n=107). Sixty per cent stated they would be interested in developing their role and 30% were very strongly interested. Recommendations are therefore made for future planning and development strategies for the health promotion role of hospital nurses.  相似文献

Cancer patients are increasingly being cared for in the community. However, the role of the community oncology nurse specialist (CONS) remains a rare appointment in the United Kingdom. There is little research evidence to substantiate the view that this is a worthwhile and effective role. This study aimed to explore the role of one CONS using a qualitative case study approach. The sample consisted of the CONS, seven patients, no relatives and five community nurses. Semi-structured interviews were used to elicit the view of these participants. In addition, documents containing details of the CONS's work to date were reviewed. A number of the CONS's role components were identified by the different respondent groups. Five themes were identified within the data collected: functions of the CONS, communication between the CONS and other health care professionals, structural and organizational factors, characteristics of the service and benefits to patients and their families. Furthermore, all respondent groups were positive about the service offered by the CONS, and felt that the role was valuable. Some negative features were identified and these were concerned with organizational aspects of the service. The findings suggest the need for further research and evaluation in this area.  相似文献
