Symptom validity testing is a major topic in the field of neuropsychological research, but until now, few studies focus on effort testing in children. Three symptom validity tests (SVTs), the Medical Symptom Validity Test, the Test of Memory Malingering, and the Fifteen Item Test plus several standard neuropsychological tests were administered to 73 German-language school children from 6 to 11 years.Participants were either instructed to give full effort or to follow a malingering scenario. It could be demonstrated that, except for one child, all participants with a basic reading level of grade 2 were able to pass all administered SVTs according to established cutoffs for poor effort (i.e., earned a score higher than the cutoff). For the experimental malingerers, however, it was fairly difficult to act according to the scenario throughout the session. While they scored worse in the neuropsychological tests, all but one of them failed at least one SVT. The results support the use of SVTs in childhood age. More elaborate experimental designs and studies with bona-fide patients and suspected malingerers are needed in order to evaluate both the appropriateness of available effort tests and the capabilities of children to fake poor performance.  相似文献

目的:探讨不同气味(苹果、香水、樟脑)对小鼠学习记忆能力及海马cAMP反应元件结合蛋白(CREB)和磷酸化的CREB(pCREB)的影响。方法:让小鼠在不同气味的环境下生活14d,在第7d开始方形水迷宫训练,3d后进行测试,连续测试5d。测试完后断髓处死动物,取出脑组织,用免疫组织化学染色观察海马pCREB和CREB表达情况,并进行图像分析。结果:樟脑组和香水组水迷宫的潜伏期较对照组延长,错误次数增多(P<0.05)。免疫组化染色显示鼠海马CREB的磷酸化水平大大降低(P<0.05),但对CREB的表达无明显影响。苹果组与对照组比各指标均无显著差异(P>0.05)。结论:樟脑气味和香水气味对小鼠记忆能力有负面作用,且这种作用可能是通过降低CREB磷酸化水平而实现的。苹果气味对小鼠记忆能力无明显影响。  相似文献

目的探讨股骨粗隆间骨折合并同侧股骨干骨折的手术治疗方法及疗效。方法回顾分析我院自1998年1月至2005年12月收治股骨粗隆间骨折合并同侧股骨干骨折患者18例,全部病例均采用手术切开复位内固定,股骨粗隆间骨折应用动力髋内固定,股骨干骨折采用记忆合金环抱器内固定。结果术后随访2~8年,平均4.2年。全部病例均获骨性愈合,疗效满意。结论股骨粗隆间骨折合并同侧股骨干骨折在下肢骨折中相对较少,但处理较为复杂和困难,治疗有一定难度,使用动力髋与记忆合金环抱器同时进行内固定,方法简单,固定牢靠,患肢可早期进行功能锻炼,这种方法是治疗单侧肢体多处骨折较好的方法之一。  相似文献

Alpha‐mannosidosis (AM) is a very rare (prevalence: 1/500000 births) autosomal recessive lysosomal storage disorder. It is characterized by multi‐systemic involvement associated with progressive intellectual disability, hearing loss, skeletal anomalies, and coarse facial features. The spectrum is wide, from very severe and lethal to a milder phenotype that usually progresses slowly. AM is caused by a deficiency of lysosomal alpha‐mannosidase. A diagnosis can be established by measuring the activity of lysosomal alpha‐mannosidase in leucocytes and screening for abnormal urinary excretion of mannose‐rich oligosaccharides. Genetic confirmation is obtained with the identification of MAN2B1 mutations. Enzyme replacement therapy (LAMZEDER) was approved for use in Europe in August 2018. Here, we describe seven individuals from four families, diagnosed at 3–23 years of age, and who were referred to a clinical geneticist for etiologic exploration of syndromic hearing loss, associated with moderate learning disabilities. Exome sequencing had been used to establish the molecular diagnosis in five cases, including a two‐sibling pair. In the remaining two patients, the diagnosis was obtained with screening of urinary oligosaccharides excretion and the association of deafness and hypotonia. These observations emphasize that the clinical diagnosis of AM can be challenging, and that it is likely an underdiagnosed rare cause of syndromic hearing loss. Exome sequencing can contribute significantly to the early diagnosis of these nonspecific mild phenotypes, with advantages for treatment and management.  相似文献

Susceptibility to apoptosis changes with age and most of the available data on lymphocytes refer to mitogen stimulated cells. We studied this susceptibility in quiescent, purified CD4+ or CD8+ T cells from a group of Italian old people compared with a group of young people. We found that an apoptotic agent such as 2-deoxy-D-ribose (dRib), which acts via glutathione depletion and oxidative stress, was more effective in CD4+ T cells from young donors, while no difference was found in CD8+ T cells. On the contrary, another agent such as TNF-alpha, which acts via receptor engagement, was more effective in CD8+ T cells from old subjects, and no difference was found in CD4+ T cells. When marker of activation-memory were investigated, no difference between young and old subjects was found when dRib was used. Differently, when TNF-alpha was used, memory and activated CD4+ T cells from old donors were less sensitive than younger counterparts, while memory CD8+ T cells from old donors were more sensitive than younger counterparts. This suggests that age-related changes in susceptibility to apoptosis of resting T cells largely depend on the type of the apoptotic stimulus which is used as well as on the memory phenotype of the cells. These results may also account, at least in part, for the deep remodelling of T cell repertoire that occurs during ageing.  相似文献

Cebus monkeys of 3 different age groups were trained to perform an automated behavioral task (delayed response), intended to measure recent memory ability. In in initial study, the aged monkeys (18 years and older) exhibit prprogressively greater performance impairments (relative to young monkeys) as they were required to remember the location of a visual stimulus for increasingly longer durations (0 to 20 sec). This deficits replicated previously published results from aged Rhesus monkeys and appeared similar to the primary memory deficits reported in elderly humans and demented patients. In subsequent studies, the effects of three different cholinomimetics were evaluated for their ability to improve the aged monkey's performance on this task. Each monkey was tested under several acute doses of the cholinergic precursor, choline, the anticholinesterase, physostigmine, and the cholinergic muscarinic receptor agonist, arecoline. The results revealed clear differences in the ability of these drugs to improve performance on this task. Choline exerted no apparent effects in the aged monkeys at any dose tested. Physostigmine clearly enhanced performance in certain aged monkeys, but the optimal dose varied dramatically between subjects, replicating previously published results with aged Rhesus monkeys and humans. Arecoline produced clear improvement within a restricted dose range, with little variation in optimal dose between subjects. In addition to demonstrating differences in the effects of different cholinomimetics on memory performance in aged primates, these data also suggest a possible rationale for future investigations. Assuming that each of these drugs primarily affected cholinergic function in the manner conventionally attributed, these data suggest that, within the cholinergic system, the more directly one stimulates the receptor, the more one might expect robust and consistent effects on memory performance in aged subjects.  相似文献

Summary Previous unpublished experiments from this laboratory had revealed only post-activation depression effects in the cerebellar cortex when its inputs were activated by high frequency trains. In the experiments reported in this paper, we found reliable long-term potentiation (LTP) effects in the deep nuclei (interpositus and vestibular) when stimulation trains were applied to the white matter at the point where inferior peduncle fibers enter the cerebellum. LTP effects were found in both acute and chronic preparations. In the chronic preparations, LTP lasted for at least 8 days in all but one animal.  相似文献

The primary interest of our laboratory is understanding how the signals that a T cell receives influence its behavior during an immune response. Recently, we have focused our attention on the examination of T cell responses at an individual cell level. Relations between orienting response and span of immediate memory were studied by measuring skin potential responses (SPR) and heart rate (HR). Four conditions were studied by presenting letters in a tachistoscope and a 1000 cycle, 100 db tone simultaneous on some but not all trials. The conditions (15 Ss in each) were: tone and letters for 10 trials, then letters alone for 10 trials; tone and letters for 20 trials; only letters for 10 trials, followed by letters and tone for 10 trials; and only letters for 20 trials. The results showed: (1) positive SPR habituated and negative did not, (2) tone produced more SP activity, (3) HR showed a shift from acceleration to deceleration over 20 trials, but tone had no influence, (4) tone had no direct influence on span scores, (5) Ss showed improvement in number ol letters reported correctly. There was a significant correlation between span and negative SPR when tone was sounded (r =. 36).  相似文献
