Time to REST: epidemiology and burden

Restless legs syndrome (RLS) is a neurological disorder characterized by a compelling urge to move, accompanied by disturbing sensations within the legs. It is now recognized that RLS is a common condition that has a substantial impact on sleep, daily activities and quality of life. Recent data from the REST (RLS Epidemiology, Symptoms, and Treatment) general population study show that symptoms of RLS are present in approximately 7% of the general population, and that 2–3% experience moderate or severe symptoms at least twice a week. Amongst this RLS cohort, approximately 88% reported sensory disturbances and 76% reported sleep-related symptoms. These symptoms had a marked effect on everyday life, with up to 50% of the RLS cohort reporting disruption of everyday activities or personal relationships. Sleep disturbances in turn lead to impaired daytime functioning. RLS is associated with a significant impairment of quality of life, comparable with that seen in chronic medical conditions such as diabetes or depression. Given the significant burden of morbidity associated with RLS, there is a strong case for treatment in patients with troublesome symptoms.     

直肠Ca男性患者手术病人麻醉后插尿管的临床观察

目的观察直肠Ca男性患者手术麻醉后插尿管对患者的好处。方法对92例直肠Ca手术男性患者随机分成实验组及对照组进行麻醉后插尿管及常规术前1小时插尿管做法对比。结果直肠Ca手术男性患者选择麻醉后插尿管更优于术前1小时常规插尿管,成功率更高。结论直肠Ca男性患者手术采用麻醉后插尿管值得推广采用。     

溃疡散预防消化性溃疡复发临床观察

目的 :观察溃疡散对消化性溃疡的抗复发治疗的临床效果。方法 :随机将 80例患者分成溃疡散治疗组 (4 2例 )和雷尼替丁对照组 (38例 )并同时观察两组患者治疗前后溃疡复发情况。结果 :治疗后治疗组溃疡复发率明显低于对照组 ,P <0 .0 1,两组对比有非常显著性差异。结论 :治疗组能明显的改善患者临床症状和降低消化性溃疡复发率。     

某地13所医院1997-2001年门诊费用的基本分析与评价

采用回顾性调查分析方法,对某地13所医院1997-2001年门诊费用变动情况进行分析。统计学处理结果提示样本医院5年来门诊费用平均发展速度和平均增长速度加快明显。作者对此提出借鉴国外教训、避免浪费的建议,以应对医疗费飞涨给全局工作带来的各种冲击。     

Leptomeninges as a site of relapse in locally controlled,diffuse pontine glioma with review of literature

Case report Leptomeningeal involvement in diffuse intrinsic brain stem gliomas is rarely diagnosed clinically and in majority of the instances diagnosed only on postmortem examination. We report two cases of diffuse pontine glioma diagnosed clinicoradiologically and treated with conventional radiotherapy.Observations On follow-up, both patients showed clinical features suggestive of meningeal spread although imaging for pontine tumor showed stable disease. Leptomeningeal disease in the spine was confirmed on imaging and in one case by cerebrospinal fluid examination also. During the follow-up of patients with pontine glioma, the possibility of leptomeningeal involvement must be borne in mind.     

宫颈癌放疗后家庭阴道冲洗效果观察

目的:为寻找一种院外阴道冲洗的有效方法,从而帮助一些由于各种原因不能来医院行阴道冲洗术的患者继续完成治疗。方法:对204例宫颈癌放疗结束后的患者随机分实验组与对照组,实验组实施家庭阴道冲洗,对照组未行阴道冲洗,通过放疗结束后1个月、3个月、6个月门诊随访,观察阴道粘膜急性放射反应治疗后消退情况,对家庭阴道冲洗患者和未行阴道冲洗者进行对照研究。结果:行家庭阴道冲洗者在阴道粘膜急性放射反应方面与未冲洗者差异有显著性(P<0.05)。结论:家庭阴道冲洗是对因各种原因出院后不能来医院行阴道冲洗的患者可以采取的一种行之有效的方法。     

福参的苷类成分

目的 从福参Angelica morii提取分离活性成分。方法 采用乙醇提取，溶剂萃取，大孔树脂、聚酰胺、硅胶柱层析，LH-20、重结晶等纯化，对福参饮片进行系统提取分离；运用波谱学方法及理化常数对照进行结构鉴定。结果 从正丁醇和水部分分离到6个苷类化合物，分别为升麻索苷(prim-O-glucosylcimifugin，Ⅰ)、紫花前胡苷(marmesinin，Ⅱ)、芹菜素7-O-β-D-葡萄糖苷(apigenin 7-O0-β-D-glucoside，Ⅲ)、木犀草素7-O-β-D-葡萄糖苷(1ute—olin 7-O-β-D-glucoside，Ⅳ)，umbeliferone 7-apiosylglucoside(V)和胡萝卜苷(daucosterol，Ⅵ)。结论 6个化合物均为首次该植物中分得；化合物Ⅲ首次从当归属中分得。     

三级医院病人就诊流向探讨

研究通过对包括专科医院在内的多家三级医院住院病人资料的分析，将疾病种类按ICD－9编码分为19类，研究各类疾病应往低级别医院分流的比例。结果表明：各大类疾病的分流比例不同，分流比例较高的主要是常见病和多发病，总的分流比大约在60％左右。     

TUVP手术时间对血红蛋白及电解质的影响

目的 了解经尿道前列腺汽化电切手术时间对血红蛋白及血清电解质的影响。方法 对64例经尿道前列腺汽化电切术患按手术时间分组，观察手术前后血红蛋白及血清电解质的变化。结果 手术时间对血清电解质的降低有的影响，对血红蛋白降低的影响不显。结论 减少TUR综合征的关键是要操作熟练，缩短手术时间及手术中注意止血。
