Chromosomes and survival in multiple myeloma. A banding study of 25 cases

The chromosomal findings in 25 cases of multiple myeloma are reported. The chromosomes were directly prepared from bone marrow cells and studied by trypsin-Leishman banding. Abnormal stem lines were found in 16 cases (64%), and were characterized by banding in 12 cases. Twelve cases were hyperdiploid; 11 cases carried marker chromosomes, 50% of which involved chromosome No. 1. Five cases were trisomic for 1q21-1q3. A vulnerable point was located on 3q27 or 28 or 29. The two cases that carried the 14q+ marker chromosome were also the only cases with plasma cells in the peripheral blood. Based on this study and the literature it is suggested that a 14q+ is necessary for the development of plasma cell leukemia. The actuarial survival curves gave no evidence for major prognostic significance of the demonstration of an abnormal cell clone. Trisomy for 1q21-1q3 may be a bad prognostic sign. The results are discussed with special reference to the aberrations in acute nonlymphocytic leukemia as reported earlier in a series of 88 cases from this laboratory.     

Production of monospecific antibodies to varicella zoster virus antigen in rabbits tolerant to human IgG and immunized with antigen immunoprecipitated with human IgG from zoster reconvalescent sera

A method for production of antisera specific to a single antigen of a human infectious agent is described with varicella zoster virus (VZV) as a model. Microisolation of immunochemically pure antigen contained in defined immunoprecipitates was achieved by crossed immunoelectrophoresis of a crude antigenic preparation (detergent solubilized, VZV infected cells) into agarose gel containing zoster reconvalescent immunoglobulin. Immunization of rabbits made tolerant to human IgG with immunoprecipitates resulted in the production of antibodies to the antigen part of the immunoprecipitate. The method represents a short cut to the production of heterologous diagnostic antisera to defined antigens of human infectious agents.

目的:为了我室血细胞分析的各参数结果与全世界拥有该型号仪器的实验室检测的结果具有可比性.方法:通过在线质控(Online QC)模式,XE-2100每天的质控数据经网络实时传送到日本在线质控统计服务器上,自动计算质控数据与同组统计结果进行正确度、精确度和趋势等三方面的比较,并自动判断是否失控,几分钟后可将回报结果显示出来.结果:XE-2100在线质控共45项参数,批号为60650802均在控.结论:在线质控最大的特点在于将传统的室内质控与室间质评很好地结合起来,不仅使每天的室内质控有了国际统一判断标准,而且比传统的室间质评更及时,包含的参数更多.     

Use of fibrin glue in thoracic surgery

The results of closure of various types of postoperative thoracic fistulas with two-component fibrin sealant in 5 patients are presented. The use of a new technique for the noninvasive closure of bronchial fistulas with fibrin sealant is also described. Implications of the management of thoracic fistulas with fibrin sealant are discussed.     

两种血液分析仪测定结果的比较

目的:比较Sysmex XE-2100血液分析仪与Abbott Cell-Dyn3700血液分析仪两者的测定结果并作出评价。方法:分别用Sysmex XE-2100血液分析仪与Abbott Cell-Dyn3700血液分析仪同时测定136例住院患者静脉血标本,并将两者测定结果比较,然后将90例标本与显微镜白细胞分类结果进行相关性分析。38份经镜检确认含NRBC的标本,同时用Sysmex XE-2100与Abbott Cell-Dyn3700进行测定,观察两者的测定结果。结果:Sysmex XE-2100与Abbott Cell-Dyn3700血液分析仪的测定结果相关性良好;在NRBC方面,Sysmex XE-2100的假阴性率比Abbott Cell-Dyn3700为低;两者白细胞分类结果与显微镜计数结果相关性都良好,Sysmex XE-2100中性粒细胞、淋巴细胞、单核细胞、嗜酸性粒细胞和嗜碱性粒细胞相关系数(r)为0.9859,0.9775,0.8053,0.8695,0.5243(P<0.01);Abbott Cell-Dyn3700的r分别为0.9798,0.9750,0.8052,0.6531,0.3568(P<0.01)。结论:Sysmex XE-2100血液分析仪和Ab-bott Cell-Dyn3700血液分析仪是较为理想的血液分析仪,可以满足不同常规实验室的不同需要。     

Assessment of an immature platelet fraction (IPF) in peripheral thrombocytopenia

A new automated method to reliably quantify reticulated platelets, expressed as the immature platelet fraction (IPF), has been developed utilizing the XE-2100 blood cell counter with upgraded software (Sysmex, Kobe, Japan). The IPF is identified by flow cytometry techniques and the use of a nucleic acid specific dye in the reticulocyte/optical platelet channel. The clinical utility of this parameter was established in the laboratory diagnosis of thrombocytopenia due to increased peripheral platelet destruction, particularly autoimmune thrombocytopenic purpura (AITP) and thrombotic thrombocytopenic purpura (TTP). Reproducibility and stability results over 48 h were good. An IPF reference range in healthy individuals was established as 1.1-6.1%, with a mean of 3.4%. Patients in whom platelet destruction might be abnormal, were studied and two of these patients followed serially during the course of treatment. The IPF was raised in several disease states. The most significant increases in IPF values were found in patients with AITP (mean 22.3%, range 9.2-33.1%) and acute TTP (mean 17.2%, range 11.2-30.9%). Following patients during treatment demonstrated that as the platelet count recovered the IPF% fell. These results show that a rapid, inexpensive automated method for measuring the IPF% is feasible and should become a standard parameter in evaluating the thrombocytopenic patient.     

Inhibition of the immediate allergic reaction in the nose by the beta-2 adrenostimulant fenoterol

Sympathomimetic agents inhibit antigen-induced release of histamine in animal experiments and in in vitro studies of human tissue, but the significance of this effect for treatment of allergic airway diseases has not been established. It was the aim of our study to investigate the protective effect of the beta-2 adrenostimulant fenoterol on allergen-induced nasal symptoms and to relate the efficacy to dose and time. In a double-blind crossover design 16 patients with hay fever were challenged with grass pollen extract in the nose after pretreatment with intranasal placebo and fenoterol in different doses, delivered as a micronized powder from a pressurized canister and as a watery solution from a pump spray. Sneezing was recorded, and rhinorrhea and changes in nasal airway resistance were measured for 15 min after the provocation. Fenoterol, micronized powder as well as solution, had a marked inhibitory effect on all three nasal symptoms (p < 0.05) in doses ordinarily used for inhalation in the treatment of branchial asthma. The effect lasted for more than 4 hr. Our study has suggested that topical application of beta-2 adrenostimulants has a mast cell stabilizing effect in the airways and that these drugs may be of value for treatment of allergic rhinitis.     

门诊病人血液分析和白血病筛查的探讨

目的探讨XE-2100血细胞分析仪血液分析结果白血病诊断筛查的方法。方法对500例仪器有提示的标本,染血涂片显微镜检查,分类200个有核细胞,计数各类细胞的百分比。结果发现500例标本中,白血病25例,粒细胞核左移血象370例,异形淋巴血象44例,有不典型淋巴细胞的血象51例(不典型淋巴细胞<10%),有10例镜检呈阴性。结论对于有提示的标本,应全面分析其原因,并加强血涂片的镜检,防止白血病漏检,提高检验质量。
