目的对近年膝关节周围截骨术治疗膝外翻性骨关节炎的手术方式研究进展进行综述。方法查阅近年国内外膝外翻性骨关节炎截骨手术治疗相关文献,对膝关节周围截骨术不同手术方式的优缺点、疗效进行总结。结果对于有症状且年轻、活动度大的膝外翻性骨关节炎患者,膝关节周围截骨术是一种安全、可靠的治疗选择。目前,临床应用的主要手术方式有股骨远端内侧闭合楔形截骨术、股骨远端外侧张开楔形截骨术、胫骨近端内侧闭合楔形截骨术、胫骨近端外侧张开楔形截骨术。不同截骨手术方式的适应证和优缺点不同,选择正确的截骨手术方式对取得良好的临床疗效至关重要。结论膝关节周围截骨术治疗膝外翻性骨关节炎有多种手术方式,为获得良好预后、提高生存率、减少并发症发生,需要根据不同情况制定最合理的手术策略。  相似文献

BackgroundFirst tarsometatarsal arthrodesis (modified Lapidus procedure) constitutes a sufficient treatment for moderate to severe hallux valgus deformity and first ray instability. The plantar plate arthrodesis was shown to provide superior mechanical stability and less postoperative complications than screw fixation or dorsal plating. Nevertheless, the in-brought hardware may cause irritation of the tibialis anterior or peroneus longus tendon requiring explantation of the material in some cases.The purpose of this study was to investigate the potential of tendon irritation after plantar first tarsometatarsal joint arthrodesis in a cadaver study.MethodsPlantar plate arthrodesis was performed as in real surgery on twelve pairs of fresh frozen cadaveric feet. Two different plate systems were randomly allocated to each pair of feet. After plate fixation careful dissection of the feet followed to analyze potential tendon irritation and to determine a “safe zone” for plantar plate placement.ResultsA “safe zone” between the insertion sties of tibialis anterior and peroneus longus tendon was found and proven to be sufficiently exposed using a standard medio-plantar approach. Both plates were fixed in this zone without compromising central tendon parts. Peripheral tendon parts were irritated in 42% using Darco Plantar Lapidus Plating System® (Wright Medical, Memphis, TN) and in 8% using the Plantar Lapidus Plate® (Arthrex, Naples, FL). Bending of the anatomically preshaped plates is often necessary to ensure optimal fit on the bone surface.ConclusionsModified Lapidus procedure with plantar plating of the first tarsometatarsal joint can be performed safely without compromising central tendon parts via standard medio-plantar approach.Level of clinical evidence5, Cadaver Study.  相似文献

The objective of this study was to find the effect of hallux valgus (HV) deformity on the inter-segmental motion of the foot using an MFM with a 15-marker set (DuPont Foot Model, DuFM) in comparison with age and sex controlled healthy adults. Fifty-eight female symptomatic HV patients and 50 female asymptomatic older female volunteers were included in this study. According to the radiographic hallux valgus angle (HVA), the study population was divided into severe HV (SHV, HVA ≥ 40°, n = 25), moderate HV (MHV, 20° ≤ HVA < 40°, n = 47), and control (CON, n = 36). MHV group was divided into symptomatic MHV group (S-MHV, n = 33) and asymptomatic MHV group (A-MHV, n = 14) according to the symptoms associated with HV. For temporal parameters, gait speed and stride length were diminished according to the severity of HV deformity. Sagittal range of motion of hallux and hindfoot decreased significantly in SHV group. Loss of push-off during the preswing phase was observed and forefoot adduction motion during terminal stance was decreased in SHV group. In a subgroup analysis of MHV, asymptomatic HV minimally affects gait and inter-segmental motion during gait. HV deformity affects gait parameters and inter-segmental motion of the foot during gait in proportion to the severity of the deformity. However, the effect of MHV itself on foot kinematics might be limited while pain or arthritic change of the joint might cause changes in gait in patients with symptomatic HV.  相似文献

目的探讨第1跖列三平面截骨联合其余跖骨基底截骨治疗中重度跖内收型[足母]外翻的早期疗效。方法2012年2月-2016年9月,收治10例(12足)中重度跖内收型[足母]外翻患者。男1例(2足),女9例(10足);年龄21~55岁,平均34.5岁。病程3~10年,平均5.8年。根据改良Sgarlato测量法对跖内收分度:中度4足、重度8足。术前美国矫形足踝协会(AOFAS)评分为(46.4±9.3)分;跖内收角(metatarsus adductus angle,MAA)为(25.41±3.66)°,[足母]外翻角(hallux valgus angle,HVA)为(41.42±9.67)°,第1-2跖骨间角(first-second intermetatarsal angle,1-2IMA)为(10.72±2.26)°。采用第1跖列三平面截骨联合其余跖骨基底截骨以及外侧软组织松解术治疗。结果术后切口均Ⅰ期愈合。患者均获随访,随访时间18~24个月,平均21.4个月。1例(1足)术后出现转移性跖痛,对症处理后症状消失。X线片复查示截骨部位均愈合,愈合时间为2.4~3.2个月,平均2.8个月。末次随访时,MAA为(8.42±0.71)°、HVA为(13.29±1.03)°、1-2IMA为(4.41±0.48)°,AOFAS评分为(89.8±5.9)分,均较术前明显改善(P<0.05)。结论第1跖列三平面截骨联合其余跖骨基底截骨治疗中重度跖内收型[足母]外翻,可获得较好早期疗效。  相似文献

We present a radiographic review of 94 patients who underwent first metatarsophalangeal joint arthrodesis. The main focus of our review was to assess the change in the intermetatarsal angle (IMA). The change in the IMA was measured for the entire group and for 2 subgroups (IMA 11° to 15° and IMA >15°). The results of the angular measurements for the total data set were as follows: mean preoperative first IMA, 15.32° (range 11° to 24°), mean postoperative IMA, 9.88° (range 3° to 18°), and mean change in IMA of 5.44° (range −2° to 13°; p < .001). Group 1, with an IMA of 11° to 15°, included 52 patients, with a mean change in the IMA of 4.21° (range −2° to 9°; p < .001). Group 2, with an IMA greater than 15° (range 16° to 24°), included 42 patients, with a mean change in the IMA of 6.83° (range 2° to 13°; p < .001). The change in the preoperative to postoperative IMA in group 1 compared with that in group 2 was statistically significant (p < .001). The results of the present study have confirmed the observations of previous investigators that arthrodesis of the first metatarsophalangeal joint for hallux abducto valgus deformity results in a reduction of the IMA and that a proportionately larger reduction can be expected when the IMA is larger.  相似文献

We observed the changes in the angular measurements commonly used in the evaluation of the first metatarsal and first metatarsophalangeal joint in cadaveric specimens before and after frontal plane rotation of the first metatarsal. Measurements of the first and second intermetatarsal angle (IMA), hallux abductus angle, proximal articular set angle, and tibial sesamoid position (TSP) were taken after varying degrees of varus and valgus rotation of the first metatarsal. Standard dorsoplantar radiographs were taken at 0°, 10°, 20°, and 30° of valgus rotation of the first metatarsal and repeated at 10°, 20°, and 30° varus rotation of the first metatarsal. The data were analyzed using a mixed linear model to compare the change in each angle measurement over the range of valgus and varus rotation. The change in the TSP was significant in both valgus and varus rotations (p = .0004 and p = .028, respectively), an increase in valgus rotation causing an increase in the TSP and an increase in varus rotation causing a decrease in TSP. The change in the IMA was significant compared with valgus rotation (p = .028), showing that as the valgus rotation increased, the IMA also increased. However, compared with the varus rotation, the correlation was not significant (p = .18). The proximal articular set angle and hallux abductus angle measurements, compared with metatarsal rotation, showed positive trends but were not statistically significant. From our results and a review of the published data, we have hypothesized that frontal plane rotation of the first metatarsal is an integral component of hallux abducto valgus pathologic features, specifically in relation to the TSP and IMA.  相似文献

摘要:目的分析胎儿心脏外翻畸形超声诊断的漏诊与误诊原因,提高胎儿心脏外翻的检出时机和检出率。方法:选取北京四季青医院2010年至2019年3000例患者中经引产或上级产前诊断机构证实诊断为心脏外翻畸形的胎儿有4例,每一例心脏外翻畸形的诊断都有产前诊断机构对照;总结超声诊断心脏外翻畸形特点,观察心脏搏动位置,心脏大血管连结方向,以及胸腹壁是否连续性中断,及胸腹壁是否有不均质回声包块,并且能在包块内发现心脏搏动。尤其是上级医院与引产后和产前诊断中心超声结果比较,分析超声发生漏诊和误诊的原因。结果:该组病例中发现心脏外翻畸形4例,其中超声诊断3例, 漏诊1例,误诊1例,其中误诊1例为将Cantrell五联征误诊为胎儿多发畸形,分别独立诊断了心脏外翻畸形,胸裂,腹裂,脊柱结构异常。结论:全面认真分析胎儿心脏外翻畸形超声检查的漏诊与误诊原因,避免出现漏诊和误诊的各种因素,准确把我提高超声对心脏外翻畸形的检出时机,达到提高疾病检出率的目的。  相似文献

目的探讨距足母囊近端2.0 cm处Z型截骨并T型微型钛板内固定治疗中重度足母外翻的临床疗效。方法2009年11月至2013年6月,对50例(81足)中重度足母外翻患者行第一跖骨远端Z型截骨后T型微型钛板内固定治疗。结果术后随访3~36个月,患者足母外翻角(halluxv algus angle,HVA)减少了11°~23°,平均13.1°,跖骨间角(intermetatarsal angle,IMA)减少了5°~10°,平均6.6°。采用美国足踝矫形学会Maryland足功能评分系统评分:90~100分,79足,80~89分2足,优良率100%,未见感染,截骨处均获骨性愈合,畸形无复发。结论距足母囊近端截骨并微型钛板内固定治疗中重度足母外翻疗效可靠,值得推广。  相似文献
