藏区非结防医生结核疑似病人确诊延迟分析   总被引:2,自引:0,他引:2  
目的 探讨四川省藏区非结核病防治机构医生对结核疑似病人确诊延迟的影响。方法 通过问卷收集初次到若尔盖和石渠县结防机构诊治的结核疑似病人的资料,调查首诊为非结防机构医生的结核疑似病人确诊延迟情况。结果 80.0%结核疑似病人首诊选择非结防机构医生,且其中最后被结防机构确诊为活动性肺结核的42例患者中,54.8%发生了确诊延迟;不同类非结防机构医生在各自接诊的病人中,藏医首诊结果怀疑结核病的比例低于个体医生、乡村医生与县级医生等(P〈0.001);首诊选择藏医的结核疑似病人确诊延迟发生率高于选择乡村医生和县级医生等的(P=0.008);未被非结防医生推荐到结防机构就诊者较被推荐者确诊延迟发生率高(P〈0.001)。结论 非结防机构医生的类型、首诊结果及其是否推荐结核疑似病人到结防机构就诊与结核疑似病人的确诊延迟有关;培训非结防机构医生的结核病知识、加强归口管理可减少结核疑似病人的确诊延迟。  相似文献

ObjectiveTo identify delays in the health care system experienced by children and adolescents and young adults (AYA; aged 0–29 years) with osteosarcoma and Ewing sarcoma using information from the Brazilian hospital-based cancer registries.MethodsPatient data were extracted from 161 Brazilian hospital-based cancer registries between 2007 and 2011. Hospital, diagnosis, and treatment delays were analyzed in patients without a previous histopathological diagnosis. Referral, hospital, and health care delays were calculated for patients with a previous histopathological diagnosis. The time interval was measured in days.ResultsThere was no difference between genders in overall delays. All delays increased at older ages. Patients without a previous histopathological diagnosis had the longest hospital delay when compared to patients with a previous histopathological diagnosis before first contact with the cancer center. Patients with Ewing sarcoma had longer referral and health care delays than those with osteosarcoma who had a previous histopathological diagnosis before first contact with the cancer center. The North and Northeast regions had the longest diagnosis delay, while the Northeast and Southeast regions had the longest treatment delay.ConclusionHealth care delay among patients with a previous diagnosis was longer, and was probably associated with the time taken for to referral to cancer centers. Patients without a previous histopathological diagnosis had longer hospital delays, which could be associated with possible difficulties regarding demand and high-cost procedures. Despite limitations, this study helps provide initial knowledge about the healthcare pathway delays for patients with bone cancer inside several Brazilian hospitals.  相似文献

It has been suggested that the brain and in particular the cerebellum and motor cortex adapt to represent the environment during reaching movements under various visuomotor perturbations. It is well known that significant delay is present in neural conductance and processing; however, the possible representation of delay and adaptation to delayed visual feedback has been largely overlooked. Here we investigated the control of reaching movements in human subjects during an imposed visuomotor delay in a virtual reality environment. In the first experiment, when visual feedback was unexpectedly delayed, the hand movement overshot the end‐point target, indicating a vision‐based feedback control. Over the ensuing trials, movements gradually adapted and became accurate. When the delay was removed unexpectedly, movements systematically undershot the target, demonstrating that adaptation occurred within the vision‐based feedback control mechanism. In a second experiment designed to broaden our understanding of the underlying mechanisms, we revealed similar after‐effects for rhythmic reversal (out‐and‐back) movements. We present a computational model accounting for these results based on two adapted forward models, each tuned for a specific modality delay (proprioception or vision), and a third feedforward controller. The computational model, along with the experimental results, refutes delay representation in a pure forward vision‐based predictor and suggests that adaptation occurred in the forward vision‐based predictor, and concurrently in the state‐based feedforward controller. Understanding how the brain compensates for conductance and processing delays is essential for understanding certain impairments concerning these neural delays as well as for the development of brain–machine interfaces.  相似文献

Loss-of-function due to expansion of a CGG repeat located in the 5''UTR of the FMR1 gene is the most frequent cause of fragile X syndrome. Less than 1% of individuals with fragile X syndrome have been reported to have a partial or full deletion or point mutation of the FMR1 gene. However, whether a copy number gain of the FMR1 gene could result in certain clinical phenotypes has not been fully investigated. Here, we report the case of a child who presented with developmental delay starting at 9 months of age, fine motor and speech delay, progressive seizures since 18 months of age and hyperactivity. Molecular workup identified a de novo microduplication in the Xq27.3 region, including the FMR1 gene and the ASFMR1 gene. The expression level of the FMR1 gene in peripheral blood did not differ from that of the controls. In addition, an inherited 363-kb duplication on the chromosome 1q44 region and an inherited deletion of 168 kb on the chromosome 4p15.31 region were detected. It is not clear whether these inherited copy number variations (CNVs) also have a modifying role in the clinical phenotype of this patient.  相似文献

Chemical, biological, radiological and nuclear (CBRN) hazards may be encountered during any major incident. General considerations include modifications to triage, managing contaminated or contagious casualties, and the identification and appropriate management of intoxicated/infected/irradiated/injured casualties. In dealing with chemical incidents, characteristics such as toxicity, latency and persistency need to be understood in order to manage casualties appropriately in terms of triage category, life-saving interventions and assessment of contamination risk to responders. Biological agents can be differentiated into live agents (bacteria, viruses and fungi) and toxins. Live agent characteristics and management depend on pathogenicity, virulence, lethality, infectivity and transmissibility, whereas toxins are treated similarly to chemical agents. Radiological and nuclear hazards are managed similarly and may cause irradiation, contamination (external and internal) or a combination with or without trauma. A generic and structured approach is advised to deal with all major incidents including those with a suspected of confirmed CBRN hazard. All healthcare professionals that may be involved in the response to such an incident need to be familiar with the principles of CBRN incident management and of CBRN casualty management as described in this article.  相似文献

The disturbance caused by various short propagation delays to the perception of external sounds and own voice for a non-occluding hearing aid was investigated. Ten normal-hearing and 10 mildly hearing-impaired individuals listened to external sounds and their own voice while wearing non-occluding devices providing 10dB of linear gain. Participants rated the disturbance caused by delays of 2, 4 and 10ms to music, running speech, and their own voices. The results indicated greater disturbance for the longest delay for both subject groups when judging own voice, with the ratings of the hearing-impaired participants being lowest. Normal-hearing participants also judged the 10-ms delay as more disturbing for the external sounds. Owing to the listening conditions with constant gain from 800Hz and above, the results apply directly only to this experiment. Disturbance ratings for all delays were low, which suggests that any of those tested would be acceptable for this application.  相似文献

The purpose of this study was to measure the effects of a seven-week therapeutic horseback riding program and to determine if changes were retained after therapeutic riding was discontinued. A repeated-measures within-participants design was used to assess performance on the Gross Motor Function Measure and timed 10-meter walk in seven developmentally delayed children. A statistically significant improvement in gross motor function was found in post-intervention measures. Improvements were maintained seven weeks after therapeutic riding had ended. No considerable difference in gait speed was noted. This study indicates that therapeutic riding may lead to improvement in gross motor function in developmentally delayed children and that these improvements remain once therapeutic riding ceases.  相似文献

目的了解护士职业延迟满足的现状,探讨护士职业延迟满足能力的影响因素,为护理管理提供科学有效的管理依据。方法用国内通用职业延迟满足量表对225名综合医院在职护士进行调查,采用SPSS并进行差异性的相关分析。结果护士的年龄和护龄的职业延迟满足过程存在显著性差异(F=3.08,3.93;P<0.05),且随着年龄和护龄的增加而递减;受教育程度高低的职业延迟特质有显著性差异(F=2.93,P<0.05),且受教育程度越高,职业延迟特质就越高;婚否的延迟满足总分也存在显著性差异(t=-2.6,P<0.05),而独身子女和户口性质对职业延迟总分没有统计学影响(t=-0.54,-0.08;P>0.05)。结论①随着年龄的增大、工作年限的延长,护士的延迟满足能力会降低;②低学历护士的延迟满足能力比高学历的低,应给予更多的关注;③未婚护士的职业延迟满足能力优于已婚护士。  相似文献
