We investigated the relationship between agricultural pesticides and colorectal cancer incidence in the Agricultural Health Study. A total of 56,813 pesticide applicators with no prior history of colorectal cancer were included in this analysis. Detailed pesticide exposure and other information were obtained from self-administered questionnaires completed at the time of enrollment (1993-1997). Cancer incidence was determined through population-based cancer registries from enrollment through December 31, 2002. A total of 305 incident colorectal cancers (212 colon, 93 rectum) were diagnosed during the study period, 1993-2002. Although most of the 50 pesticides studied were not associated with colorectal cancer risk, chlorpyrifos use showed significant exposure response trend (p for trend = 0.008) for rectal cancer, rising to a 2.7-fold (95% confidence interval: 1.2-6.4) increased risk in the highest exposure category. Aldicarb was associated with a significantly increased risk of colon cancer (p for trend = 0.001), based on a small number of exposed cases, with the highest exposure category resulting in a 4.1-fold increased risk (95% confidence interval: 1.3-12.8). In contrast, dichlorophenoxyacetic acid showed a significant inverse association with colon cancer but the association was not monotonic. Our findings should be interpreted cautiously since the literature suggesting that pesticides are related to colorectal cancer is limited. Nonetheless the possibility of an association between exposure to certain pesticides and incidence of colorectal cancer among pesticide applicators deserves further evaluation.  相似文献

Cancer incidence among Finnish farmers, 1979-93   总被引:2,自引:0,他引:2  
A cohort of approximately 120,000 male and 85,000 female farmers registered in the Farm Register of Finland was followed-up for cancer incidence through the Finnish Cancer Registry in 1979-93. Nearly 17,000 cancer cases were observed. Compared with the total cancer incidence of the Finnish population, both genders experienced a 17 to 18 percent decreased risk of cancer. The risks were low in smoking-related cancers, cancers associated with an urbanized way of life, and cancers inversely associated with multiparity. The only cancer sites showing significantly raised standardized incidence ratios (SIR) were lip cancer (SIR=1.51, 95 percent confidence interval [CI]=1.35-1.66) and Hodgkin's disease (SIR=1.35, CI=1.08-1.67) in males. Poisson regression was used in working out the roles of the size and type of farm after adjustment for gender, age, and geographic location of the farm. Risk of Hodgkin's disease was highest in farms without animals (SIR=1.74, CI=1.12-2.59). Multiple myeloma was found in excess among farmers on pig or poultry farms but not on other types of farms.  相似文献

BACKGROUND: Farmers are exposed to a wide variety of sensitizers. Since occupational asthma (OA) can lead to permanent disability, exposure discontinuation is the preferred treatment. When this is not possible, the identification of the causative allergen may allow an alternative therapy. METHODS: We present three farmers diagnosed with OA as a consequence of handling fodder. We carried out skin tests with common and occupational allergens and with oilseed rape (OSR) extract. Total and specific serum IgE levels were measured. The patients underwent the OSR-bronchial provocation test (OSR-BPT). The day before and 24 h after the OSR-BPT, the methacholine (M)-BPT and induced sputum were performed. Eosinophil percentages and ECP levels were measured in the sputum samples. RESULTS: OSR sensitization (skin tests and specific serum IgE) was detected in all the patients. The OSR-BPT elicited early responses in two subjects. Methacholine sensitivity, sputum eosinophils, and sputum ECP levels increased 24 h after the OSR-BPT in all the patients. CONCLUSIONS: We have demonstrated that inhalation of OSR flour causes bronchoconstriction, induces an eosinophilic inflammatory bronchial response, and increases bronchial hyperresponsiveness in sensitized asthmatics. OSR flour contained in animal fodder should be considered another potential cause of OA among farmers.  相似文献

BACKGROUND: In Poland small, nonspecialized farms, growing different crops and raising usually a few animals of various species constitute the majority. OBJECTIVE: The aim of the case-control study was to evaluate the risk factors of work-related respiratory symptoms and occupational asthma and/or rhinitis in Polish farmers and investigate whether the farming characteristics may have influence on the prevalence of atopy and allergic diseases. METHODS: The study groups comprised 100 cases who were farmers reporting work-related asthmatic and/or rhinitis symptoms from randomly selected family doctors practices (80 of 353) and 102 healthy controls. All subjects were examined by means of a questionnaire, skin prick tests (SPT) to common and occupational allergens, total serum immunoglobulin (Ig)E level and the presence of specific IgE and allergen-specific inhalation challenge tests were performed. The provocation tests were monitored with the spirometry, histamine challenge test and evaluation of nasal symptoms score and nasal washings. RESULTS: Respiratory allergic disease was recognized in 68 symptomatic patients, including 41 cases because of occupational allergens (asthma n=38, rhinitis n=41). Step-wise logistic regression analysis confirmed the protective role of small farms against the development of work-related symptoms (OR 0.23; 95% CI 0.11, 0.47) as well as the significance of positive SPT to cereals (OR 5.55; 95% CI 1.6, 19.21) and storage mites (OR 3.73; 95% CI 1.27, 10.96) as a risk factor of these symptoms. Cereal farming (OR 13.75; 95% CI 2.39, 78.83) and positive SPT to cereals (OR 26.92; 95% CI 5.33, 135.9) and storage mites (OR 44.07; 95% CI 8.40, 231.1) were found to be significant risk factors of occupational asthma and/or rhinitis. CONCLUSIONS: Cereal farming and hypersensitivity to cereals plays the significant role in the development of occupational asthma among Polish farmers. It also seems that working on small farms may protect farmers against work-related respiratory symptoms that are mostly because of allergy.  相似文献

Pig farming is a risk factor for increased nasal Staphylococcus aureus colonization. Using sequence typing and phylogenetic comparisons, we showed that overcolonization of farmers was caused by a few bacterial strains that were not present in nonfarmers but often caused swine infections. BACKGROUND: The study was undertaken to assess the prevalence and risk factors of self-reported asthma, symptoms of chronic bronchitis, hay fever, and work-related respiratory symptoms in Swiss farmers as well as to compare the prevalence rates of respiratory symptoms with the Swiss population (SAPALDIA-Study). METHODS: An epidemiological study was performed with a representative sample of 1,542 Swiss farmers using a self-administered questionnaire. To investigate the effect of the type of farming on reported symptoms, the farmers were subdivided into seven groups according to the time farmers spent in different animal confinement buildings. A multivariate analysis was performed by the methods of binary and multivariate logistic regression adjusted for age and smoking habits. RESULTS: In farmers the prevalence rate was 16.0% for chronic bronchitis, 15.4% for asthma symptoms, and 42.0% for reporting at least one work-related symptom. Using logistic regression analysis, it was established that poultry farming and pig/cattle farming was a risk factor for reporting nasal irritation at work [OR 5.33, (1.57-18.0), OR 3.37 (1.04-10.87)]. Poultry farmers experienced the highest symptom rates. In crop farmers, the prevalence for chronic bronchitis was increased [OR 2.32 (1.03-5.23)]. Over 4 hr spent per day in animal confinement buildings more than doubled the risk for reporting chronic bronchitis [OR 2.61 (1.01-6.76)] and phlegm [OR 2.3 (0.99-5.4)] independent of the type of farming. The comparison of Swiss farmers with the Swiss population showed a twofold elevated risk of reporting chronic bronchitis [OR 1.89 (1.32-2.95)] and a 4.5-fold elevated risk for bringing up phlegm regularly [OR 4.5 (3.25-6.69)] in farmers. In contrast, the risk of farmers to report nasal allergies was less than half as high [OR 0.40 (0.29-0.56)] as that of the general population. CONCLUSIONS: This study shows that agricultural work in Switzerland is associated with an elevated risk for reporting symptoms of chronic bronchitis and chronic phlegm compared with the general Swiss population. These main results most likely indicate occupational disorders as the exposure-response relationship (hours spent in animal confinements) was particularly obvious for these symptoms.  相似文献

Background

Over the past years there has been an increase in the use of pesticides in developing countries. This study describes pesticide use among small-scale farmers in Uganda and analyses predictors of pesticide poisoning (intoxication) symptoms.

Method

A cross-sectional study was conducted using a standardized questionnaire. Some 317 small-scale farmers in two districts in Uganda were interviewed about pesticide use, knowledge and attitude, symptoms of intoxication, personal protective equipment (PPE) and hygiene. The risk of reporting symptoms was analysed using logistic regression analysis.

Results

The most frequently used pesticides belonged to WHO class II. The farmers had poor knowledge about pesticide toxicity, and the majority did not use appropriate PPE nor good hygiene when handling pesticides. There was no significant association between the number of times of spraying with pesticides and self-reported symptoms of pesticide poisoning. The only significant association was between blowing and sucking the nozzle of the knapsack sprayer and self-reported symptoms of pesticide intoxication (OR: 2.13. 95% CI: 1.09 – 4.18).

Conclusion

Unlike the practice in several other developing countries, small-scale farmers in Uganda do not use the most hazardous pesticides (WHO class 1a and 1b). However use of WHO class II pesticides and those of lower toxicity is seen in combination with inadequate knowledge and practice among the farmers. This poses a danger of acute intoxications, chronic health problems and environmental pollution. Training of farmers in Integrated Pest Management (IPM) methods, use of proper hygiene and personal protective equipment when handling pesticides should be promoted.  相似文献

Hantaviruses are an established cause of haemorrhagic fever with renal syndrome (HFRS) in Europe. Following a confirmed case of HFRS in the UK, in an individual residing on a farm in North Yorkshire and the Humber, a tidal estuary on the east coast of Northern England, and the subsequent isolation of a Seoul hantavirus from rats trapped on the patient’s farm, it was considered appropriate to further investigate the public health risk of this virus in the region. Of a total 119 individuals tested, nine (7.6%) were seropositive for hantavirus antibodies. Seven of the seropositive samples showed a stronger reaction to Seoul and Hantaan compared to other clinically relevant hantaviruses. Observation of rodents during the day, in particular mice, was associated with a reduced risk of seropositivity. In addition to one region known to be at risk following an acute case, five further potential risk areas have been identified. This study supports recently published evidence that hantaviruses are likely to be of public health interest in the region.  相似文献

目的测定1%氟虫胺杀蟑胶饵对德国小蠊的实验室和在农贸市场现场的防治效果。方法按照GB/T13917.7—2009和GB/T13917.10—2009进行实验室室内药效和模拟现场药效试验;现场药效试验采用GB/T23795—2009中的粘捕法进行密度监测。结果1%氟虫胺杀蟑胶饵实验室内对德国小蠊的LT50是1.18d,72h死亡率为100%;模拟现场对德国小蠊的LT50为1.12d,72h死亡率为100%;施药后第3天4个农贸市场德国小蠊密度明显下降,密度平均下降率为63.3%,连续补充施药后第15天的下降率分别为95.9%~97.2%。结论1%氟虫胺杀蟑胶饵对德国小蠊在实验室和农贸市场均具有较好的防治效果;它的良好连锁杀灭效果,使其单独使用时能有效地控制各种复杂环境的德国小蠊。  相似文献
