Tendon healing is a complex process consisting of a large number of intricate pathways roughly divided into the phases of inflammation, proliferation, and remodeling. Although these processes have been extensively studied at a variety of levels in recent years, there is still much that remains unknown. This study used microarray analyses to investigate the process at a genetic level in healing rat Achilles tendon at 1, 7, and 21 days postinjury, roughly representing the inflammation, proliferation, and remodeling phases. An interesting temporal expression profile was demonstrated, identifying both known and novel genes and pathways involved in the progression of tendon healing. Both inflammatory response and pro-proliferative genes were shown to be significantly upregulated from 24 h postinjury through to 21 days. Day 7 showed the largest increase in genetic activity, particularly with the expression of collagens and other extracellular matrix genes. Interestingly, there was also evidence of central nervous system-like glutamate-based signaling machinery present in tendon cells, as has recently been shown in bone. This type of signaling mechanism has not previously been shown to exist in tendon. Another novel finding from these analyses is that there appears to be several genes upregulated during healing which have exclusively or primarily been characterized as key modulators of proliferation and patterning during embryonic development. This may suggest that similar pathways are employed in wound healing as in the tightly regulated progression of growth and development in the embryo. These results could be of use in designing novel gene-based therapies to increase the efficacy and efficiency of tendon healing.  相似文献

目的:研究复方胰岛素凝胶剂对大鼠皮肤溃疡的促愈合作用,并探讨其可能的作用机制。方法:18只Wistar大鼠背部各制备4个皮肤溃疡模型,共复制溃疡72个,随机将溃疡分为复方胰岛素凝胶治疗组(A组)、凝胶基质对照组(B组)、空白对照组(C组)。以治疗后不同时间点各组溃疡面积、溃疡愈合速度及组织病理学检查评价修复效果。结果:在伤后头10天内,A组溃疡缩小最明显,愈合速度明显快于B组、C组。伤后14天,A组溃疡基本愈合,皮肤结构完整、上皮化较厚;而B、C组溃疡仍未完全愈合,皮肤结构不完整、上皮化较薄。结论:局部应用复方胰岛素凝胶剂对大鼠皮肤溃疡有促愈合作用。  相似文献

Nonunion is a challenging problem that may occur following certain bone fractures. However, there has been little investigation of the molecular basis of nonunions. Bone morphogenetic proteins (BMPs) play a significant role in osteogenesis. However, little is known about the expression patterns of BMPs in abnormal bone healing that results in nonunion formation. These facts prompted us to investigate and compare the gene expression patterns of BMPs and their antagonists in standard healing fractures and nonunions using rat experimental models. Standard closed healing fractures and experimental atrophic nonunions produced by periosteal cauterization at the fracture site were created in rat femurs. At postfracture days 3, 7, 10, 14, 21, and 28, total RNA was extracted from the callus of standard healing fracture and fibrous tissue of nonunion (n=4 per each time point and each group). Gene expression of BMPs, BMP antagonists, and other regulatory molecules were studied by methods including Genechip microarray and real-time quantitative RT-PCR. Gene expression of BMP-2, 3, 3B, 4, 6, 7, GDF-5, 7, and BMP antagonists noggin, drm, screlostin, and BAMBI were significantly lower in nonunions compared to standard healing fractures at several time points. Downregulation in expression of osteogenic BMPs may account for the nonunions of fracture. The balance between BMPs and their endogenous antagonists is critical for optimal fracture healing.  相似文献

目的 观察不同种属脱细胞真皮基质 (acellulardermalmatrix,ADM)与自体皮复合移植的效果 ,为异种ADM的临床应用提供理论依据。 方法 本地产白色小猪 6头 ,分为异种 (人 )ADM +自体刃厚皮组 (A组 )、同种异体 (猪 )ADM +自体刃厚皮组 (B组 )、单纯自体刃厚皮组 (C组 )及单纯自体中厚皮组 (D组 )。观察术后 2、4、8、12、2 4周内移植物存活率 ,以及移植皮片收缩程度、移植区组织学变化等情况。 结果 A、B组移植后皮片外观光滑、有弹性 ;两组均获得了满意的皮片成活率 ,并可迅速诱导成纤维细胞、血管内皮细胞等宿主修复细胞的长入 ;两组移植皮片收缩面积有大于C、D组的趋势 (P <0.0 5);术后 2 4周移植区组织与单纯中厚皮移植组织结构一致。 结论 在观察期内(复合移植后 2 4周 ) ,与自体皮复合移植时 ,同、异种ADM具有相近的生物学作用 ,异种来源的ADM可能具有更广阔的应用前景  相似文献

目的了解急性重度眼烧伤的临床特点、预后及并发症发生情况。方法对笔者单位病案室保存的1977--2002年住院眼烧伤患者135例(155只眼)的病历进行回顾性统计分析。分析不同深度、原因眼烧伤的眼表修复率、修复时间、致盲情况及并发症。结果治疗后Ⅲ度眼烧伤的修复时间短于Ⅳ度眼烧伤,其修复率也明显高于Ⅳ度眼烧伤(P〈0.01);在不能修复的Ⅳ度眼烧伤中,酸烧伤致角膜穿孔6只眼,显著高于碱烧伤(1只眼,P〈0.05)。Ⅲ度眼烧伤视力≥0.05者有38只眼,〈0.05者51只眼;Ⅳ度眼烧伤视力≥0.05者0只眼,〈0.05着66只眼。78只酸、碱烧伤眼中,52只眼致盲,致盲率为66.7%;77只热烧伤眼中,65只眼致盲,致盲率为84.4%。Ⅲ度眼烧伤的并发症发生率低于Ⅳ度眼烧伤。其中Ⅲ度眼烧伤角膜穿孔、睑球粘连的发生率(1.1%、36.O%)均显著低于Ⅳ度眼烧伤(27.3%、59.1%,P〈0.01)。在酸烧伤患者中,角膜穿孔及睑外翻的发生率(29.2%、37.5%)较高。而热烧伤患者中眼睑球粘连的发生率(53.2%)较高。结论Ⅳ度眼烧伤伤后的预后差,致盲率高。目前的治疗方法尚不能完全改善其视功能及预后,有待进一步研究。  相似文献

目的:探讨感觉神经肽P物质(SP)对离体培养的肉芽组织成纤维细胞的促增殖作用及其对表皮生长因子(EGF)基因表达的调控作用。方法:采用MTT法测定SP对肉芽组织成纤维细胞的促增殖作用;采用逆转录-聚合酶链反应(RT-PCR)方法检测SP对成纤维细胞EGF基因表达的调控作用,观察时间及剂量-效应关系。结果:10^-9~10^-5mol/L的SP在体外对肉芽组织成纤维细胞均具有明显的促增殖作用(P<0.01),且具有明显的剂量依赖性(γ=0.594,P<0.01),EGF抗体只能部分抑制这一作用(与对照组比较,P<0.01;与10^-7mol/L SP组比较,P<0.05)。10^-7mol/L SP可诱导成纤维细胞EGF mRNA的表达,在作用后6h与对照组比较,差异有非常显著性意义(P<0.01);SP在10^-8~10^-6mol/L范围内可以显著促进成纤维细胞EGF mRNA表达,在10^-7mol/L达到峰值,当浓度>10^-7mol/L时,其促EGF mRNA表达的效应强度随浓度升高而有所降低,至10^-5mol/L时与对照组比较,差异无显著性意义。结论:SP对肉芽组织成纤维细胞具有明显的促增殖作用,这种作用与其诱导成纤维细胞EGF表达有关。  相似文献

目的 探讨重组人表皮生长因子(rhEGF)对促进Ⅱ度烧伤创面愈合的作用。方法 采用自身对照研究方法,选取46例Ⅱ度烧伤患者观察最适治疗浓度创面局部应用的治疗效果及不良反应。结果 在浅Ⅱ度烧伤创面,观察组的愈合时间为(8.7±2.1)天,对照组为(12.2±2.4)天,而深Ⅱ度烧伤创面观察组的愈合时间是(13.4±2.6)天,对照组为(19.2±3.2)天。与对照组相比,观察组创面愈合时间均有明显缩短(P<0.01),结论 rhEGF对Ⅱ度烧伤创面均可有效地促进组织修复,加快创面愈合速度,有良好的促进愈合作用,而药物反应极轻微。  相似文献

A consecutive series of 509 patients undergoing abdominal surgery were entered into a randomized, observer and patient blind, controlled, prospective, study to evaluate the efficiency of co-amoxiclav (‘Augmentin’, SmithKline Beecham, UK) compared with cefuroxime (‘Zinacef’ Glaxo, UK) plus metronidazole (Flagyl, M&B, UK) for the prevention of postoperative wound infections. One or three doses of antibiotics were given depending on the type of surgery and operative factors. Co-amoxiclav was given to 230 patients with a total wound infection rate of 5·6% and cefuroxime plus metronidazole were given to 225 patients with a total wound infection rate of 3%. The difference between infection rates was not significant. Both groups were comparable in terms of demographic details, type and duration of surgery, risk factors associated with surgical procedures and postoperative management. Although not statistically significant, a difference in the wound infection rate for those patients undergoing colorectal surgery was seen: for the co-amoxiclav group and for the cefuroxime/ metronidazole group. The estimated cost to our hospital (October 1993) of one dose of co-amoxiclav was less that half the cost of cefuroxime and metronidazole. This study demonstrates that co-amoxiclav is an effective prophylactic antibiotic for abdominal surgery.  相似文献

Diethyl maleate (DEM) which binds and thus depletes tissue glutathione levels was used to aggravate the injury and to determine its effect on incisional healing. A 5 cm dorsal midline skin incision was performed on 40 albino Wistar rats in two groups and then closed by interrupted sutures. Groups received 0.9% NaCl and DEM at a dosage of 1 mg/kg/day intraperitoneally for seven days, respectively. On postoperative days 7 and 14, histopathological assessment and tensile strengths were measured. The DEM treated group had a marked inflammation with poorly defined collagen formation and the tensile strength measurements revealed a significant decrease (p <0.001) on the 7t day. On the other hand, the first group showed better collagenization and a lesser degree of inflammation. However, on the 14th day, there was no noticeable histopathological difference between the two groups; but, tensile strength values of the second group were still lower (p <0.05). In this animal model, DEM postponed the healing process and reduced the tensile strength.  相似文献
