The accuracy of 2D phase contrast (PC) magnetic resonance angiography (MRA) depends on the alignment between the vessels and
the imaging plane. PC MRA imaging of blood flow is challenging when the flow in several vessels is to be evaluated with one
acquisition. For this purpose, semi-automatic determination of the plane most perpendicular to several vessels is proposed
based on centerlines extracted from 3D MRA. Arterial centerlines are extracted from 3D MRA based on iterative estimation-prediction,
multi-scale analysis of image moments, and a second-order shape model. The optimal plane is determined by minimizing misalignment
between its normal vector and the centerlines’ tangent vectors. The method was evaluated on a phantom and on 35 patients,
by seeking the optimal plane for cerebral blood flow quantification simultaneously in internal carotids and vertebral arteries.
In the phantom, difference of orientation and of height between known and calculated planes was 1.2° and 2.5 mm, respectively.
In the patients, all but one centerline were correctly extracted and the misalignment of the plane was within 12° per artery.
Semi-automatic centerline extraction simplifies and automates determination of the plane orthogonal to one vessel, thereby
permitting automatic simultaneous minimization of the misalignment with several vessels in PC MRA. 相似文献

The aim of this study was to evaluate whether a prevalent vertebral deformity predicts mortality and fractures in both men and women. In the city of Malmö, 598 individuals (298 men, 300 women; age 50–80 years) were selected from the city's population and were included in the Swedish part of the European Vertebral Osteoporosis Study (EVOS). At baseline the participants answered a questionnaire and lateral spine radiographs were performed. The prevalence of subjects with vertebral deformity was assessed using a morphometric method. The mortality during a 10-year follow-up period was determined through the register of the National Swedish Board of Health and Welfare. Eighty-five men and 43 women died during the study period. The subsequent fracture incidence during the follow-up period was ascertained by postal questionnaires, telephone interviews and by a survey of the archives of the Department of Radiology in the city hospital. Thirty-seven men and 69 women sustained a fracture during the study period. Data are presented as hazard ratios (HR) with 95% confidence interval (95% CI) within brackets. Prevalent vertebral deformity, defined as a reduction by more than 3 standard deviations (SD) in vertebral height ratio, predicted mortality during the forthcoming decade in both men [age-adjusted HR 2.4 (95% CI 1.6–3.9)] and women [age-adjusted HR 2.3 (95% CI 1.3–4.3)]. In men there was an increased mortality due to cardiovascular and pulmonary diseases and in women due to cancer. Prevalent vertebral deformity predicted an increased risk of any fracture during the forthcoming decade in both men [age-adjusted HR 2.7 (95% CI 1.4–5.3)] and women [age-adjusted HR 1.8 (95% CI 1.1–2.9)]. Prevalent vertebral deformity predicted an increased risk of any subsequent fragility fracture in women [age-adjusted HR 2.0 (95% CI 1.1–3.5)]; however, in men the increased risk was nonsignificant [age-adjusted HR 1.9 (95% CI 0.7–5.1)]. In summary, a prevalent vertebral deformity can predict both increased mortality and increased fracture incidence during the following decade in both men and women. Introduction Vertebral deformities are a common consequence of osteoporosis and are known to decrease quality of life. The Qualeffo–41
is a quality-of-life questionnaire especially developed for measuring quality of life in patients with vertebral deformities.
It consists of 41 questions arranged in five domains: pain, physical function, social function, general health perception,
and mental function. The objectives of this study were: (1) to develop a shorter version of the Qualeffo–41 by removing redundant
questions; and (2) to investigate the scale characteristics, reliability, and validity of this shorter version.
Methods The study was performed using data from the Qualeffo validation study and the Multiple Outcomes of Raloxifene Evaluation (MORE)
study. The analyses were performed in patients with vertebral deformities (n=579). Factor analysis on polychoric correlations and an item response theory (IRT) model, i.e., the generalized partial credit
model (GPCM), were used to create a shorter version of Qualeffo–41. Using GPCM, scoring weights were computed for all items.
Results Three items were removed from the data set because of too many missing values. Factor analysis identified three instead of
five domains: (1) pain, (2) physical function, and (3) mental function. Five items had factor loadings <0.4 and were not included
in the GPCM. After excluding several items, the domains pain (four items), physical function (18 items), and mental function
(nine items) showed a good, reasonable, and excellent fit, respectively. This indicates that the mental function domain and
the pain domain are more unidimensional than the physical function domain. All three domains showed a very high correlation
(r ≥0.95) with the corresponding domains of the Qualeffo–41.
Conclusions Qualeffo–31 was developed, consisting of three domains with a reasonable to excellent fit to the GPCM. Although the fit to
the GPCM supports the construct validity of the Qualeffo–31, validation in a new study should be performed before using it

目的探讨颅内出血性椎动脉夹层的治疗方法。方法回顾分析了2000-2005年40例出血性颅内段椎动脉夹层的治疗。其中支架辅助的弹簧圈栓塞33例,夹层段动脉闭塞6例,夹层近端主干动脉闭塞1例。结果手术无死亡病例。术后随访1-46个月,平均16.3个月。发现夹层动脉通畅,无夹层再出血,无新发后循环梗死。1例支架辅助弹簧圈栓塞患者夹层复发,行补充填塞弹簧圈。结论血管内夹层近端闭塞以及夹层段动脉闭塞是治疗颅内出血性椎动脉夹层的经典术式。支架辅助的弹簧圈栓塞治疗保留了载瘤动脉,避免了术后对侧椎动脉血流负荷量的增加;减少了后循环缺血的危险,扩大了血管内治疗出血性椎动脉夹层的手术适应证。本组结果显示支架辅助弹簧圈栓塞术并未增加夹层再通以及穿支动脉闭塞。本组经验初步表明支架辅助的弹簧圈栓塞是治疗出血性颅内椎动脉夹层的合理选择之一。 相似文献

In addition to estrogen widely used all over the world for the prevention of postmenopausal osteoporosis, calcitonin and vitamin
D derivatives are commonly employed to treat established osteoporosis at higher age in Japan. In order to critically assess
the usefulness of vitamin D derivatives and calcitonin alone or in combination on the advancement of vertebral deformity at
higher age, 32 osteoporotic patients with vertebral deformity with the mean age of 79 were randomly divided into 4 groups
with indistinguishable age and severity of the vertebral deformity. Group 1 served as the control without specific medications
for osteoporosis. Group 2 was treated with 10 units elcatonin (eel calcitonin derivative) injected intramuscularly twice a
week. Group 3 was given 0.75 to 1.5μg/day 1α (OH) vitamin D3 orally. Group 4 was given a combination of treatments used in Groups 2 and 3.
In the lateral X-ray film of the spine taken prior to the test and every 6 months thereafter, the shape of the vertebral body
T8 through L4 was monitored by measuring the anterior, central and posterior heights. Decrease of the vertebral height ratio; anterior
or middle height/posterior or adjacent intact posterior height, by more than 20% of the original value or from above to below
0.80 both appeared to be inhibited during administration of 1α (OH) vitamin D3. Such effect seems to be augmented by simultaneous administration of elcatonin. Actual decrease of vertebral height ratio
values and the per cent fall from the original value significantly less in Groups 3 and 4 than in Group 1. Development of
vertebral deformity assessed by the changes of the vertebral height thus appears to decrease during treatment with 1α (OH) vitamin D3 especially together with calcitonin in established osteoporosis. 相似文献

One hundred ninety-eight postmenopausal women (aged 50–65 years) with vertebral bone density (VBD) 1 SD below the mean value
for normal, age-matched, postmenopausal subjects were enrolled in six Italian centers and 134 completed 2 years of treatment.
All subjects were randomly allocated to a 2-year treatment with oral ipriflavone (200 mg t.i.d.) or a matching placebo, according
to a double-blind, parallel group design. All patients also received an oral daily calcium supplement of 1 g as calcium carbonate.
VBD and markers of bone turnover were measured at baseline, and every 6 months. A complete routine analysis of liver and kidney
functions along with hematological parameters were measured before and at the end of treatment period. The valid completers
analysis showed a significant increase of VBD in ipriflavone-treated women with average percent changes of +1.4 after 1 year,
and +1% at the end of treatment period (P < 0.05). The placebo group presented a significant decrease of VBD after 2 years of treatment (P < 0.05). The difference between treatments was significant (P < 0.01). The intention to treat analysis confirmed the significant decrease of VBD in the placebo group, with no changes
in ipriflavone-treated women. Skeletal ALP significantly decreased in ipriflavone-treated women (P < 0.05). Serum BGP and urine HOP/Cr showed a significant decrease only in ipriflavone-treated women, suggesting an inhibitory
effect on bone turnover rate. Adverse reactions, mainly gastrointestinal, occurred to a similar extent in the two treatment
groups. The evaluation of patients' compliance, assessed by residual tablets count, revealed a drug intake of more than 80%
after 2 years in 92.5% and 92.8% of patients treated with ipriflavone or placebo, respectively. This study demonstrates that
ipriflavone can prevent bone loss in postmenopausal women with low bone mass.

The morbidity of osteoporosis is caused by fractures. Vertebral fractures lead to pain and disability and a decrease in quality of life. A Working Party of the European Foundation for Osteoporosis has developed a specific questionnaire for patients with established vertebral osteoporosis. This questionnaire is intended for use in clinical trials. The questionnaire consists of questions and visual analogue scales in the following domains: pain, activities of daily living, jobs around the house, mobility, leisure and social activities, general health perception and mood. The questionnaire has been translated from English into French, German, Italian, Hebrew, Swedish and Dutch. The questionnaire is currently being validated in a multicentre study involving patients with stable osteoporosis and control subjects. Preliminary results indicate that the reproducibility is sufficient and that the questionnaire is able to discriminate between patients with vertebral osteoporosis and control subjects. 相似文献

Bone mineral “density” (BMD) measured by dual-energy X-ray absorptiometry (DEXA) does not represent the volumetric density (grams per cubic centimeter), but rather the areal density (grams per square centimeter). This distinction is important during growth. The purpose of this study was to measure vertebral dimensions in cadavers of young pigtail macaques (Macaca nemestrina), and to derive equations to predict the volumetric bone density from noninvasive measurements. We measured the areal bone density by DEXA, vertebral volume by underwater weighing, mineral content by ashing, dimensions of lumbar vertebrae by calipers, and dimensions of vertebrae by radiography. Somatometric measurements of the female lumbar vertebral bodies showed that the shape changed during growth. The bone mineral content from the densitometer correlated significantly with the ash weight (r = 0.99, error 8.7%). The correlation coefficient between the volumetric bone mineral density and areal BMD measurement was significant (r = 0.68, p < 0.0001) with a 9.5% error; this improved significantly to 0.82 (7.2% error) when the BMD was divided by the vertebral depth from the radiograph. Areal BMD showed a strong correlation with age (r = 0.82, p < 0.0001), with an average increase of 7.4%/year. In contrast, volumetric mineral density showed a weak relationship with age (r = 0.43, p < 0.01), for an average increase of 1.5%/year. When studying bone mineral density during growth, the differences between volumetric and areal bone mineral density should be taken into consideration. ( 相似文献
