手助腹腔镜巨脾和胆囊联合切除术

目的 :总结手助的腹腔镜巨脾切除手术技术。方法 :用手助技术完成腹腔镜巨脾和胆囊联合切除术治疗 1例脾肿大、脾功能亢进伴胆囊结石患者。结果 :4 5h完成手术 ,术中出血较少。患者术后恢复良好。结论 :手助的腹腔镜巨脾和胆囊联合切除术操作安全 ,手术时间短 ,技术上完全可行

犬自体脾块移植后部分脾功能变化的研究

24条本地健康杂种犬随机分成3组:①切脾组;②对照组;③自体脾组织移植组。实验表明犬自体脾块大网膜内移植存活良好,并恢复部分脾功能。切脾后犬周围血嗜中性粒细胞的细菌吞噬率和杀菌功能均低于正常犬,而行脾组织移植后能使之接近正常水平。但移植脾的脾小体中T淋巴细胞数和红髓中单核吞噬细胞数均少于正常脾组织。     

Aussagekraft der präoperativen Thrombozytenmarkierung bei Patienten mit idiopathischer thrombozytopenischer Purpura

Summary ⁵¹Cr-platelet kinetic studies were performed in 77 patients with idiopathic thrombocytopenic purpura. The sequestration site was splenic for 63, splenic/hepatic for 7 or hepatic for remaining 7 patients. In 20 patients platelet survival was extremely shortened to 0–3 h, whereas only 26 patients had a survival time of more than 24 h. Those patients with low platelet counts also had a very short platelet survival time, whereas patients with higher platelet counts (>50×10⁹/l) had longer platelet survival times. 51 patients (66%) were splenectomized following the kinetic studies. 25 patients who had a splenic sequestration site had normalized platelet counts and 6 patients had platelet counts between 80–149×10⁹/l 12 months after splenectomy (i.e. in 92% of cases with splenic sequestration site a full or partial remission). Of the 11 patients with a hepatic or splenic/hepatic sequestration site, 2 patients had full remission, 1 partial remission, 3 patients had minimal improvement and 5 other patients were treatment failures in respect to the splenectomy.

脾切除术治疗Wilson病合并脾亢患者的血液学改变及临床疗效

目的探索Wilson病（WD）合并脾功能亢进患者血液肝功能等指标与其脾脏大小相关性,及脾脏切除的临床疗效。方法回顾性分析2018年1月至2021年12月在安徽中医药大学神经病学研究所附属医院住院的63例行脾切除术的WD患者的临床资料,并收集行脾切除术前、后的病例资料,采用Spearman相关性分析脾切除前后血液肝功能等指标与脾脏大小相关性。结果采用Spearman相关性分析法得出脾脏厚度与PLT(r=-0.54,P<0.001)呈负相关,与PT(r=0.28,P=0.026)、SOD(r=0.28,P=0.028)、TBA(r=0.30, P=0.017)、TBIL(r=0.36, P=0.004)呈正相关。脾脏长度与PLT(r=-0.37,P=0.003)呈负相关、与TBA(r=0.28,P=0.025)呈正相关。WD患者脾切除后SOD、ALB、PLT水平较术前升高,PT以及TBIL水平较术前下降,差异有统计学意义（P<0.05）。结论脾切除术对WD合并脾亢患者的PLT下降具有改善作用,可部分改善肝脏的解毒、代谢、合成等功能指标,并对改善凝血功能具有积极影响。     

Splenic abscess following blunt abdominal trauma

A rare case of splenic abscess occurring after blunt abdominal trauma in a previously healthy boy is reported. The diagnosis was made by ultrasonography. The patient recovered after splenectomy and drainage of subphrenic and intraperitoneal pus.     

脾切除对小鼠腹腔巨噬细胞数量和功能的影响

目的：研究脾切除对小鼠腹腔巨噬细胞数量和功能活性的影响。方法：术后4周获取腹腔常居巨噬细胞和炎症巨噬细胞,检测其数量、吞噬功能和一氧化氨水平。结果：与正常对照组和假手术组比较,脾切除组小鼠的2种巨噬细胞不仅数量明显减少,而且它们的吞噬功能和一氧化氮水平呈明显下降。结论：提示脾切除造成的单核吞噬细胞继发性炎症反应的捐伤和巨噬细胞功能活性的降低,可能是脾切除后(凶险性)感染的重要因素。     

医源性脾损伤28例诊治分析

目的 分析医源性脾损伤的原因及有关的治疗手段。方法 回顾分析28例脾损伤和有关文献报道的病例。结果 脾损伤与胃十二指肠溃疡、胃癌、食管癌、肝肿瘤手术等有关；心肺复苏和纤维结肠镜检查也可致脾损伤；自发性脾破裂可发生在白血病等疾病的发病过程中。结论 根据损伤原因及损伤分级不同，采取不同的治疗方式。可提高保脾手术成功率。     

HELLP syndrome in a pregnant patient with a past history of splenectomy for idiopathic thrombocytopenic purpura. Case report

Case: We present a case of HELLP syndrome occuring in a patient who at the age of 8 years had undergone a splenectomy for idiopathic thrombocytopenic purpura. She was diagnosed as having HELLP syndrome 37 weeks of gestation and had a cesarean section. Received: 4 June 1996 / Accepted: 7 October 1996     

脾动脉部分栓塞术治疗难治性特发性血小板减少性紫癜疗效观察及护理

目的:探讨脾动脉部分栓塞治疗难治性特发性血小板减少性紫癜的临床效果和护理措施.方法:将29例患者随机分为实验组(16例)和对照组(13例),对照组采用常规的脾切除治疗,实验组改为采用脾部分栓塞治疗,对比疗效.结果:脾栓塞组与脾切除组的近期、远期疗效比较无显著性差异(P＞0.05).结论:脾部分栓塞治疗不仅达到了与脾切除相似的疗效,还能保留脾脏部分免疫功能;且手术创伤小,安全性好,采取适当的护理后无严重并发症的发生,患者更易接受.
