Background and purpose Even small design variables of the femoral stem may influence the outcome of a hip arthroplasty. We investigated whether design-related factors play any role in the risk of non-aseptic revision of the 3 most frequently used primary cemented stem designs in the Swedish Hip Arthroplasty Register.

Patients and methods We studied 71,184 primary cemented femoral stem implants (21,008 Exeter polished stems, 43,036 Lubinus SPII stems, and 7,140 Spectron EF Primary stems) that were inserted from 1999 through 2006. Design-specific characteristics were analyzed using separate Cox regression models that were adjusted for sex, age, diagnosis, incision, and number of operations (first vs. second).

Results The crude revision rate varied between 0.8% (Lubinus SPII) and 1.4% (Spectron Primary). For the Exeter stem, the smallest femoral head diameter (22 mm) was associated with a higher risk of revision. No other design-specific parameters influenced the risk of revision of the Exeter stem. The smallest Lubinus stem size, a stem with extended neck length combined with a femoral head with increasing neck length, or the use of a cobalt-chromium head had a negative influence on the outcome. For the Spectron stem, the risk of revision was elevated for the smallest stem and for increasing offset calculated as the combined effect of high offset design and increasing neck length.

Interpretation Overall revision rates were low, but for two of the stems studied design factors such as size and neck length or offset influenced the risk of non-aseptic revision.  相似文献

BackgroundOrthostatic hypotension (OH) may negatively affect physical functioning and aggravate morbidities, but existing evidence is contradictory.MethodsMEDLINE (from 1946), PubMed (from 1966) and EMBASE databases (from 1947) were systematically searched for studies on the association of OH and physical functioning in older adults, categorized as: balance, gait characteristics, walking speed, Timed Up and Go time, handgrip strength (HGS), physical frailty, exercise tolerance, physical activity, activities of daily living (ADL), and performance on the Hoehn and Yahr scale (HY) and Unified Parkinson’s Disease Rating Scale (UPDRS). Study quality was assessed using the Newcastle Ottawa Scale.ResultsForty-two studies were included in the systematic review (29,421 individuals) and 29 studies in the meta-analyses (23,879 individuals). Sixteen out of 42 studies reported a significant association of OH with worse physical functioning. Meta-analysis showed a significant association of OH with impaired balance, ADL performance and HY/UPDRS III performance, but not with gait characteristics, mobility, walking speed, TUG, HGS, physical frailty, exercise tolerance, physical activity and UPDRS II performance.ConclusionsOH was associated with impaired balance, ADL performance and HY/UPDRS III performance, but not with other physical functioning categories. The results suggest that OH interventions could potentially improve some aspects of physical functioning.  相似文献

This study describes the development of a new portable muscle testing device, using air pressure as a biofeedback and strength testing tool. For this purpose, a pressure air biofeedback device (PAB®) was developed to measure and record the isometric extension strength of the lumbar multifidus muscle in asymptomatic and low back pain (LBP) persons. A total of 42 subjects (age 47.58 years, ±18.58) participated in this study. The validity of PAB® was assessed by comparing a selected measure, air pressure force in millibar (mb), to a standard criterion; calibrated weights in kilograms (kg) during day-to-day tests. Furthermore, clinical trial-to-trial and day-to-day tests of maximum voluntary isometric contraction (MVIC) of L5 lumbar multifidus were done to compare air pressure force (mb) to electromyography (EMG) in microvolt (μV) and to measure the reliability of PAB®. A highly significant relationship were found between air pressure output (mb) and calibrated weights (kg). In addition, Pearson correlation calculations showed a significant relationship between PAB® force (mb) and EMG activity (μV) for all subjects (n?=?42) examined, as well as for the asymptomatic group (n?=?24). No relationship was detected for the LBP group (n?=?18). In terms of lumbar extension strength, we found that asymptomatic subjects were significantly stronger than LBP subjects. The results of the PAB® test differentiated between LBP and asymptomatic subject’s lumbar isometric extension strength without any risk to the subjects and also indicate that the lumbar isometric extension test with the new PAB® device is reliable and valid.  相似文献

The benefits of aerobic moderate-to-vigorous physical activity (MVPA) on major non-communicable diseases (NCDs) are well established. However, much less is known whether muscle-strengthening activities (i.e., resistance/weight/strength training) confer similar benefits. Herein, we conducted a narrative literature review and summarized the existing evidence from large prospective cohort studies on muscle strengthening activities and risk of major chronic diseases and mortality in adults generally free of major NCDs at baseline. Current epidemiologic evidence suggests that engagement in muscle-strengthening activities over 1–2 sessions (or approximately 60–150 min) per week was associated with reduced risk of cardiovascular disease (seven studies; approximately 20%–25% reduction), type 2 diabetes (four studies; approximately 30% reduction), cancer mortality (four studies; approximately 15%–20% reduction) as well as all-cause mortality (six studies; approximately 20%–25% reduction). For diabetes, the risk appears to lower further with even higher levels of muscle-strengthening activities, but some studies for cardiovascular and all-cause mortality suggest a reversal whereby higher levels (≥2.5 h/week) have less benefit, or are even harmful, relative to lower levels of activity. The likely mechanisms contributing to a benefit include improvement in body composition, lipid profile, insulin resistance and inflammation. The evidence supports engaging in 1–2 sessions (up to 2.5 h) per week, preferably performed complementary to the recommended levels of aerobic MVPA. Although data are limited, caution is suggested for training exceeding 2.5 h per week. Further studies are required to better understand the influence of frequency, duration and intensity of muscle-strengthening activities on major NCDs and mortality in diverse populations.  相似文献

ABSTRACT

This study investigated the relationships between different types of family resilience and various specific forms of family crisis. In recent decades, numerous studies have examined how people manage crisis and how resilience is developed to overcome periods of chaos and disruption. Most of these studies look only at individual cases, or focus on general concepts, theories, or fundamental frameworks addressing the basic interaction between resilience and crisis. This study uses the Family Adjustment and Adaptation Response model (Paterson, 1988) and the Family Strength Index (Orthner et al., 2003) to measure how family resilience relates to different kinds of family crisis. The results show that of the 6 types of family strength which comprise general family resilience, only economic, problem-solving, and family cohesion strength significantly predicted participants’ level of confidence in managing family crisis. Such a discrepancy from the findings of previous work may be explained by cultural factors, which are further discussed in this article.  相似文献

Antibiotic-loaded acrylic bone cements (ALABC) spacers are routinely used in the treatment of prosthetic joint infections. The objectives of our study were to evaluate different ALABC for elution kinetics, thermal stability, and mechanical properties. A 10 or 20% mixture (w/w) beads of medium viscosity bone cement (DePuy, Inc) and vancomycin (VAN), gentamycin (GM), daptomycin (DAP), moxifloxacin (MOX), rifampicin (RIF), cefotaxime (CTX), cefepime (FEP), amoxicillin clavulanate (AmC), ampicillin (AMP), meropenem (MER), and ertapenem (ERT) were formed and placed into wells filled with phosphate-buffered saline. Antibiotic concentrations were determined using high-performance liquid chromatography. Antimicrobial activity was tested against Micrococcus luteus ATCC 9341 or Escherichia coli ATCC 25922. AmC, AMP, and FEP concentration rapidly decreased after day 2, being almost undetectable at day 4. Sustained and high elution rates were observed with VAN, GM, MOX, and RIF for the 30-day duration of the experiment. DAP, MER, ERT, and CTX elution rates constantly decreased from day 4. All antibiotics tested retained antimicrobial activity proving thermal stability. Mechanical properties of ALABC were maintained except when RIF was used.  相似文献
