Introduction A few epidemiologic studies have comprehensively attempted to identify risk factors for low bone mineral density (BMD) in elderly Asian women. The purpose of this study was to identify demographic, lifestyle, and biochemical factors correlated with BMD in elderly Japanese women 69 years of age and over.Methods The study design was cross-sectional. The subjects were 583 ambulatory women aged 69 years and over, and their average age was 74.3 (SD 4.4) years. Predictor variables were age, reproductive history, anthropometric indices, grip strength, calcium intake, lifestyle information, and serum 25-hydroxyvitamin D, 1,25-dihydroxyvitamin D (1,25(OH)2D), osteocalcin (OC), and undercarboxylated osteocalcin (ucOC) values. The outcome variable was forearm BMD measured with a DTX-200 osteometer.Results Simple linear regression analyses showed that BMD was significantly positively associated with body height, weight, body mass index, grip strength, serum albumin concentration, and “housework,” and negatively associated with age, years since menopause, age at menarche, number of children, serum 1,25(OH)2D concentration, serum OC concentration, and ucOC concentration. The stepwise multiple regression analysis showed that weight (β=0.00316, SE=0.00028, R2=0.180), age (β=−0.00321, SE=0.00050, R2=0.108), log-transformed serum OC (β=−0.0445, SE=0.0064, R2=0.053), log-transformed serum 1,25(OH)2D (β=−0.0401, SE=0.0074, R2=0.050), “farmwork” (β=0.00904, SE=0.00426, R2=0.005), and serum 25(OH)D concentration (β=0.000281, SE=0.000120, R2=0.003) were significantly associated with BMD.Conclusion It was concluded that body weight is a major predictor of forearm BMD among the factors measured in this study in independent Japanese women 69 years of age and over and that serum 1,25(OH)2D concentration may be associated with cortical BMD. Maintenance of body weight is very important for maintaining BMD in this population, unless a large weight aggravates obesity-related diseases. 目的 探讨人骨形态发生蛋白-2(hBMP-2)基因修饰的人脂肪源性间充质干细胞(ADSCs)的成骨能力及基因转染对ADSCs成骨分化的生物学调控。方法 流式细胞技术鉴定从人脂肪组织中提取的细胞为间充质来源的干细胞。将ADSCs转染hBMP+2基因,免疫沉淀+Western blotting法和酶联免疫吸附试验(ELISA)检测hBMP-2表达,确定hBMP-2表达量及表达稳定性,碱性磷酸酶(ALP)染色、ALP定量测定、钙结节染色及RT—PCR分析hBMP-2基因转染对ADSCs向成骨细胞转化的调控,并将转基因细胞注入裸鼠股部肌内,通过X线及组织学检查观察体内成骨情况。结果 流式细胞术证实从脂肪中提取的细胞为间充质来源的干细胞。转基因后免疫沉淀+Western blotting法和ELISA法显示转染hBMP-2的ADSCs具有较高且稳定的hBMP-2的表达,转染21d后表达量无明显降低。ALP染色、ALP定量测定、钙结节染色及RT—PCR发现转基因ADSCs向成骨细胞方向发生分化。裸鼠肌内注射转基因细胞后2周即显示有异位骨形成,4周明显增多,对照组无骨组织形成。结论 转染hBMP-2基因的ADSCs在持续稳定高表达目的蛋白的同时,自身向成骨细胞发生分化,并在裸鼠体内形成异位骨。因此,ADSCs有望作为新的基因工程种子细胞。 相似文献

目的:探讨动力髁螺钉(DCS)治疗股骨髁上骨不连的临床疗效和特点。方法:14例段骨髁上骨不连均采用DCS固定 自体筋骨植骨治疗,4例伴有膝关节僵硬者同时行膝关节松解术。结果:所有病例获得随访,时间12—48个月,平均18个月。在4—9个月内均获骨性愈合。有2例膝关节僵硬获得明显改善。参照Shelbourne疗效评定标准,ll例疗效优良,优良率78.6%。结论:应用DCS固定 自体筋骨植骨后骨折端可获得坚持内固定及压应力,手术操作简便、安全,可早期进行膝关节和股四头肌功能锻炼,是一种治疗段骨髁上骨不连的有效方法。 相似文献

To study the effect of chemotherapy on normal fat, skeletal muscle, and bone marrow, T1 and T2 relaxation times were measured in 15 patients with bone sarcoma before and after each cycle of preoperative chemotherapy. A section plane containing the tumor and if possible the nonaffected extremity was imaged with combined multiecho spin echo and inversion recovery pulse sequences. T1 and T2 relaxation times were calculated in the normal-appearing tissues. Although some variation was found in the values in the individual patient and between patients, no systematic changes of relaxation times of fat, muscle, or bone marrow occurred in the course of treatment. We conclude that the chemotherapy used in bone sarcoma has no effect on relaxation times of normal fat, muscle, and bone marrow, and that therefore these tissues may serve as a reference for the signal intensity of tumor. 相似文献

The records of 1018 patients with low back pain in a tertiary spine referral practice were reviewed. One hundred thirty-nine out of 1018 (13.6%) underwent technetium-99m planar bone scanning as part of their investigation. Seventy-three out of 139 scans (52%) showed increased uptake in some area, but only 27 out of 139 (19.4%) showed increased uptake specifically in the low back. Scans consistently yielded no findings with reference to the back when the prescan diagnosis was spinal stenosis, lumbar pain syndrome, herniated nucleus pulposus, or postlaminectomy syndrome. Some scans gave positive findings in patients with a diagnosis of degenerative disc disease, pseudarthrosis, spondylolisthesis, fracture, infection, metabolic disorder, or tumor. Positive scans were generally obtained early after presentation (within 3 months) and negative scans obtained later (after 6 months), suggesting that clinical suspicion is still the main indication for early scanning. Planar bone scanning was helpful in both diagnosis and therapeutic decisionmaking in many conditions. 相似文献

目的 :观察骨延长术对血清微量元素的影响。方法 :采用实验家兔 6只 ,造成左胫骨中段骨延长模型 ,定期耳缘静脉采血。原子吸收分光光度仪测定血清中锌、铜、铁的含量。结果 :骨延长术后血清铜元素含量呈上升趋势 ,血清铁、锌元素含量明显下降。在延长结束后 5d时与术前比较有明显差别 (P <0 .0 5 )。结论 :骨延长术后 ,血清铁、锌元素含量呈阶段性下降 ,适时予以补充 ,有利于骨组织的修复。 相似文献

Transient osteoporosis is an infrequent condition of uncertain etiology with pain, limited range of motion and radiographic
evidence of osteoporosis affecting one or more joints. It is self-limited, reversible and can involve only the hip (transient
osteoporosis of the hip, TOH) or, less frequently, one or more joints contemporaneously or at different times (regional migratory
osteoporosis, RMO). We studied four men with transient osteoporosis, including two with TOH and two with RMO. All patients
underwent a standard radiographic work-up of the affected joints, arteriovenous Doppler US, computed tomography, magnetic
resonance imaging (MRI) and three-phase bone scanning. In all patients, symptoms were related to bone marrow edema demonstrated
at MRI and to a transitory regional arterial hyperflow observed at the early scintigraphic analysis. On the basis of our observations,
we hypothesize that regional arterial hyperflow may be the cause of the bone marrow edema and therefore of the transient osteoporosis. 相似文献

Background: In recent years, biomaterials are being found and frequently utilized in bone defects. They have also gained significant precedence
in hand surgery.
Objectives: The respective requirements for such replacement material will be cited and acknowledged in this article. The individual material
groups will also be referred to in this review. An introduction to some of the customary bone replacement materials will be
cited and concluded with a corresponding recommendation.
Conclusion: The implantation of autologous cancellous bone is still regarded today as the “gold standard”. Nevertheless, the usage of
bone replacement material can be an enormous advantage in certain indications.
