Kidney Transplantation in Patients with Antibodies against Donor HLA Class II

The immunologic risk associated with donor-specific antibodies (DSA) against Class II human leukocyte antigens (HLA) in kidney transplant (KTx) recipients is unclear. The aim of this study was to determine the outcome of KTx when DSA was detected only against HLA Class II. To isolate the impact of anti-Class II DSA, we retrospectively analyzed 12 KTx recipients who at baseline had a positive B-cell flow cytometric crossmatch (FXM) and a negative T-cell FXM. Using alloantibody specification analysis, 58.3% (7/12) had DSA against donor Class II and 41.7% had no demonstrable DSA. Biopsy-proven AMR occurred in 57% (4/7) in the Class II(+) group and 0% in the Class II(-) group (p > 0.05). Peritubular capillaries stained positive for C4d in 86% (6/7) of the Class II(+) patients and in 40% (2/5) of the Class II(-) patients (p > 0.05). One patient in the Class II(+) group lost their graft at 3 months to accelerated transplant glomerulopathy, while all other grafts were functioning 3-37 months posttransplant despite the persistence of anti-Class II DSA. We conclude that KTx recipients with clearly defined anti-Class II DSA are at risk for humoral rejection suggesting that desensitization and/or close posttransplant monitoring may be needed to prevent AMR.     

家兔阴茎皮肤感觉脊神经来源的电生理研究

目的:探讨家兔阴茎感觉神经来源。方法:健康成年雄性新西兰白兔12只,随机均分为两组:每组6只,A组在左侧脊神经上记录,B组在右侧脊神经上记录。对家兔阴茎施加不同大小的机械刺激,用神经单纤维记录技术,在同侧S1~S4脊神经上记录单纤维放电。结果:通过对家兔阴茎施加各种不同机械刺激,在同侧S2~S3脊神经上能记录到放电,S1、S4脊神经上未能记录到放电。左侧脊神经放电纤维数量分别为:S2:39.67±3.14,S3:21.00±2.19;右侧脊神经放电纤维数量分别为:S2:40.00±3.16,S3:19.67±2.58,左右侧差异无显著性(P>0.05)。结论:家兔阴茎皮肤感觉来源于S2~S3脊神经。     

Safety, tolerability and pharmacokinetic study of recombinant human parathyroid hormone in healthy male Chinese subjects

AIM： To determine the safety, tolerability and pharmacokinetic parameters of a new drug recombinant human parathyroid hormone [ rhPTH （1-84）] in healthy male Chinese subjects. METHODS： domly divided Thirty-six healthy male volunteers were rangroups received into 3 groups. The volunteers in these single subcutaneous injection of rhPTH （ 1-84） in a dosage of 1, 2 and 4 μg/kg respectively. Blood samples were obtained before and after administration within 24 hours. The rhPTH concentrations in sennn were determined by enzyme linked immunosorbent assay （ELISA）. The pharmacokinetic parameters determined with use of standard noncompartmental analysis were the maximum serum concentration （ Cmax ）, the time to attain that concentration （ tmax ）, and the area under the serum concentration-time curve up to 24 hours（ AUC0-24 ） and up to infinity （AUC0-∞）. Dose proportionality of pharmacokinetic parameters （AUC, Cmax of every volunteer of each dosage and A UC was computed from log transformed data） and was examined by mean of analysis of variance （ANOVA） using SPSS software package. In the study, subjects＇ symptoms, objective signs, and vital signs, including blood pressure, heart rate, respiratory rate and body temperature, were checked and 12-lead electrocardiography was recorded before and after drug administration within 24 hours. Routine laboratory tests, including hematology, blood biochemistry, serum electrolyte, and urinalysis, were performed before and after drug administration within at 24 hours.[第一段]     

烟叶硒蛋白对人红细胞的辐射溶血及自由基的作用

本文观察了烟叶硒蛋白对人红细胞６０Ｃｏγ照射溶血的影响，发现硒蛋白对红细胞γ照射溶血有极好的预防效果，同时采用电子自旋共振法（ＥＳＲ）发现硒蛋白对羟自由基有明显的清除作用，并具有明显的剂量－效应关系，在相同浓度下具有优于亚硒酸钠及不含硒的同类蛋白的作用，提示硒蛋白有更好的生物效应。     

Cavernous sinus involvement in recurrent Nelson's syndrome

Summary Two cases of meningiomas, which are considered to have been caused by the preceding irradiation, are reported. In both cases, the cytokinetic study of the tumour using bromodeoxyuridine (BrdU) was performed. The percent of the tumour cells in the S phase of the cell cycle was less than 1% in both cases. The low labelling indices might suggest a relatively slow growing potential of these tumours, though the radiation-induced meningiomas were reported as being rapidly growing and malignant.     

Single fiber electromyography of extraocular muscles: A sensitive method for the diagnosis of ocular myasthenia gravis

We performed single fiber electromyography (SFEMG) in the superior rectus and levator palpebralis (SR-LP) muscles of 17 patients with pure ocular myasthenia gravis (MG) and 9 controls. Thirteen patients were also assessed with SFEMG in the orbicularis oculi (OO) muscle. All the MG patients but none of the control subjects showed abnormal SFEMG jitter in the SR-LP muscles. On the other hand, only 62% of the MG patients had abnormal SFEMG jitter in the OO muscle. The procedure was well tolerated by the patients, and complications were minor. We conclude that SFEMG of the SR–LP muscles is a safe and highly sensitive technique for the diagnosis of ocular MG. © 1995 John Wiley & Sons, Inc.     

Effects of 5-Hydroxytraptamine on Aggregation,Release Reaction and Mobilization of Cytosolic Free Calcium Concentration in Rabbit Normal and Washed Single Platelets

In rabbit platelet rich plasma (PRP), 5-hydroxytraptamine (5-HT,0.03～3μmol/L) induced decrease in light transmission (DLT) dose-de pendently with centralization, as revealed by electron microscopy. However, 5-HT did not induced platelet aggregation and release reaction. 5-HT-induced DLT was inhibited by methysergide (0.3～30μmol/L), indomethacin (0.3～10μmol/L) and PGE_1 (0.01～0.3μmol/L) in a dose-dependent manner, but not EGTA(0.3～3mmol/L). Collagen(Coll), arachidonic acid (AA), adenosine diphosphate (ADP) and a stable thromboxane A2 analoge(STA_2) also induced DLT before aggregation by themselves, which was also inhibited by PGE_1, but not inhibited by EGTA except for Coll However,Coll-, AA-, STA_2-and ADP-induced DLT were reduced by pretreatment of PRP with 5-HT dose-depen-dently. The duration of DLT by Coll and AA were decreased from 151.4±5.93 sec and 15(?)38±0.60sec to 45.44±4.04 sec and 9.00±1.25 sec respectively ((?)±s(?) P<0.01), but not by ADP and STA_2, 3μmol/L of ADP-and 0.3μmol/L of STA_2-induced aggregation which was not accompanied with release reaction were enhanced by 5-HT pretreatment, but in those (Coil 5μg/ml, AA 100～200μmol/L and STA_2 1～3 μmol/L) with release reaction, the amount of adenosine triphosphate(ATP)were suppressed significantly (P<0.001) by 5-HT pretreatment without the effect on the magnitude of aggregation, The mobilization of cytosolic free calcium concentration ([Ca~(2+)]i) was observed after 5-HT treatment in single washed platelet, the results indicated that the basic level of [Ca~(2+)] i was 64.78±3.24nmol/L and this level was increased dose-dependently and significantly at 30 sec after administration of 5-HT and the time of peak value of [Ca~(2+)] i was at 90～100 sec.The similar time courses of suppression of ATP released during aggregation, in cases of Coll(5μg/ml), AA (200μmol/L) and STA_2(3μmol/L), by 5-HT were also found in the present experiment.     

女孩雌激素受体基因6号内含子区域两个新SNPs位点报道

目的：在女性性早熟患儿中检测雌激素受体基因6号外显子及内含子基因序列可能存在的突变。方法：PCR扩增雌激素受体基因6号外显子及内含子，对扩增产物进行DNA序列分析。结果：在6号内含子一侧，发现两个100％连锁的单核苷酸多态性(SNPs)，分别为．53G—67A和53T—67G。结论：这一对单核苷酸多态性并不引起氨基酸的替代，与女孩性早熟的发生也无关，但是否和其他疾病有连锁关系尚待进一步探讨。     

紫外分光光度法同时测定增效黄连素胶囊中二组分含量

本文提出以紫外分光光度法测定增效黄连素胶囊含量。样品经乙醇溶解、水稀释后，不经分离提取，直接在３４６ｎｍ波长处测定盐酸黄连素；在３４６ｎｍ和２７１ｎｍ波长处以系数倍率法测定甲氧苄氨嘧啶，测得二组分的平均回收率分别为９９．１％和９９．６％，相对标准偏差为０．６７％和０．８２％（ｎ＝９）．本法简便，快速，适用于增效黄连素胶囊的常规分析．
