Obtaining certification is a critical step on the road to professionalism. There are many questions to consider in this decision. Included among these are cost, preparation, and return on the investment. Important answers to these questions are discussed.     

“Patient Care in Radiology”—The Staff's Perspective

The aim of this study was to research how the staff experience care expressed during the brief encounter with the patients in a diagnostic imaging department. This was a qualitative study with a phenomenological and hermeneutical frame of reference. The data were collected using field observations and semistructured interviews and analyzed according to the guidelines for meaning condensation by Giorgi. The imaging staff found that care is expressed in an administrative, an instrumental, and a compassionate sense. The imaging staff perceived care in a way that clearly differs from the traditional perception of care understood as the close relations between people. In their self-understanding, the staff found that care not only comprised the relational aspect but also that it was already delivered during the preparatory phases before the actual meeting with the patient and up until the image was electronically forwarded. And, care expressed in between was perceived as care in the traditional sense and termed as “patient care in radiology.”     

A retrospective analysis comparing set up errors from standard versus customised headrests for head and neck radiotherapy

IntroductionIn response to advice from The National Institute for Health and Care Excellence ⁽¹⁾ to reduce hospital visits during COVID-19, standard headrests were introduced for head and neck radiotherapy within Northern Centre for Cancer Care (NCCC). The standard headrest requires one mould room appointment compared to 3 appointments with customised headrests.MethodsTwo groups of 10 patients treated between December 2019 and June 2020 were retrospectively analysed by 1 observer. Groups were stratified according to age, sex and tumour site. One group had customised headrest and the other had standard headrest. Five hundred and forty seven cone beam computed tomography images were reviewed. A 6 Degree of Freedom match was performed then chin, shoulder and spine position were assessed using dosimetrist drawn structures. Structures out of the tolerance were recorded. A chi-squared test was used for statistical analysis.ResultsThe out of tolerance chin position count recorded was 21 for customised headrest and 36 for standard headrest, p-value 0.046. The shoulder position count was 13 for customised headrest and 77 for standard headrest p-value <0.001. The spine position count was 3 for CHR and 21 for standard headrest, p-value <0.001. This means the headrests compared are not equivalent in terms of set up reproducibility. Overall the standard headrest group had 10 set-up re-scans and no set up re-scans were recorded in the customised headrest group.ConclusionFewer hospital visits with SHR reduce patient exposure to COVID-19. However, CHR provided a more reliable level of immobilisation in this study.Implications for practiceThe radiotherapy service will be reviewed in line with these findings.     

Improving Radiology Report Quality by Rapidly Notifying Radiologist of Report Errors

Radiology report errors occur for many reasons including the use of pre-filled report templates, wrong-word substitution, nonsensical phrases, and missing words. Reports may also contain clinical errors that are not specific to the speech recognition including wrong laterality and gender-specific discrepancies. Our goal was to create a custom algorithm to detect potential gender and laterality mismatch errors and to notify the interpreting radiologists for rapid correction. A JavaScript algorithm was devised to flag gender and laterality mismatch errors by searching the text of the report for keywords and comparing them to parameters within the study’s HL7 metadata (i.e., procedure type, patient sex). The error detection algorithm was retrospectively applied to 82,353 reports 4 months prior to its development and then prospectively to 309,304 reports 15 months after implementation. Flagged reports were reviewed individually by two radiologists for a true gender or laterality error and to determine if the errors were ultimately corrected. There was significant improvement in the number of flagged reports (pre, 198/82,353 [0.24 %]; post, 628/309,304 [0.20 %]; P = 0.04) and reports containing confirmed gender or laterality errors (pre, 116/82,353 [0.014 %]; post, 285/309,304 [0.09 %]; P < 0.0001) after implementing our error notification system. The number of flagged reports containing an error that were ultimately corrected improved dramatically after implementing the notification system (pre, 17/116 [15 %]; post, 239/285 [84 %]; P < 0.0001). We developed a successful automated tool for detecting and notifying radiologists of potential gender and laterality errors, allowing for rapid report correction and reducing the overall rate of report errors.
