目的:探讨钛合金板置入在颅骨缺损修复中的应用。方法:对12例颅骨缺损的患者先进行三维CT检查,设计个性化的钛合金板置入以修复颅骨缺损。结果:本组患者均愈合良好,无感染、钛板外露及头皮血运障碍,避免了再次手术。结论:应用钛合金板修复颅骨缺损,具有良好的生物学力度,抗击性强,并发症少,生物相容性好等优点,可获得较好的治疗效果。  相似文献

义齿PMMA基托表面粗糙度影响的实验研究   总被引:2,自引:0,他引:2  
目的:研究技工操作因素对义齿树脂基托表面粗糙度的影响。方法:在不同粉液比、充填时期、聚合温度、混合方式以及出盒温度成型的上颌半口义齿基托上选取1.5cm^2范围,按照统一的打磨顺序,由粗到细打磨抛光完成,浸入冷水中,7天后进行粗糙度测试,所得数据经t检验统计处理。结果:在单体含量降低组、丝状期充填组.热处理时直接放入70℃水中维持9h组与对照组相比,均存在显著差异(P〈0.05)。结论:影响PMMA树脂表面粗糙度的制作工艺因素有单体含量降低、丝状期充填和未经100℃高温热处理。  相似文献

目的:通过对相同长度、宽度、不同厚度的Ti-6Al-7Nb合金铸件挠度的测量,观察铸件厚度与挠度的关系,为确定Ti-6Al-7Nb合金铸造义齿基托的适合厚度提供实验依据。方法:参照ISO关于义齿基托材料挠度的标准,测量不同厚度Ti-6Al-7Nb合金铸件的挠度,并与纯钛、Ti-6Al-4V合金及Co-Cr合金对比。结果:Ti-6Al-7Nb合金、纯钛及Ti-6Al-4V合金铸件的厚度在0.65mm时可以达到义齿基托挠度的标准;Co-Cr合金在0.57mm时可达到标准要求。结论:在临床制作Ti-6Al-7Nb合金义齿基托时,厚度应不低于0.65mm。  相似文献

The primary objective of this study was to evaluate morbidity associated with facial translocation approaches for skull base and results of various technical modifications. Forty consecutive patients who underwent facial translocation approaches for accessing skull base tumors from July 2005 to June 2010 were included in this study. There were 25 patients who underwent standard facial translocation, 4 patients medial mini, and 11 patients underwent extended facial translocation. Thirteen patients had benign disease and 27 patients had malignant disease. Resection was R0 in 36 and R1 in 4 patients. Most patients had acceptable cosmetic results. None of the patients had problems related to occlusion or speech and swallowing. The commonest complication observed was nasal crusting in 16 patients. Grade 2 trismus and exposure of mini plate was seen in three patients. Two patients developed necrosis of translocated bone. Three patients developed palatal fistula before modification of palatal incision. Facial translocation provides a satisfactory access for adequate clearance of skull base tumors with satisfactory aesthetic and functional results. With modifications of the surgical technique and implementation of new surgical tools, the morbidity of facial translocation approaches will continue to decrease.  相似文献

The endoscopic endonasal technique is currently used by otolaryngologists for the management of different extradural lesions located below the ethmoidal planum. The cooperation between ENTs and neurosurgeons has recently pushed the use of such approach also in the removal of some intradural lesions, which has promoted the interest for an anatomic study to identify the anatomical landmarks and the dangerous points during the endoscopic approach to this area. In six fresh cadaver heads, unilateral and bilateral measurements between the main landmarks of the approach were performed by means of an endoscopic endonasal approach. A wide exposure of the midline anterior skull base was realized. The maximum of lateral extension was obtained between the two medial orbital walls, at the middle of the cribriform plate (mean distance 25,33 mm), while the mean distance between the anterior and posterior ethmoidal arteries at the level of the lamina papyracea was 16 mm. The endoscopic endonasal route can be considered a minimally invasive technique to approach the ethmoidal planum. It requires adequate anatomical knowledge and endoscopic skill for its realization. Due to the wide window realizable through this corridor, it could be considered in selected cases for the removal of intradural lesions such as meningiomas or estesioneuroblastomas.  相似文献

目的探讨鼻内镜径路治疗颅底疾病的价值。方法2002年5月~2006年12月,对112例颅底疾病行鼻内镜径路手术,其中脑脊液鼻漏39例,垂体瘤4例,脑膜膨出或脑膜脑膨出3例,蝶筛囊肿9例,蝶窦炎伴息肉2例,真菌性蝶窦炎12例,内翻性乳头状瘤11例,鼻咽纤维血管瘤6例,骨化纤维瘤2例,骨纤维异常增殖2例,脊索瘤7例,颅咽管瘤2例,鳞癌10例,未分化癌1例,乳头状瘤癌变2例。结果全组均在鼻内镜下处理,其中20例病变为次全切除,包括鳞癌8例、未分化癌1例、脊索瘤7例、颅咽管瘤2例、骨纤维异常增殖2例。脑脊液鼻漏1次手术成功31例(79.5%),2次成功4例,3次成功4例,最终成功率100%。1例脑膜脑膨出术后2个月复发并伴脑脊液漏,内镜下再次手术成功。1例内翻性乳头状瘤术后11个月复发,改行鼻侧切开术。1例真菌性蝶窦炎术后4个月复发行再次内镜手术。1例脑脊液鼻漏术后颅内感染。结论鼻内镜径路可以处理多种颅底疾病,是一种安全、有效、微创的手术方式,但仍需严格掌握手术适应证,特别是恶性肿瘤。  相似文献

BACKGROUND: The relative importance of surgery within multimodality regimens commonly used to treat advanced sinonasal malignancies remains unknown. METHODS: One hundred two patients with locally advanced sinonasal cancers treated with proton beam radiation therapy with or without surgery were retrospectively reviewed. Extent of surgery and outcome variables of local control, disease-free survival, and overall survival were evaluated. Patterns of failure were also assessed. RESULTS: Extent of surgery correlated with disease-free survival and overall survival rates. Local control rate, however, was independent of the degree of surgical resection achieved. Overall, treatment failure most commonly resulted from distant metastases, which occurred in 30% of patients and also correlated with extent of surgical resection. Tumor type-specific outcomes reveal differences associated with the extent of surgery achieved. CONCLUSION: High-dose radiotherapy with proton beam resulted in excellent local control rates in patients with locally advanced sinonasal cancer, irrespective of the extent of surgery. Complete resection, however, was predictive of improved disease-free survival and decreased rate of distant metastasis.  相似文献

目的:报道采用经前颅底入路切除前颅底肿瘤的临床效果。方法:自1997年6月-2000年12月采用该入路切除前颅底中线部位肿瘤20例,其中肿瘤位于筛板5例,鼻筛窦4例,筛蝶窦7例,蝶斜坡区4例。术后10例行放射治疗。结果:本组20例中,肿瘤全切除12例,次全切除6例,大部分切除2例。术后1例死亡,6例有脑神经麻醉。结论:该手术入路能充分暴露病变,术野开阔,切除肿瘤较安全,对脑组织损伤小。  相似文献

The management of clival chordoma remains problematic. We present the case of a 48-year-old woman with clival chordoma who underwent multiple surgeries and radiation therapy, including gamma knife stereotactic radiosurgery (GK-SRS), during a 10-year clinical course. The tumor was initially removed by gross total resection via the trans-sphenoidal approach, followed by external linac radiation therapy. The tumor recurred at the clivus 5 years after the initial operation. After repeated trans-sphenoidal removal of recurrent tumors, she twice underwent GK-SRS for a tumor remnant adjacent to the brainstem. Although this part of the tumor was controlled by GK-SRS, there was further tumor extension toward the sphenoid and maxillary sinuses. Ultimately, lower cranial nerve dysfunction developed due to tumor extension into the lower part of the clivus and the patient died of respiratory failure. Autopsy revealed the tumor to extend from the lower clivus to the bilateral middle fossae. The lower part of the tumor extended to the nasal cavity and to the posterior wall of the pharynx, resulting in compression of the upper pharyngeal region. The tumor around the jugular foramen compressed the lower cranial nerves bilaterally. Tumor cells did not, however, invade the intradural space microscopically. Although chordoma is not biologically malignant, this tumor can show massive extension with destruction of bony structures and extracranial invasion of connective tissues. Therefore, the optimal treatment strategy is to remove the tumor mass as extensively as possible, including normal bony structures and connective tissues surrounding the tumor, using skull base surgical techniques.  相似文献
