摘 要 目的:探讨影响肺癌患者化疗后血常规与肝肾功能自我管理能力的因素,为临床药师和医师对肺癌患者的指导和管理提供思路。 方法: 通过面对面交流和问卷调查两种方式对使用静脉化疗药物进行一线治疗的原发性支气管肺癌患者进行调查分析,调查项目包括年龄、性别、学历、经济情况、职业、居住区域、伴随的慢性疾病及用药、肿瘤类型及分期、体力状况 (PS) 评分等基本情况,以及是否定期复查、是否规律处理相应异常指标等依从性评价内容,分析影响患者化疗后血常规与肝肾功能自我管理能力的因素。 结果: 患者年龄、PS评分对患者的复查依从有明显影响 (P<0.05),患者年龄对规范处理血常规与肝肾功能异常指标的意识有明显影响(P<0.05)。经Logistics回归分析, 年龄、PS评分是影响肺癌化疗患者复查依从的独立风险因素,年龄是影响血常规与肝肾功能异常指标规范处理意识的独立风险因素(P<0.05)。 结论: 药师有必要在患者每一周期入院化疗时均开展药学服务,尤其需强化年龄≥60岁的老年患者、PS评分为1的患者以及家属对血常规与肝肾功能规范管理的认识。  相似文献

摘 要 目的:探讨长链非编码RNA PVT1通过激活Wnt信号通路对肝癌干细胞自我更新能力的调控作用。 方法: 设HEPG2肝癌干细胞组、LNCRNA PVT1 mimics组、LNCRNA PVT1 inhibitor组,培养结束后,检测各组细胞活力、单克隆形成数目、成球率、凋亡率水平、G1期、PVT1 mRNA、WNT、SWI、SNF蛋白表达水平。 结果: LNCRNA PVT1 mimics组OD值、存活率水平、克隆形成数目、成球率、G1期、PVT1 mRNA、WNT、SWI、SNF蛋白表达水平高于HEPG2肝癌干细胞组(P<0.05),细胞凋亡率低于HEPG2肝癌干细胞组(P<0.05);LNCRNA PVT1 inhibitor组OD值、存活率、克隆形成数目、成球率、G1期、PVT1 mRNA、WNT、SWI、SNF蛋白表达水平低于HEPG2肝癌干细胞组、LNCRNA PVT1 mimics 组(P<0.05),细胞凋亡率高于HEPG2肝癌干细胞组、LNCRNA PVT1 mimics 组(P<0.05)。LNCRNA PVT1 mRNA 与克隆形成数目、成球率、G1期、WNT、SWI、SNF正相关关系明显,与凋亡率负相关关系明显(P<0.05)。 结论: LNCRNA PVT1通过激活SWI/SNF复合物进而激活Wnt信号传导来促进肝癌干细胞的自我更新和肿瘤增殖。  相似文献

BackgroundOsteoarthritis (OA) is a chronic degenerative disease which can result in chronic pain, loss of joint function and consequently decline in quality of life.Research questionA variety of instruments that measure the different dimensions of health status in patients with OA are available. However, despite the fact that WOMAC and other questionnaires and scales may provide additional understanding regarding the patient’s condition, some studies have reported discrepancies between patients’ perceptions and their actual ability to perform the task. The aim of the present study was investigate the physical capabilities with the function domain of WOMAC.MethodsThis study has a cross-sectional design including patients diagnosed with moderate to severe knee OA (i.e., grades II, III and IV) according to the Kellgren-Lawrence. These patients were submitted to the battery of functional tests recommended by the OARSI group (30-second chair stand test, 40 m fast paced walking test, Stair climb test, timed “Up and Go”, and Six-minute walking test) and filled the WOMAC. Pearson’s correlation and multiple linear regression was applied.ResultsA total of 153 patients were included. A significant and weak correlation was observed between WOMAC and the 40-meter walking test, TUG, stair-climbing test, and the 6MWT. In addition, 30-second chair stand test demonstrated a significant and moderate correlation (r=-0.503). The multiple regression analysis results indicated that only 30-second chair stand test was a significant (p = 0.001) predictor of WOMAC. This result remains significant even after adjusting for age, BMI, total muscle mass, and number of knees affectedSignificanceThe 30-second chair stand test is associated with the WOMAC function domain. There is no correlation of this domain with any other functional tests, emphasizing the importance of including other tests for a global evaluation.  相似文献

Venous leg ulceration has a high recurrence rate. Patients with healed or frequently recurring venous ulceration are required to perform self‐care behaviours to prevent recurrence or promote healing, but evidence suggests that many find these difficult to perform. Bandura's self‐efficacy theory is a widely used and robust behaviour change model and underpins many interventions designed to promote self‐care in a variety of chronic conditions. By identifying areas where patients may experience difficulty in performing self‐care, interventions can be developed to strengthen their self‐efficacy beliefs in performing these activities successfully. There are currently a variety of self‐efficacy scales available to measure self‐efficacy in a variety of conditions; but not a disease‐specific scale for use with venous ulcer patients. The aim of this study, therefore, was to develop and validate a disease‐specific, patient‐focused self‐efficacy scale for patients with healed venous leg ulceration. This scale will need further validation studies; however, it is ready for use in clinical practice and will enable practitioners to identify those patients who may need additional support in performing self‐care activities to prevent recurrence.  相似文献

Peripheral nerve injuries can result in lifelong disability. Primary coaptation is the treatment of choice when the gap between transected nerve ends is short. Long nerve gaps seen in more complex injuries often require autologous nerve grafts or nerve conduits implemented into the repair. Nerve grafts, however, cause morbidity and functional loss at donor sites, which are limited in number. Nerve conduits, in turn, lack an internal scaffold to support and guide axonal regeneration, resulting in decreased efficacy over longer nerve gap lengths. By comparison, peptide amphiphiles (PAs) are molecules that can self-assemble into nanofibers, which can be aligned to mimic the native architecture of peripheral nerve. As such, they represent a potential substrate for use in a bioengineered nerve graft substitute. To examine this, we cultured Schwann cells with bioactive PAs (RGDS-PA, IKVAV-PA) to determine their ability to attach to and proliferate within the biomaterial. Next, we devised a PA construct for use in a peripheral nerve critical sized defect model. Rat sciatic nerve defects were created and reconstructed with autologous nerve, PLGA conduits filled with various forms of aligned PAs, or left unrepaired. Motor and sensory recovery were determined and compared among groups. Our results demonstrate that Schwann cells are able to adhere to and proliferate in aligned PA gels, with greater efficacy in bioactive PAs compared to the backbone-PA alone. In vivo testing revealed recovery of motor and sensory function in animals treated with conduit/PA constructs comparable to animals treated with autologous nerve grafts. Functional recovery in conduit/PA and autologous graft groups was significantly faster than in animals treated with empty PLGA conduits. Histological examinations also demonstrated increased axonal and Schwann cell regeneration within the reconstructed nerve gap in animals treated with conduit/PA constructs. These results indicate that PA nanofibers may represent a promising biomaterial for use in bioengineered peripheral nerve repair.  相似文献

目的编制中国积极信念自评预测量表并进行信度、效度检验。方法通过预测验构建初问卷,并采用探索性因素分析、相关分析、t检验等方法分析数据,检验其信效度。结果根据因子分析结果,提取1个因子,正式量表含有10个条目,累计方差贡献率为51.85%。总量表的Cronbach'sα系数为0.783,分半系数为0.852。10个条目与总量表的相关系数在0.624~0.778(P0.01)。高分组与低分组比较,各条目得分差异显著(t=2.57~5.38,P0.01)。结论中国积极信念自评量表的信度、效度符合心理测量学要求,可作为中国人积极信念的自评工具。  相似文献

Introduction

The purpose of this study was to evaluate the fracture strength of roots instrumented with the Self-Adjusting File (SAF; ReDent-Nova, Ra’anana, Israel) and the ProTaper system (Dentsply Maillefer, Ballaigues, Switzerland) and filled with the cold lateral compaction technique.

Methods

In total, 50 mandibular premolar teeth were sectioned at or below the cementoenamel junction to obtain roots 13 mm in length. The roots were balanced with respect to buccolingual-mesiodistal diameters and weight. They were distributed into 4 experimental groups and 1 control group (n = 10): no instrumentation (control group), instrumentation with ProTaper rotary files but no filling (PT), instrumentation with ProTaper rotary files and filling with cold lateral compaction (PT filling), instrumentation with SAF but no filling (SAF), and instrumentation with SAF and filling with cold lateral compaction (SAF filling). AH Plus sealer (Dentsply DeTrey, Konstanz, Germany) was used along with gutta-percha points. One week later, a vertical load was applied to the specimen’s canal until fracture occurred. Data were statistically analyzed using 1-way analysis of variance (P = .05).

Results

The mean fracture load was 244 ± 51 N for the control group, 226 ± 21 N for the PT, 242 ± 45 N for PT filling, 233 ± 14 N for the SAF, and 271 ± 44 N for SAF filling. However, the differences were not statistically significant (P > .05).

Conclusions

Instrumentation with the SAF or the ProTaper rotary system did not change the fracture strength of standardized roots with respect to cross-sectional diameter and weight.  相似文献

目的:调查灼口综合征(BMS)患者的心理抑郁及焦虑状况,分析心理状况与疼痛感受的相关性.方法:收集2012年1月~2014年1月期间因灼口综合征就诊患者135例及100名健康志愿者.采用焦虑自评量表(SAS)和抑郁自评量表(SDS)对两组对象进行问卷测试.对BMS患者的主观疼痛程度进行分级.采用SPSS11.0统计分析软件分析BMS患者抑郁、焦虑状况及其与疼痛感受的相关性.结果:BMS患者SAS值及SDS值均高于对照组,差异具有统计学意义(P<0.01),SAS值及SDS值在不同疼痛程度的患者中有显著差异(P<0.01).结论:BMS患者具有明显的抑郁及焦虑心理,其心理抑郁及焦虑的严重程度与患者临床疼痛的主观感受有关.  相似文献
