中段尿道吊带术治疗女性压力性尿失禁的手术并发症防治   总被引:2,自引:2,他引:0  
目的探讨中段尿道吊带术的并发症及其防治措施。方法采用中段尿道吊带术治疗女性压力性尿失禁48例,其中,TVT术26例,TVT-O术19例,Sparc术3例。结果手术并发症包括:1例(TVT)术中膀胱镜检发现穿刺针穿入膀胱,经重新调整穿刺纠正。2例(Sparc术和TVT-O术各1例)分别于术后1、3个月发现阴道前壁吊带外露,经再次手术修补阴道前壁治愈。4例出现尿急症状,给予黄酮哌酯对症治疗2周后症状明显改善。2例出现股内侧轻度疼痛,未经特殊治疗,1周内症状自行缓解。结论中段尿道吊带术治疗女性压力性尿失禁方法简便、疗效可靠,但其手术并发症的发生应引起重视。  相似文献

目的探讨无张力阴道吊带术(tension-free vaginal tape,TVT)治疗女性压力性尿失禁的疗效。方法13例经尿动力学检查证实为压力性尿失禁在连续硬膜外麻醉下经阴道前壁行无张力阴道吊带术,低平截石位,经阴道前壁向上穿刺尿道两侧间隙,从耻骨上腹壁引出TVT吊带,调整张力,关闭切口。结果手术时间15~45min,平均35min。13例随访6~24个月,平均13个月,12例治愈,1例改善,无尿失禁复发或排尿困难。结论TVT操作简单,创伤小,手术时间短,术后恢复快,治疗压力性尿失禁疗效好。  相似文献

目的探讨经闭孔无张力尿道中段悬吊术(trans-obturator tension free vaginal tape,TVT-O)治疗女性压力性尿失禁(stress urinary incontinence,SUI)的临床疗效。方法2006年1月~2007年9月,对15例SUI行TVT-O,取阴道前壁纵行切口1cm,组织剪分离尿道旁阴道黏膜下间隙至闭孔膜,放入蝶形导引杆,将螺旋形穿刺针沿导引杆紧贴耻骨下支穿过闭孔膜,从大腿根部皮肤穿出,将吊带无张力放置于尿道中段下方。结果手术时间15~40min,平均30min。术中出血量10~40ml,平均20ml。术中无并发症。1例术后尿潴留,留置尿管5d后缓解。10例术后大腿根部疼痛,术后3~10d消失。15例随访2~19个月,平均8个月,均治愈,无复发。结论TVT-O操作简单、安全有效。  相似文献

目的总结用人工合成材料的吊带经不同的途径治疗女性压力性尿失禁的方法和结果。方法采用人工合成材料的吊带经耻骨固定(In—Fast)技术治疗3例,用经腹壁固定(TVT和IVS)技术治疗13例。结果绝大多数病人均排尿通畅.无尿失禁复发。但TVT组有1例排尿不畅,3个月后剪断吊带后变为轻度尿失禁。In-fast组有1例性交不适伴阴道分泌物增加。结论用人工合成材料的吊带进行悬吊技术治疗女性真性压力性尿失禁是安全、微创和有效的手术方式。经耻骨固定技术和用经腹壁固定技术这两种方法各有自己的优缺点。应根据病人的具体情况去选择使用。  相似文献

PURPOSE: Biofeedback treatment is often offered to patients in colorectal centers; however, standards of treatment are still lacking. A dedicated team approach is desirable but difficult to coordinate. We present our three-year experience of electromyographic-based biofeedback treatment offered within a multicenter, statewide organization. METHODS: Between October 1992 and October 1995, 188 patients completed a biofeedback treatment program in one of five coordinated centers within a 200-mile radius. A unified common database was established and continuously updated. A colorectal surgeon served as statewide director, and dedicated teams were established at each location. Each local team included the medical director and a certified biofeedback therapist and had access to a dietitian and a nurse data coordinator. Electromyographic-based biofeedback sessions were given weekly, and a home trainer program was established. RESULTS: A total of 116 patients with chronic constipation had a mean of eight (range, 2–14) weekly sessions. A total of 72 patients with fecal incontinence had a mean of seven (range, 2–11) weekly sessions. A total of 84 percent of the constipated and 85 percent of the incontinent patients had significant improvement with biofeedback treatment. Patient compliance and satisfaction were high. Constipated patients increased the mean number of weekly unassisted bowel movements from 0.8 to 6.5. Incontinent patients decreased the mean number of weekly gross incontinence episodes from 11.8 to 2. CONCLUSIONS: Biofeedback treatment can be extremely successful in both incontinent and constipated patients. A large geographic area can be covered with coordinated centers in which each dedicated team uses a unified treatment protocol, and a common database is established.Poster presentation at the meeting of The American Society of Colon and Rectal Surgeons, Seattle, Washington, June 9 to 14, 1996.  相似文献

The aim of the study was to determine the role of neurogenic damage to pelvic floor muscles on the outcome of Burch colposuspension. Thirty women objectively continent after Burch colposuspension and 18 women with recurrent stress urinary incontinence (RSUI) were investigated with concentric needle electrode electromyography (EMG) in both pubococcygeus muscles and the external anal sphincter muscle. Neurogenic EMG patterns were significantly more often seen in the pubococcygeus muscles in women with RSUI than in women continent after the colposuspension (P<0.05). The distribution of neurogenic EMG patterns in the investigated muscles was significantly more pronounced in women with RSUI than in continent women: at least one pubococcygeus muscle with neurogenic EMG pattern, 72% vs. 34% (P<0.05); both pubococcygeus muscles, 50% vs. 13% (P<0.05); and all three investigated muscles 41% vs. 10% (P<0.05). In conclusion, the results imply an association between the outcome of the Burch colposuspension and the occurrence of neuropathy in the pelvic floor muscles. Occurrence of neurogenic damage in the pubococcygeus muscles seems to impair the outcome of Burch colposuspension.  相似文献

Laparoscopic colposuspension is one of many new operations for treating female urinary stress incontinence. With initially reported success rates similar to those of the traditional open procedure, it appears to combine the advantages of laparoscopy (such as minimal invasiveness and quicker return to normal activities) with the effectiveness of the standard procedure. Different methods and approaches are used, but endoscopic suture techniques remain difficult and time-consuming. The use of endostapling devices for fixation of alloplastic material has been a tempting alternative. We present a case during which laparoscopic colposuspension was performed using staples and mesh. Incontinence did not improve, and the patient suffered severe chronic pain for 18 months postoperatively. Removal of the alloplastic material and traditional abdominal resuspension led to complete cure.  相似文献
