Interactive technology applications for behavioral counseling: issues and opportunities for health care settings

CONTENT: This article discusses the rationale for, and the potential benefits and limitations of, computer-based interactive health communication (IHC) programs for health behavior counseling. We describe common barriers to health behavior counseling in medical settings and show how IHCs can address these issues. Following an overview of current and likely near-future IHCs, the potential impact of IHCs on the patient-provider relationship is considered. Results from evaluations of IHCs are summarized and important and unique issues in evaluating IHCs are discussed. We conclude with recommendations for clinical applications, including recommendations for consumers considering purchase or adoption of IHCs and recommendations for future research.     

Left Ventricular Thrombus After Pregnancy: Choice of Surgical Access Using Dynamic Three-Dimensional Echocardiography

Two idiopathic thrombi loosely attached to the left ventricular wall were detected by dynamic three-dimensional echocardiography. Because the thrombi were shown to be lobulated and extremly mobile, urgent surgical treatment was mandatory. Three-dimensional image reconstruction enhanced appreciation of left ventricular thrombi by enabling accurate imaging of the spatial relationship between the thrombus, the myocardial wall, and the valvular apparatus, so that the most advantageous surgical access could be chosen.     

Report on the Computer Software Contest at 38th Congress of the Japan Society of Anesthesiology

We held a computer software contest at 38th Congress of the JSA, held in March, 1991. The aim is to encourage the members of the Society to write softwares and to help distribute them, especially as Freewares. We received 25 entries for the contest; two-thirds of these are for computers of NEC PC9801 series and a third are for Macintosh. We received donations 3 million yen worth of instruments and goods for prizes plus some cash, which as prizes were distributed to those who made entries for the contest.Most of these programs have been registered as freewares at various computer networks, including our Ether-Net, one of the common computer network SIGBBSs among Japanese anesthesiologists.(Suwa K, Miyasaka K, Tanaka Y, et al.: Report on the computer software contest at 38th congress of the Japan society of anesthesiology. 可生物降解的注射用药物传递系统研究进展

目的在大量文献的基础上对可注射的体内半固态药物传递系统进行综述 ,指出它们作为局部植入药物传递系统所存在的优、缺点及其前景。方法根据在体内获得固态化的机制不同 ,介绍形成这些系统的不同方法及类型。结果与结论这些系统主要是由可生物降解材料制成的 ,具有生物相容性好、应用方便、可局部给药、延缓药物释放等优点 ,促进了新型药物传递系统的发展     

COMMENTS ON THE HOLISTIC ACTION OF ACUPUNCTURE IN VIEW OF ITS INFLUENCE ON NERVE-ENDOCRINE-IMMUNE NETWORK

In the present paper, the author makes some comments on the holistic action of acupuncture from the regulative effect on the nerve-endocrine-immune network, and the close relationship between the nerve-endocrineimmune network and the meridian-collateral system of TOM. The wholism concept of TOM refers to the organism being an integrated entirety, and the regulatory effect of acupuncture on functional activities of the organism relies on the integral connection of the acupoint--meridian-collateral-zangfu-organ system. When stimulated with acupuncture, the human body will brings its potential force into full play in preventing and treating diseases.     

放射治疗计算机信息管理系统的开发与应用

目的　开发放射治疗信息管理系统计算机软件。方法　采用局域网、客户机 服务器模式 ,以SQL SERVERA为数据库服务器 ,使用VisualFoxpro 5 .0为编程语言进行开发。结果　该系统包括放射治疗计划系统和放射治疗科数据管理系统。结论　该系统运行稳定 ,数据安全可靠 ,操作简单 ,易于临床推广使用。     

三维适形低分割放射治疗110例非小细胞肺癌

[目的]探讨非小细胞肺癌(NSCLC)三维适形低分割放射治疗(3D-CHRT)的疗效和有关影响因素.[方法]110例因内科或外科原因不能手术或患者不同意手术非小细胞肺癌,采用美国Varian 6MeV直线加速器,美国Renderplan三维治疗计划系统,单次剂量5Gy～10Gy(中位剂量6Gy),总剂量30Gy～48Gy(中位剂量40Gy).[结果]获CR 62例,PR 38例,NC 7例和PD 3例,总有效率为89.1%,1年生存率74.5%,严重并发症为大咳血(3例).[结论]立体定向适形放射治疗是NSCLC的有效治疗方法.影响治疗效果的主要因素是期别、病理类型和肿瘤大小.Ⅰ、Ⅱ期的鳞癌,肿瘤长径小者疗效较好;单次大剂量照射易引起严重并发症.     

胸段食管癌三维适形放疗摆位误差研究

Objective To study the setup errors in three-dimensional conformal radiotherapy (3DCRT) for thoracic esophageal carcinoma using electronic portal imaging device(EPID) and calculate the margins from CTV to PTV. Methods Forty-one patients with thoracic esophageal carcinoma who received 3DCRT were continuously enrolled into this study. The anterior and lateral electronic portal images (EPI) were aquired by EPID once a week. The setup errors were obtained through comparing the difference between EPI and digitally reconstructed radiographs(DRR). Then the setup margins from CTV to PTV were calculat-ed. By using self paired design,22 patients received definitive radiotherapy with different margins. Group A: the margins were 10 mm in all the three axes;Group B: the margins were aquired in this study. The differ-ence were compared by Paired t-test or Wilcoxon signed-rank test. Results The margins from CTV to PTV in x,y and z axes were 8.72 mm, 10.50 mm and 5.62 mm, respectively. Between the group A and group B, the difference of the maximum dose of the spinal cord was significant(4638.7 cGy±1449.6 cGy vs. 4310.2 cGy±1528.7 cGy; t=5.48, P=0.000), and the difference of NTCP for the spinal cord was also significant (4.82%±5.99% vs. 3.64%±4.70%;Z=-2.70,P=0.007). Conclusions For patients with tho-racic esophageal carcinoma who receive 3DCRT in author's department,the margins from CTV to PTV in x, y and z axes were 8.72 mm, 10.50 mm and 5.62 mm, respectively. The spinal cord could be better protected by using these setup margins than using 10 mm in each axis.
