Subjective bias in reports of poor work adjustment in depressed patients

Twenty-five depressed inpatients were asked to report their adjustment at work during the 4-month period preceding their hospitalization. Two separate reports were given: the first during the acute illness phase and the second one 10-28 days later, after symptomatic remission following standardized pharmacotherapy. Thirty-two percent more patient ratings showed absence of adjustment problems on the second report compared with the first. Significant differences between the 2 reports were found in 7 of the 9 items on the Structured and Scaled Interview to Assess Maladjustment, which is designed to assess specific aspects of maladjustment at work. The changes in the scores of maladjustment correlated with the changes in the scores of depressive symptoms, and 40% of the variation in maladjustment scores was accounted for by the pessimism item of the Montgomery-Asberg Depression Rating Scale. Much of the poor work adjustment reported by the acutely depressed patients thus seems to be caused by symptom-related subjective bias.     

Aortic valve injury 10 years after tetralogy of Fallot repair

Summary Although recognized by pediatric cardiac surgeons, aortic insufficiency as a technical complication after tetralogy of Fallot repair is poorly documented, especially if it occurs late. The case of a boy with aortic insufficiency 10 years after complete tetralogy repair is described. No documentation in the literature other than summary statements in textbooks of this occurence was found.     

骨盆骨折合并阴道损伤（附5例报告）

报告5例女性骨盆骨折合并阴道损伤，占骨盆骨折41例中的12.2%,均为交通事故伤，都合并有失血性休克，开放伤3例，闭合伤2例，治疗作阴道修补术2例，用碘仿纱条填塞阴道2例。未作处理1例，均治愈，阴道损伤表现为四型；横行撕裂伤，纵行撕裂伤，斜行撕裂伤和刺伤，强调早期诊断，及时治疗以防止合并症。     

成人结核性脑膜炎误诊原因分析

报告了成人结核性脑膜炎11例误诊,并收集国内报告的221例误诊病例,对结核性脑膜炎的误诊原因进行了综合分析。     

335例退行性骨关节病直流感应电康复总结

报告335例退行性骨关节病变,均经 x 线摄影证实,用直流感应电治疗。痊愈62例,占18.51%;显效115例,占34.33%;好转139例,占41.49%;无效19例;占5.67%。总有效率94.33%。有复发者再经治疗仍可收到效果。并对作用机制进行了探讨。     

大肝癌手术切除探讨（附16例分析）

报告16例大肝癌手术切除治疗效果，癌块直径>7cm。其中巨块型12例，癌块>10cm9例；癌块>14cm3例，均经病理证实。Ⅱ期切除3例，均经肝固有动脉结扎加插管注药化疗，肿块缩小后再手术，术后生存1年4例，2年3例，3年4例，4年1例，全组中位生存期24.1个月，并对大肝癌的手术适应证及术式选择等进行了探讨。     

胆囊小隆起性病变的诊断及治疗

报告37例胆囊小隆起性病变，其中良性颈变34例，胆囊癌3例。结果：本组23例有临床症状，多表现为反复发作的右上腹疼痛或不适；病灶部位，大小，多少，有无合并胆石及病变性质与临床症状有关；B-us,OCG,CT的诊断符合率例如辚33／37，9／24和5／10（x^2=18.75,P<0.005)。认为对胆囊小隆起性病变的病灶≥10mm，有增大趋势，呈恶性图像，合并胆石或血清CEA升高的病例应及早手术治疗。     

老年主动脉瓣闭锁不全的彩色多普勒超声心动图研究

报告接受彩色多普勒超声心动图检查的老年患者162例，其中75例有不同程度的主要动脉瓣关闭不全（Ar)。老年（Ar)的病因构成与中，老年前期者明显不同，主动脉，瓣钙化及原因不明者的比例占多数（67％）。主动脉瓣钙化者Ar程度多较重。主动脉瓣钙化及原因不明Ar患者的其他三个瓣膜返流检出率及和程度均明显高于无Ar的老年患者。表明多瓣膜退行性病变是此类Ar患者的原因之一。     

胸腰椎骨折脱位157例临床分析

本文报告157例胸腰椎骨折脱位病人,非手术治疗36例,手术治疗116例。作者对临床治疗中的有关问题进行了讨论。     

Benefits of the DICOM Structured Report

Recently, the digital imaging and communications in medicine (DICOM) standard introduced rules for the encoding, transmission, and storage of the imaging diagnostic report. This medical document can be stored and communicated with the images in picture archiving and communication system (PACS). It is a structured document that contains text with links to other data such as images, waveforms, and spatial or temporal coordinates. Its structure, along with its wide use of coded information, enables the semantic understanding of the data that is essential for the Electronic Healthcare Record deployment. In this article, we present DICOM Structured Report (SR) and discuss its benefits. We show how SR enables efficient radiology workflow, improves patient care, optimizes reimbursement, and enhances the radiology ergonomic working conditions. As structured input significantly alters the interpretation process, understanding all its benefits is necessary to support the change. Biography Rita Noumeir is a professor at the Department of Electrical Engineering of the University of Quebec, école de Technologie Superieure in Montreal. A founding member of the Imaging, Vision, and Artificial Intelligence Laboratory (LIVIA), her main research interest is the Healthcare Information Technology, specifically, Interoperability, Electronic Patient Record, Security, Information Confidentiality, and Image Processing. As a member of both Technical and Planning International IHE Radiology Committees, Dr. Noumeir took part over the last 5 years in developing many Integrating the Healthcare Enterprise (IHE) Integration Profiles in Radiology and in organizing several Integration Demonstrations. She is a cofounder of IHE Canada. Dr. Noumeir contributed to many research and development projects in collaboration with several Canadian and international companies in medical imaging and healthcare information. Currently, she collaborates with the Diagnostic Imaging Team of Canada Health Infoway to define the principles and architecture for sharing imaging information between multiple healthcare institutions. She plays a leading role in the development of this solution that is published as an IHE Integration Profile for which she is the editor. Rita Noumeir holds a Ph.D. and a Masters degree in Biomedical Engineering from école Polytechnique of Montreal specializing in Medical Imaging. She is a professional engineer, and a member of the Ordre des ingénieurs du Québec.
