医源性桡神经损伤

总结３１例医源性桡神经损伤的经验教训。方法手术治疗２５例,非手术治疗６例,手术行端端鞘膜法缝合１３例,束膜法缝合５例,神经移植４例,粘连松解３例,结果按朱家恺标准评价,优者２４例,良者６例,可者１例。随访时间最长者１０年,最短者１年。;结论熟知桡神经和肱骨干及桡骨小头的密切解剖关系和细致的手术操作加强医生的责任心,医源性桡神经损伤是安全可以避免的。手术应尽量进行,不能考虑骨折愈合与否。     

Chronic hematuria and localized bladder damage following combined cyclophosphamide and local radiotherapy.

A 13-year-old white male had Ewing sarcoma of the right pubic and ischial bones. Initial therapy consisted of 5,400 rads in seven weeks to the right side of the pelvis and 14 intravenous injections of cyclophosphamide (Cytoxan, CTX) at 500 mg/m2; BCNU and Adriamycin maintenance therapy continued for a total of two years. He has now been disease-free for five years. Three months following the completion of the right pelvic radiotherapy (RT), while on intravenous CTX, severe hematuria appeared, which subsided, but at present he has continuous microscopic hematuria, as well as periodic episodes of gross hematuria. Serial cystocopies initially revealed thickening and hemorrhagic and edematous changes on the right (irradiated) side of the bladder, and recent multiple telangiectatic patches have been demonstrated as a late "healing" phase. This case demonstrates the additive toxicity to the bladder of CTX and RT, illustrating that the hemorrhagic cystitis can be extremely protracted lasting five years.     

Iatrogenic seeding of anaplastic astrocytoma following stereotactic biopsy

We present a case of probable tumor seeding along the needle tract following computer tomography-guided stereotactic biopsy of an anaplastic astrocytoma in a 23 year old male. Six months after the initial biopsy and 3 months following a second stereotactic procedure for cyst aspiration, a second lesion appeared directly along the biopsy trajectory at a distance from the primary tumor. This lesion is presumed to be recurrent tumor and appears to have been spread iatrogenically following the biopsy of the initial tumor and the subsequent cyst aspiration along the same tract.     

外伤性和医源性上睑提肌腱膜损伤致上睑下垂

目的分析外伤性和医源性上睑提肌腱膜损伤的原因及治疗。方法对54例各种外伤及手术引起的上睑提肌腱膜损伤所致上睑下垂进行回顾性分析。结果各种眼外伤及一些医源性原因所引起的不同程度的上睑提肌损伤，造成上睑提肌腱膜与睑板分离，形成腱膜性上睑下垂，上睑皱襞过高，上眶区凹陷，二次手术行上睑提肌腱膜折叠术及断端缝合术，效果良好。结论外伤和医源性原因引起的腱膜性上睑下垂，经过手术矫正效果理想。     

Tissue necrosis under a Nance palatal arch: a case report

A Nance palatal arch is a method of reinforcing anchorage to help prevent mesial movement of maxillary first permanent molars. It is recognised that it is difficult to maintain oral hygiene around the appliance and that iatrogenic damage can occur. We present a case of a 22-year-old male patient in whom the palatal mucosa became necrotic, palatal bone became exposed and long-term periodontal damage occurred. We describe the case, his treatment and the resolution of the inflammation as well as exploring the causes for the damage and alternate treatment options.     

Hydrophilic polymer microemboli in a patient with a chronic cutaneous ulcer: a case report

Hydrophilic polymer coatings were designed to reduce friction between the catheter and vessel wall and facilitate intravascular manipulations during catheterization and placement of drug‐eluting stents. One newly reported complication of such hydrophilic coatings is the fragmentation and embolization of the polymer, which can lead to ischemia and infarct by blocking the small vasculature. In this report, we present a patient with a non‐healing ulcer on the leg. Biopsy from the ulcer revealed the presence of hydrophilic polymer emboli within the dermal vessels. This is one of the few reports of such a complication involving the skin. Our objective is to emphasize the histopathologic features of this uncommon iatrogenic phenomenon.
