接触镜与泪液形态关系的临床评价

目的 :观察配戴不同种类接触镜后眼表面泪膜的形态、稳定性及泪液量的变化。方法 :对日戴SCL、RGPCL和Ortho KCL的 2 14名近视患者 4 2 8只眼 ,利用表面泪膜观察装置 (DR 1)观察戴镜前后眼表泪液膜和戴镜状态下镜上泪液膜的形态分级 ,同时测定泪膜破裂时间 (BUT)。利用酚红染色棉丝测定SCL戴镜前后的泪液量 ,并用折射仪 (AtagoCL 1)测量SCL含水量的变化。结果 :SCL配戴者随戴镜时间延长 ,BUT明显缩短 (P <0 .0 1) ,RGPCL和Ortho KCL配戴者戴镜 5年以内 ,BUT无明显变化 (P >0 .0 5 )。戴镜后RGPCL ,Ortho KCL和非戴镜正常眼三组间眼表泪膜分级比例差异无显著性 ,SCL组Ⅰ级比例为 6 .5 % ,显著低于其他组(16 .1%～ 2 3.0 % )。镜上泪膜 ,SCL、RGPCL和Ortho KCL三组均以Ⅳ级为主 (分别占 5 4 .4 %、5 3.4 %和 5 1.5 % ) ,SCLⅠ级比例仅 1.9% ,显著低于另二组 (分别占 6 .2 %和 12 .4 % )。SCL配戴者随戴镜时间延长 ,泪液量轻度减少 ,SCL含水量在戴镜 1w后即显著降低。结论 :科学配戴接触镜对泪液膜质量无明显影响 ,但SCL长期使用可能对泪膜有一定干扰作用。     

起搏器高度依赖性患者起搏器置换方法与疗效探讨

目的 :探讨起搏器高度依赖性患者起搏器置换方法与疗效。　方法 :对 8例起搏器高度依赖性患者 ,在起搏器与电极的接口松解前 ,应用在体起搏器间断接触法 ,使自身逸搏心律得已恢复 ,然后再行松解接口以及必要的起搏参数测试。　结果 :起搏器离体次数为 3～ 7(5± 2 )次 ,最长逸搏间期为 3.38～ 6 .2 0 (4.5 1± 1.11)s,自身逸搏心率为 30～ 4 3(38± 5 )次 /min ,均顺利完成起搏器置换。　结论 :在体起搏器间断接触法切实可行 ,能够明显提高起搏器高度依赖性患者起搏器置换的安全性     

Modulation of cytokine production from an EpiOcular corneal cell culture model in response to Staphylococcus aureus superantigen

The present study investigated the hypothesis that Staphylococcus aureus enterotoxin B (SEE) produces epithelial cell death and releases inflammatory cytokines that produce stromal infiltration during contact lens induced peripheral ulceration. Epithelial cells were incubated with different doses of SEB for various time periods. Culture supernatants were assayed for cytokines IL- lo, IL-6 and chemotactic agents IL-8 and LTB,. SEE induced the production of IL- I p and IL-8. Epithelial cells exposed for longer periods (48 h) with low concentrations of SEB produced significantly higher levels (N0.02) of IL-Ip and IL-8 (P<0.05) compared t o a 24 h exposure. Contact urticaria from protein hydrolysates in hair conditioners

Protein hydrolysates (PHs) are added to hair-care products (to "repair" broken hair), soaps, bath gels, creams, etc. From one to 22 PHs used in hair-care products (collagen, keratin, elastin, milk, wheat, almond, and silk) were tested in three patient groups: A) 11 hairdressers with hand dermatitis B) 2160 consecutive adults with suspected allergic respiratory disease subjected to routine skin prick tests C) 28 adults with atopic dermatitis.
In group A, all the 22 PHs were tested with scratch and patch tests. In groups B and C, one to three PHs were tested with prick tests. Positive scratch/prick test reactions were seen in 12 patients from three PHs altogether. All were women with atopic dermatitis, and all reacted to at least hydroxypropyl trimonium hydroly:ed collagen (Crotein Q"). In three patients, prick and open tests with a hair conditioner containing Crotein Q were performed with positive results. One patient reported contact urticaria on her hands, and two reported acute urticaria on their head, face, and upper body from a hair conditioner containing Crotein Q. In seven of the eight studied sera, specific IgE to Crotein Q was detected. In conclusion, PHs of hair cosmetics can cause contact urticaria, especially in patients with atopic dermatitis.     

Comfort of fenestrated hydrogel lenses

A controlled, double-masked, randomized study was conducted on ten subjects to determine the effect of fenestration size on the initial comfort of hydrogel contact lenses. Four fenestrated lenses were tested, each lens containing four mid-peripheral fenestrations of the same size. The diameter of the fenestrations used in the four lenses ranged from 0.39 to 0.96 mm. An unfenestrated lens was also tested. All lenses were made of HEMA and were ordered with the following specifications: -3.00 D, 14.0 mm diameter, 8.4 mm back central optic radius and 0.06 mm centre thickness. There was a significant negative correlation between comfort and fenestration size, indicating that larger fenestrations are less comfortable. Even the lens with the smallest fenestrations (0.39 mm) was significantly less comfortable than the unfenestrated lens. The implication of this finding is that fenestrations may not be clinically efficacious in view of the poor comfort (and presumably increased mechanical effect of the fenestration edges on the tarsal conjunctiva) of fenestrated lenses.
