Unusual course of an epidural rhabdomyosarcoma of the upper thoracic spine

Summary This report deals with a case of rhabdomyosarcoma in the upper thoracic spine. It is of particular interest, not only for the rarity of type and location of this tumour, but for its clinical course, which presented fluctuations of neurological status, included an acute demonstration of complete paraplegia followed by full recovery after conservative treatment, and gradual relapsing of neurological deficit, one year later.     

EFFECTS OF BKCa CHANNELS ON THE REDUCTION OF CYTOSOLIC Ca2+ IN cGMP-INDUCED RELAXATION OF GUINEAPIG TRACHEA

1. In order to examine the mechanisms of cGMP-induced relaxation in airway smooth muscle, the effects of atrial natriuretic peptide (ANP) and 8-brom cGMP on muscle tone were studied by measuring isometric tension, while the effects on cytosolic Ca²⁺ concentrations were studied by measuring the spectra of fura-2 loaded in guinea-pig tracheal strips. 2. Atrial natriuretic peptide and 8-brom cGMP caused a concentration-dependent inhibition of spontaneous tone in the guinea-pig trachea. The relaxant effects of these agents on spontaneous tone were markedly suppressed in the presence of iberiotoxin (IbTX), a selective inhibitor of large-conductance Ca²⁺-activated K⁺ (BKca) channels. Iberiotoxin (30 nmol/L) markedly affected the maximal effect induced by ANP and 8-brom cGMP and augmented EC₇₀ values for ANP and EC₅₀values for 8-brom cGMP approximately 27- and 17-fold, respectively. The inhibitory effects of IbTX on relaxation induced by these agents were diminished in the presence of 1 μmol/L nifedipine, an antagonist of voltage-operated Ca²⁺channels (VOCC). 3. The inhibitory action of ANP and 8-brom cGMP on spontaneous tone was not affected by the presence of 10 μmol/L glibenclamide, an inhibitor of ATP-sensitive K⁺ channels, and 100 nmol/L apamin, an inhibitor of small-conductance Ca²+-activated K⁺ channels. When these agents were applied to tissues precontracted by high (40mmol/L) K⁺, the relaxant effects of these agents markedly diminished. 4. The extracellular Ca²⁺-dependent contraction was inhibited in the presence of 0.3 μmoI/L ANP or 0.1 mmol/L 8-brom cGMP. Concentration—response curves to extracellular Ca²⁺ (0.03—2.4 mmol/L) were markedly diminished by exposure to these agents. The maximal effect induced by extracellular Ca²⁺ was affected by these agents. 5. Atrial natriuretic peptide caused an inhibition of spontaneous tone accompanied by a reduction in the intracellular Ca²⁺ concentration. In the presence of IbTX, the elimination of both muscle tone and cytosolic Ca²⁺ by ANP was suppressed. 6. We conclude that ANP and 8-brom cGMP activate BKca channels and that the inhibition of Ca²⁺ influx through VOCC, mediated by BKca channel activation, may be involved in cGMP-dependent bronchodilation.

八十年代初中期 ,中国戏剧积极拓展我国传统戏剧的时空观、写意性特征 ,大胆借鉴西方现代戏剧的荒诞化、意象性技巧 ,出现了探索创新、多元发展的崭新局面。其中对“三一律、一堵墙”戏剧观的突破 ,运用意识形象外显人物的内心世界 ,嫁接多种艺术手段将现实时空转化为艺术时空 ,是新时期戏剧艺术探索最突出的三项成就。而过分强调形式技巧 ,脱离观众欣赏习惯 ,致使演出市场萎缩 ,观众锐减 ,无疑是新时期戏剧艺术探索中面临的最大困顿。     

The callous elbow and aging of the upper arm

The author describes his surgical approach to the problems of the aging upper arm and elbow region. Surgery at an appropriate age can prevent the progressive wrinkling of the elbow which is further aggravated by aging of the upper arm. The author's operation is aimed at correcting both portions of the arm anatomy at the same time.     

取消统编教材,开拓学术空间--关于取消中医院校统编教材之我见

中医院校的教材采用统编方式已不适合现代科技信息发展的需要，相反，取消中医统编教材，给院校和教师更多的主动权，有利于中医学新知识、新信息的传播，有利于教师参与教学研究，发挥独创精神，有利于学问的兼收并容和中医学术的发展，也有利于学生增强对专业的认识和信心，并有利于素质教育的开展。     

咽旁间隙原发性肿块流行病学分析

目的：对咽旁间隙原发性肿块进行流行病学分析。方法：对咽旁间隙原发性239例肿块进行统计分析。结果：原发良性肿瘤患病比为61．09％，其中上皮源性肿瘤患病比最高，占19.25％；神经源性肿瘤第二位，占17．57％。原发性恶性肿瘤的患病比为35．15％，其中上皮源性恶性肿瘤的患病比最高，占21.76％；恶性淋巴瘤第2位，占7．95％。炎症的患病比为3．77％，其中淋巴结反应性增生最高。结论：咽旁间隙原发性肿块良性肿瘤的患病比最高，恶性肿瘤次之，炎症居第3位，比例约为17：9：1。     

咳嗽变异型哮喘多项检测指标的Bayes判别分析

目的探讨咳嗽变异型哮喘（CVA）肺功能、气道弹性阻力、支气管激发试验、最大呼气峰流量（PEF）日内波动率[△PEFR（％）]、支气管肺泡灌洗液（BALF）细胞学以及血清IgE的变化特点及诊断意义。方法对22例CVA、20例慢性阻塞性肺病（COPD）稳定期患者以及20名健康成年人进行最大呼气流量-容积曲线（MEFV曲线）、气道弹性阻力、支气管激发试验、[△PEFR（％）]、BALF细胞学分析以及血清IgE测定。结果（1）CVA患者呼气流量指标[PEF、一秒用力呼气容积（FEV1）、FEV，／FVC、最大呼气中段流量（MMEF）]与健康对照组比较差异无统计学意义（P〉0．05）。CVA组气道弹性阻力增高，与健康对照组比较差异有统计学意义（P〈0．01），但与COPD组比较差异无统计学意义（P〉0．05）；（2）CVA组患者气道反应性明显增高，支气管激发试验阳性，Mch—PC20-FEV1明显降低、[APEFR（％）]增大（〉20％）、BALF中嗜酸性粒细胞明显增高、血清IgE明显增高与健康对照组及COPD组比较差异有统计学意义（均P〈0．01）。结论气道弹性阻力、支气管激发试验、[APEFR（％）]、BALF细胞学以及血清IgE的测定有助于CVA的诊断且与COPD等的鉴别。根据Bayes判别结果，鉴别CVA与COPD的上述病因学指标作用大小依次为Mch—PC20-FEV1、JgE、[△PEFR（％）]、BALF嗜酸性粒细胞计数、FEV1／FVC、FEV1、PEF、MMEF和气道弹性阻力，其中主要为Mch—PC20-FEV1、IgE、[APEFR（％）]、BALF嗜酸性粒细胞计数。
