小柴胡汤口服液药效作用的研究

观察了小柴胡汤口服液的主要药理作用。研究表明,小柴胡汤口服液有显著抑制角叉菜胶诱发的大鼠踝关节水肿(p<0.05),保护四氯化碳所致的大鼠急性肝功能损害,有极显著降低血清SGPT及LDH的作用(P<0.01)。对家兔发热反应也有较好的抑制作用。此外,小柴胡汤口服液对小鼠兔疫反应也有一定的增强作用,可促进小鼠碳粒廓清速率,提高血清溶血素水平及增强鸡红细胞所致的迟发性过敏反应。     

EFFECTS OF ANGIOTENSINS II AND III ON GLOMERULOTUBULAR BALANCE IN RATS

1. The role of angiotensin as a modulator of proximal glomerulotubular (GT) balance was investigated in anaesthetized rats by examining the relationship between glomerular filtration rate (GFR) and absolute proximal reabsorption (APR) during removal of endogenous angiotensin II (AII) and III (AIII) with enalaprilat (CEI) and then during their subsequent replacement by intravenous infusions. 2. Enalaprilat lowered mean arterial blood pressure (MABP) and increased renal blood flow (RBF), GFR, urine flow rate and sodium excretion. Filtration fraction (FF) was not altered. Absolute proximal reabsorption, derived from fractional lithium clearance, increased by only 48% of the change expected for 'perfect' GT balance. 3. Angiotensin II replacement corrected MABP, GFR and plasma renin level, but reduced RBF and increased FF; APR was decreased and GT balance was restored. Urine flow and sodium excretion remained above control values with AII. 4. Replacement with AIII did not correct the hypotension but completely reversed the renal and renin responses to enalaprilat and restored GT balance without affecting FF. 5. It was concluded that the relation between proximal reabsorption and GFR is considerably modified by the intrarenal angiotensin concentration. The findings are best explained by a direct stimulation of proximal tubular sodium transport by angiotensin at the concentrations existing in anaesthetized rats.     

上尿路结石致肾积水患者锂清除率的测定

采用原子吸收光谱法对４０例上尿路结石致肾积水患者进行了锂清除率测定。结果显示：梗阻所致肾积水患者的ＣＬｉ明显低于正常人（Ｐ〈０．００１），ＣＣｒ、ＣＮａ、Ｃｋ与正常对照组相比差异无显著意义（Ｐ〉０．０５）；并发现ＣＬｉ与肾积水程度呈负相关。认为可将ＣＬｉ作为发现梗阻性肾病早期肾功能损害的可靠指标，评价肾功能损害的程度和预后。     

Sputum induction as a research tool for the study of human respiratory mucus

The study objectives were to compare in vitro transportability and physical properties of respiratory mucus, obtained invasively by direct collection (DC) right after endotracheal intubation and non-invasively by sputum induction with 3% hypertonic saline solution inhalation (SI) 24 h before the anesthesia. Twenty-two patients with no pulmonary disease scheduled for elective abdominal surgical procedures were studied. The parameters analyzed and the main results are as follows. (1) Transportability by cilia (MCT), SI was higher than DC (0.94+/-0.25 and 0.62+/-0.25; P<0.001). There was a significant correlation between the two methods and DC could be estimated by: DC=0.21+(0.44 SI) (r=0.44; P<0.001). (2) Transportability by cough (CC), SI was higher than DC (68.23+/-32.1 and 33.58+/-19.04 mm; P=0.002). (3) Contact angle (CA), SI was lower than DC (10+/-3 degrees and 22+/-14 degrees ; P=0.025). (4) Rheological properties (no significant difference obtained between SI and DC). These results indicated that SI changes mucus physical properties and transportability in non-expectorators.

为了解白细胞介素 - 8的体内行为 ,用 Bolton- Hunter法对 IL- 8进行 1 2 5I标记 ,并测定它在小鼠体内的分布 ;得到了 1 2 5I- IL- 8在小鼠血、心、肝、肺、肾、骨、脾等脏器中的分布以及它在血液中的快相半排期 T1 /2α为 0 .3 2 h和慢相半排期 T1 /2β为 8.0 1h。1 2 5I- IL- 8主要通过肾排除     

Intracellular recordings from isolated rabbit retinal Müller (glial) cells

Müller (glial) cells were isolated from rabbit retinae by papaine and mechanical dissociation. The cells were fixed on a gelatine-covered glass slide by means of concanavalin A, and the slide was mounted in a perfusion chamber under a light microscope with modified optics. Besides the recording microelectrode, two other micropipettes could be adjusted with their tips near the cell. These micropipettes were used for application of test solutions into the environment of the cells. On application of high K⁺ solutions, the cell depolarized strongly but during prolonged application there was a marked repolarization. After the end of high K⁺ application the cells showed a hyperpolarization which was enhanced in both amplitude and duration with prolongation of the K⁺ exposure. Both repolarization and afterhyper-polarization disappeared under ouabain. Ouabain application itself caused a small reversible depolarization. Na⁺ free solution caused hyperpolarization. The results suggest the existence of an active membrane pump mechanism in our cells. This pump seems to be electrogenic under our experimental conditions and seems to be activated even in the absence of sodium. The cell membrane is demonstrated to contain a significant Na⁺ conductance.     

On the interaction between phenytoin and digoxin

Summary An open, randomized, single-blind cross over trial to investigate phenytoin-digoxin interactions at steady state was performed in 6 healthy male volunteers. Coadministration of phenytoin caused a significant reduction in the elimination half-life of digoxin from 33.9 to 23.7 h and a diminution in AUC_0–48 from 31.6 to 24.4 ng · ml^–1 · h. Renal digoxin clearance was not significantly altered from 135.7 to 120.3 ml · min^–1. Assuming no change in -acetyldigoxin absorption, the in decrease time-course the serum digoxin concentration was due to a significantly increased total digoxin clearance from 258.6 to 328.3 ml · min^–1. An insignificant reduction in the digoxin distribution volume from 749.4 to 668.0 l was also observed. No relevant change in the pharmacokinetic parameters (elimination half-life, area under the serum concentration time-curve, protein binding) of phenytoin was observed when phenytoin and digoxin were co-administered. The data suggest that with this drug combination the serum digoxin concentration should be carefully monitored and, if necessary, the daily digoxin dose should be increased.     

Hepatic elimination of drugs with concentration-dependent serum protein binding

For a drug with concentration-dependent serum protein binding, the unbound fraction of drug decreases during the drug elimination process. The clearance of the drug at a given blood flow rate is lower than would be expected from the observed unbound fraction in venous blood from a noneliminating organ. Based on both the well-stirred and parallel tube models, simulations demonstrated that consideration of concentration-dependent binding during drug elimination is important when the intrinsic clearance is higher than the blood flow and when the unbound drug concentration is much greater than the dissociation equilibrium constant of the binding complex.Supported in part by Grant GM 28423 from the National Institutes of General Medical Sciences, NIH.     

血必净注射液联合利奈唑胺治疗重症肺炎有效性及安全性的Meta分析

目的 评价血必净注射液联合利奈唑胺治疗重症肺炎的有效性与安全性。方法 检索中国知网（CNKI）、万方数据库（Wanfang）、维普数据库（VIP）、中国生物医学文献数据库（CBM）、Cochrane Library、PubMed、Web of Science和Embase数据库,检索时限为各数据库建库至2022年6月,收集血必净注射液联合利奈唑胺治疗重症肺炎的随机对照试验,采用Review Manager 5.4.1对纳入文献进行质量评价、数据整合分析和偏倚风险评估;Stata14.0进行敏感性分析。结果 共纳入12项随机对照试验,涉及949例患者,观察组478例,对照组471例。Meta分析结果显示,观察组的临床总有效率[RR=1.24, 95%CI (1.18, 1.31), P<0.000 01]、细菌清除率[RR=1.38, 95%CI (1.22, 1.55), P<0.000 01]显著高于对照组。观察组的血常规恢复正常时间[MD=-1.38, 95%CI (-1.55,-1.20), P<0.000 01]、体温恢复正常时间[MD=-1.68, 95%C...
