Sysmex~(TM)K—1000全自动血液分析液对白细胞分类的评价

本文对198份静脉血标本分别用Sysmex~(TM)K—1000和显微镜进行白细胞分类,用直线回归和相关系数进行相关性分析,两种方法中性粒细胞、淋巴细胞、中间细胞的相关系数(依次用r_N、r_L、r_M表示)分别为:①无旗标r_N=0.9630,r_L=0.8655,r_M=0.4057;②WL出现r_N=0.9141,r_L=0.8629,r_M=0.4781;③T_2出现r_L=0.9225;④T_1出现为仪器不分类情况。上述r_N、r_L相关性非常显著,r_M相关性较低。中间细胞假阳性较多,假阴性较少。结果表明,K—1000可作为对白细胞分类的筛选手段,但其性能不够完善,尚不能完全取代显微镜检查,在特殊情况下仍须进行人工分类。     

EP9-A在血液分析仪比对中的应用

目的应用正常人新鲜全血对多台血液分析仪进行比对,使血液分析仪各参数之间具有可比性。方法我们根据美国临床实验室标准化委员会（NCCLS）标准文件EP9-A的要求,以WBC、RBC、HGB、PLT作为比对项目;以BAYER ADVIA120作为参比仪器对待评仪器SYSMEX XT-1800进行比对分析。结果SYSMEX XT-1800的WBC、RBC、H6B、PLT四项指标的相关系数r都〉0.975,在医学决定水平处的系统误差（Bc）均小于规定的允许总误差（Ea）范围。结论为保证检验结果的准确、一致,当在同一实验室内具有不同品牌、不同型号的分析仪,或用不同仪器、不同测定方法测定同一检测项目时必须要进行对比分析。     

Measurement uncertainty of platelet concentration using the Sysmex XN automated hematology analyzer

We estimated the measurement uncertainty (MU) of platelet concentration measured using the Sysmex XN system with two reference platelet counting methods described by DIN 58932-5 (PTB method) and the International Council for Standardization in Haematology (ICSH method). Ten blood samples were used to estimate and compare the MU of the XN system, and 30 samples were used to compare the methods. The standard uncertainty of the reference method was significantly higher for the ICSH method; the PTB method showed higher platelet concentrations than the ICSH method. When applying different methods with the XN system, optic counting showed higher MU compared to the other methods. There was good correlation among the two reference methods and three automated platelet-counting methods. We evaluated the MU in platelet concentrations measured using an automated hematology analyzer. Our results suggest that using the PTB method for calculating MU of the automated hematology analyzer is superior to the ICSH method because of its lower standard uncertainty.     

UF1000i尿液分析仪性能验证结果的探讨

目的对UF1000i全自动尿沉渣仪进行性能评价。方法采用新鲜小便标本，对尿沉渣仪的不精密度（批间）、线性、携带污染率、正确度等性能进行验证。结果不精密度（高值、低值）红细胞为3．7％、8％，白细胞为114％、5．1％，管型为6．4％、15．6％，细菌为5．4％、8．9％。白细胞在68．5～1182．5μL，线性范围内的相关系数为0．99。白细胞、红细胞与细菌携带污染率符合要求。正确度符合要求。结论全自动尿沉渣仪性质量能评价各项指标结果与厂家声明一致，在工作中定期进行各种性能检查，以保证仪器高速检测、结果可靠。     

Underestimation of the coexistence of iron deficiencies and thalassemia minors: a single institution experience in Taiwan

Some physicians neglect the possible coexistence of an iron deficiency with a thalassemia minor and do not treat the iron deficiency accordingly. This motivated us to conduct this study. We retrospectively reviewed the records of 3892 patients who visited our clinics and had hemoglobin (Hb) electrophoreses performed in our hematologic laboratory from August 1, 2007 to December 31, 2012. The thalassemia minors were identified by characteristic complete blood count (CBC) parameters obtained from an autoanalyzer and Hb electrophoresis, and some cases were confirmed with molecular tests. Then, we checked iron studies [ferritin and/or serum iron with total iron-binding capacity (TIBC)] to determine the coexistence of an iron deficiency with a thalassemia minor and a response to iron, if such treatments were given. We found 792 cases with thalassemia minors, and excluded those without iron studies, with 661 cases as our sample. A total of 202/661 cases (31%) also had iron deficiencies. They had lower red blood cell (RBC) counts, Hb, and ferritin levels as compared to those thalassemia minor cases without coexistence of iron deficiencies. We concluded that the thalassemia minor patients did not have iron overload complications in our population. On the contrary, iron deficiencies commonly coexist in the clinical visits. We propose that if Hb < 11.5 g/dL in a case of thalassemia minor, one should screen for iron deficiency simultaneously. The sensitivity is 79.8% and the specificity is 82.6%. Therefore, physicians should be aware of this coexisting condition, and know how to recognize and treat it accordingly.     

Improvements to alpha-particle spectrometry techniques

Improvements to the conventional methods for alpha-particle spectrometry measurement and analysis have recently been implemented in our laboratory. They include the application of corrections for energy drift in long-duration measurements, the development and application of an efficient deconvolution method for complex alpha spectra using the new computer code ALFITeX, an alpha–gamma coincidence system using a dual-parameter multichannel analyzer, and the digitization of this coincidence system.     

OLYMPUS AU600全自动生化分析仪的机械臂维护

笔者介绍了在工程师的指导下将 OL YMPUS AU 6 0 0全自动生化分析仪加样针的机械部分拆开进行清洗除尘。首先 ,要将仪器电源关闭 ,这样可以使机械臂完全放松下来。接下来 ,要将仪器的部分配件拆下来 ,主要是加样针的保护盖及仪器的后盖。第三步 ,则是由别人来帮忙 ,上下拉动加样针的机械臂 ,一个人在仪器的后面用干净的纱布将机械臂螺纹钢擦干净。之后 ,再用吸管给螺纹钢上一点润滑油。最后 ,再将拆下来的部分安装上去。这样一来 ,机械臂的阻力则大为减小 ,仪器也能正常工作了
