Base anatomique d'un nouveau concept de stabilité du carpe

Résumé Le concept de stabilité du carpe repose sur l'anatomie et la biomécanique. Celles-ci nous permettent de constater que le lunatum est le centre de deux axes fonctionnels: l'un longitudinal, mécanique, l'autre transverse, dynamique. Le lunatum est fermement stabilisé au radius et au complexe ligamentaire cubital par le V proximal: il devient ainsi un élément porteur; il est de plus fermement uni aux os voisins, le scaphoideum et le triquetrum, avec lesquels il forme une unité fonctionnelle intercalaire différenciée. Le guidage de cette unité est assuré par le V distal et le ligament radio-triquetrum dorsal.

Summary Based on anatomical and biomechanical evidence, we conclude that the stability of the carpus relies on the lunate bone which is the centre of the longitudinal as well as the transverse functional axis. The lunate is firmly stabilised proximally to the radius and the radio-ulnar ligament complex by the proximal V-ligament, and so becomes a weight bearing structure. It is anchored by strong ligaments to the scaphoid on one side and the triquetrum on the other, thus forming a 3-bone intercalary system, which is in turn guided by the palmar distal V-ligament and the dorsal radio-triquetral ligament.

     

沈阳市某农村社区居民健康状况分析

目的：了解沈阳市农村社区居民健康状况及主要健康问题,为农村健康促进工作提供基础资料。方法：采用随机抽样的方法,选择东陵区某中等经济水平农村社区18岁以上成年人进行一般状况调查及健康体检。结果：本社区居民超重和肥胖、高血粘、血压异常、脂肪肝、咽炎、尿路感染、女性阴道炎检出率较高,分别为48.0%,41.5%,33.8%,27.1%,20.7%,7.0%,47.5%。结论：该社区需加强健康教育和健康促进为主导的综合干预措施,进一步促进农村社区人群健康状况。     

乳腺导管扩张症的诊断与鉴别(附6例临床病理报道)

本文对6例乳腺导管扩张症进行了临床病理报道,6例术前皆被误诊乳腺其他疾病,根据此种情况作了如何正确诊断的阐述.提出了常见误诊病例如何鉴别。     

Personal experience with the procurement of 132 liver allografts

A single donor surgeon's experience procuring the livers from 132 donors is described. Thirty-seven grafts (28.9%) had hepatic arterial anomalies, 19 (14.4%) of which required arterial reconstruction prior to transplantation. Of the 121 grafts evaluated for early function, 103 grafts (85.2%) functioned well, whereas 14 grafts (11.6%) functioned poorly and 4 grafts (3.3%) failed to function at all. The variables associated with less than optimal function of the graft consisted of donor age (P<0.05), duration of donor's stay in the intensive care unit (P<0.005), abnormal graft appearance (P<0.05), and such recipient problems as vascular thromboses during or immediately following transplantation (P<0.005). A new preservation fluid, University of Wisconsin solution, allowed safe and longer cold storage of the liver allograft than did Euro-Collins' solution (P<0.0001). A parameter of liver allograft viability, which is simple and predictive of allograft function prior to the actual transplant procedure, is urgently needed.     

化疗药物外渗后局部组织损伤处理的实验研究

目的根据化疗药物外渗后局部组织损伤的特点寻找理想的处理方法。方法制作化疗药物外渗的动物坏死模型,分别采用临床常用的三种处理方法进行动物实验,进行肉眼观察及组织学观察。结果1∶5 000呋喃西林加季德胜蛇药外敷对化疗药物外渗所致的局部组织坏死疗效最佳,其次为复方利多卡因,50%硫酸镁外敷疗效甚微,对照组无效。结论临床处理化疗药物外渗应根据其局部特点选择合理有效的方法。     

《野叟曝言》创作人格批判

《野叟曝言》的“文格”与夏敬渠的创作人格有一致性，但存在明显的、不可忽视的矛盾，这种矛盾对《野叟曝言》的文本价值产生了极复杂的影响。导致夏敬渠“人格”与《野叟曝言》“文格”矛盾的原因是多方面的，研究视角不能简单化、绝对化。     

下颌骨侧位摄影X线中心线倾斜角度与垫板倾斜角度关系之探讨

目的探讨下颌骨侧位摄影分别采用一倾斜角度为α的垫板及未采用时，X线中心线倾斜角度各为β、γ之间的关系，为更简捷地拍好下颌骨提供理论参数和实践依据。方法立足于教科书，运用X线摄影原则与数学几何原理进行推理演绎。结果二者之间存在着可替换的关系，即γ=α＋β，可以大胆地去除垫板进行直接拍摄。结论不用垫板，只需通过增大X线中心线倾斜角度的方法，同样也能拍摄出一张好照片，且可操作性强，简便易行，实际使用价值更大。     

An explanation of the noncorrespondence between assessment methods of cyclosporin

Various methods of determining cyclosporin (CyA) levels in patients after kidney transplantation were compared. These included polyclonal antibody (pcAb-), specific and nonspecific monoclonal antibody (S- and NmcAb-) radioimmunoassays (RIA), and high performance liquid chromatography (HPLC). The results obtained by the various methods when compared showed some correlation but did not correspond. A probable explanation for part of this noncorrespondence is the presence of monoclonally crossreactive metabolites (CyA-M). Another reason was that the concentration of CyA in the standards supplied with the RIA kits was found to be higher than stated.     

Effect of electric and chemical stimulation of the raphe obscurus on phrenic nerve activity in the cat

The effect of electrical and chemical (l-glutamate) stimulation of the raphe obscurus on phrenic nerve activity was examined in the cat. Phrenic nerve activity was recorded from a C5 nerve root in anesthetized, paralyzed and artificially ventilated cats. Neural discharge was quantitated by integrating the phrenic nerve activity. The respiratory frequency was determined from the integrated nerve signal. Focal electrical stimulation (18–144 μA; 5–40 Hz; 100 μs pulse duration) resulted in significant (P < 0.05) increases in both integrated phrenic nerve (IPN) amplitude and respiratory frequency. These changes were dependent upon current intensity and frequency of stimulation. The largest increases in IPN amplitude and respiratory frequency were47 ± 17%and146 ± 8%, respectively. To insure that the changes in integrated phrenic nerve activity (IPNA) were the result of stimulation of cell bodies and not axons of passage,l-glutamate (100, 200 nmol) was microinjected (100 nl) into the raphe obscurus. Significant (P < 0.05) dose-related changes occurred in integrated phrenic nerve amplitude with an increase of44 ± 13% at 100 nmol and80 ± 13% at 200 nmoll-glutamate. No significant increase in respiratory frequency was observed withl-glutamate microinjection. The results suggest that the raphe obscurus may be involved in respiratory control.     

个性化护理干预对妇科恶性肿瘤手术后生活质量的影响

[目的]探讨个性化护理干预对妇科恶性肿瘤手术后生活质量的影响。[方法]将46例在我科因妇科恶性肿瘤手术的患者随机分为实验组和对照组，通过实施个性化护理干预，对两组患者进行SF-36量表和Olson婚姻质量问卷进行测评。[结果]对妇科恶性肿瘤手术后患者进行个性化护理干预后其生活质量明显提高，差异有显著性意义（P〈0．01）。[结论]个性化护理干预能提高妇科恶性肿瘤手术后生活质量。
