Background:
Controversies exist in the literature regarding the management of complex fractures of the calcaneum. We evaluated a series of complex fractures of the calcaneum managed by ligamentotaxis using Joshi''s external stabilization system (JESS) for its efficacy.Materials and Methods:
Forty-five patients having complex (comminuted, intra-articular fracture with compromised soft tissue condition) fractures of the calcaneum, who were treated by external fixator (JESS) based on the principle of ligamentotaxis. The gradual distraction was done to bring the articular margins together to maintain both alpha and beta angles to near normal range. Thirteen (28.9%) patients underwent additional corticocancellous bone grafting with elevation of posterior facet. All patients were evaluated for their functional outcomes by American Orthopedic Foot and Ankle society (AOFAS) Score for the ankle and hind foot. Mean duration of follow-up was 20.5 months.Results:
Forty-two (93.4%) of our patients did well with the ligamentotaxis. On evaluating final outcomes by AOFAS, approximately 71% of cases showed good results. Eleven patients (24.4%) complained of persistent heel pain in the long-term follow-up. Out of these, eight (17.8%) patients were those who had severe comminution with almost total loss of calcaneal height. The origin of heel pain was not the subtalar joint in all of these patients.On long-term follow-up none of these patients suffered from such severe pain so as to compel them to change the nature of their activity.Conclusion:
We conclude that ligamentotaxis by JESS provides a viable and user-friendly alternative method of management of these complex calcaneal fractures. 相似文献992.
Background:
Over the years, there has been a tremendous increase in the use of fluoroscopy in orthopaedics. The risk of contracting cancer is significantly higher for an orthopedic surgeon. Hip and spine surgeries account for 99% of the total radiation dose. The amount of radiation to patients and operating surgeon depends on the position of the patient and the type of protection used during the surgery. A retrospective study to assess the influence of the radiation exposure of the operating surgeon during fluoroscopically assisted fixation of fractures of neck of femur (dynamic hip screw) and ankle (Weber B) was performed at a district general hospital in the United Kingdom.Materials and Methods:
Sixty patients with undisplaced intertrochanteric fracture were included in the hip group, and 60 patients with isolated fracture of lateral malleolus without communition were included in the ankle group. The hip and ankle groups were further divided into subgroups of 20 patients each depending on the operative experience of the operating surgeon. All patients had fluoroscopically assisted fixation of fracture by the same approach and technique. The radiation dose and screening time of each group were recorded and analyzed.Results:
The radiation dose and screening time during fluoroscopically assisted fixation of fracture neck of femur were significantly high with surgeons and trainees with less than 3 years of surgical experience in comparison with surgeons with more than 10 years of experience. The radiation dose and screening time during fluoroscopically assisted fixation of Weber B fracture of ankle were relatively independent of operating surgeon''s surgical experience.Conclusion:
The experience of operating surgeon is one of the important factors affecting screening time and radiation dose during fluoroscopically assisted fixation of fracture neck of femur. The use of snapshot pulsed fluoroscopy and involvement of senior surgeons could significantly reduce the radiation dose and screening time. 相似文献993.
Nowadays in cases of nonunions of the femoral neck, the surgeon is tempted to perform prosthetic replacement of the hip, more so if there is also evidence of avascular necrosis of the head of femur. This provides rapid pain relief and allows early mobilization. However, long-term results of hip arthroplasties, especially in younger people and in the presence of osteopenia, are not always as expected; and a less radical approach is worth considering. The intertrochanteric valgization osteotomy, described by Pauwels, is an excellent alternative for healthy patients up to 65 years of age with a nonunion of the femoral neck. A union rate of 80-90% of the nonunion is described by most authors. Leg length inequallity, rotational and angular deformities can be corrected at the same time. During the period 1973-1995, we performed valgization osteotomy according to Pauwels in 66 patients of, 18-72 years old (mean 49.5 years). 24 (37%) of our patients died 4 months to 24 years (mean: 9.5 years) after the operation. Union of the femoral neck was achieved in 58 (88%) of the 66 patients; union of the osteotomy in 65 patients (99%). A good or excellent result was achieved in 62% (23 uneventful and 13 with healed, necrosis/arthrosis without need for further treatment) of our patients. However, the method has its limits. We feel if there is too little bone stock inside the femoral head, a valgization osteotomy does not give good result. The radiographic signs of avascular necrosis in patients over 30 years of age is considered a contraindication for an osteotomy. However our results show that it is worthwhile trying to save the joint of young patients even in case of a segmental collapse. In the race between revascularization and collapse, often revascularization is the winner. We deliberately give nature its chance and don''t rely on the result of bleeding from drill holes in the head, nuclear scans and other methods to estimate vascularity. A secondary total hip replacement if necessary because of avascular necrosis or osteoarthritis is considerably postponed; and better milieu for hip replacement can be achieved by the development of sclerotic bone in the subchondral areas of the acetabulum and femoral head. Between 65 and 80 years of age, a total hip replacement is probably the best option for fit patients. We treat fresh femoral neck fractures with a hemiarthroplasty in patients over the biological age of 80 years. Logically the same choice will be made for patients with a nonunion. During the period 1973-1995 we performed hemiarthroplasty (n = 34) in patient with low general condition. Their mean age was 79 years. The average survival in these patients was less than three years and that explains probably the low late complication rate: in this group. Total hip replacement was performed in 37 younger patients with a mean age of 69 years. They were not considered for a valgization osteotomy because of age being over 70 years, severe osteoporosis or a total collapse of the femoral head. Background:
The treatment of displaced intracapsular femoral neck fracture is still an unsolved problem. Non-union and avascular necrosis are the two main complications of this fracture, especially if patient presents late. Muscle pedicle bone grafting has been advocated to provide additional blood supply. We present analysis of our 32 cases of displaced femoral neck fracture treated by internal fixation and quadratus femoris based muscle pedicle bone grafting.Materials and Methods:
Open reduction and internal fixation with muscle pedicle grafting was done in 32 patients. The age of patients varied from 14-62 years (average age 45 years) with male to female ratio of 13:3. Twenty-nine fractures were more than three weeks old. All the cases were treated by Meyers'' procedure. The fracture was internally fixed after open reduction and then a muscle pedicle graft was applied. It was supplemented by cancellous bone graft in seven cases. Fixation was done by parallel cancellous lag screws (n = 19), crossed Garden''s screws (n = 7), parallel Asnis screws (n = 5) and Moore''s pin (n = 1).Quadratus femoris muscle pedicle graft was used in 32 cases. In the initial 12 cases the graft was fixed with circumferential proline sutures, but later, to provide a secure fixation, the graft was fixed with a cancellous screw (n = 20). Postoperative full weight bearing was deferred to an average of 10 weeks.Results:
Union was achieved in 26/29 (89.65%) cases which could be followed for an average period of 3.4 years, (2-8.5 years) with good functional results and had the ability to squat and sit cross-legged. Results were based on hip rating system given by Salvatti and Wilson. The results were excellent in 15 cases, good in four cases, fair in four cases and poor in six cases. Complications were avascular necrosis (n = 2), transient foot drop (n = 2), coxa-vara (n = 1) and temporary loss of scrotal sensation (n = 1).Conclusion:
Muscle pedicle bone grafting with internal fixation is a viable treatment option in displaced femoral neck fractures with late presentation. 相似文献995.

Many patients undergoing total hip resurfacing arthroplasty present with reduced anterior femoral neck concavity as a part of the degenerative process. Painful hip impingement may develop or persist after hip resurfacing when the retained femoral neck abuts against the metallic acetabular component or the anterior acetabular bony wall. We report on two cases of painful postoperative hip impingement after hip resurfacing, in one of whom anterior neck-contouring osteoplasty restored pain-free range of motion (ROM). To restore natural hip ROM, surgeons performing hip total hip resurfacing arthroplasty should aim to reproduce the normal femoral head-neck offset ratio. Femoral head-neck offset restoration can be achieved by proper femoral component positioning (especially optimal translation), by femoral neck osteoplasty or by increasing femoral component head size appropriately. 相似文献

Summary In a 12-year prospective study of 100 adult volunteers, incident vertebral fractures were analyzed for potential predictors.
Decreased sagittal spinal curvature and pre-existing fractures were identified as independent predictors. The risk of vertebral
fracture increased to 8.6-fold with a 1 SD decrease in both thoracic and lumbar curves.
Introduction Physiological spinal curvature acts as a shock absorber: however, it has not been demonstrated whether reduced spinal curvature
is a risk of vertebral fracture. The aim of this prospective study is to assess the relationship between the extent of sagittal
spinal curvature and incidence of vertebral fractures.
Methods One hundred community-based volunteers aged 50+ years were followed radiologically for at least 10 years. Entire spine radiograms
of volunteers in erect position were used for evaluating sagittal spinal curvatures, grade of osteoporosis, and incident vertebral
fractures.
Results Mean baseline age was 61.9 years and mean follow-up period was 12.0 years. In multivariate analysis, preexisting vertebral
fracture (relative risk [RR] 3.70, 95% confidence interval [CI] 1.22-11.20), and a decrease in either thoracic or lumbar curvature
by 1-SD (RR 3.06, 95% CI 1.04-9.00) were independent predictors. The fracture risk even increased in the presence of a 1-SD
decrease in both thoracic and lumbar curvature (RR 8.64, 95% CI 1.39-53.78).
Conclusions Reduction of physiological curvatures in both thoracic and lumbar spine led to the increased risk of vertebral fracture by
more than eightfold. The importance of evaluating sagittal spinal alignment should be emphasized, because it might indicate
the risk of the first vertebral fracture, which is another strong predictor of subsequent fractures. 相似文献

Summary A fracture risk assessment tool (FRAX™) is developed based on the use of clinical risk factors with or without bone mineral
density tests applied to the UK.
Introduction The aim of this study was to apply an assessment tool for the prediction of fracture in men and women with the use of clinical
risk factors (CRFs) for fracture with and without the use of femoral neck bone mineral density (BMD). The clinical risk factors,
identified from previous meta-analyses, comprised body mass index (BMI, as a continuous variable), a prior history of fracture,
a parental history of hip fracture, use of oral glucocorticoids, rheumatoid arthritis and other secondary causes of osteoporosis,
current smoking, and alcohol intake 3 or more units daily.
Methods Four models were constructed to compute fracture probabilities based on the epidemiology of fracture in the UK. The models
comprised the ten-year probability of hip fracture, with and without femoral neck BMD, and the ten-year probability of a major
osteoporotic fracture, with and without BMD. For each model fracture and death hazards were computed as continuous functions.
Results Each clinical risk factor contributed to fracture probability. In the absence of BMD, hip fracture probability in women with
a fixed BMI (25 kg/m2) ranged from 0.2% at the age of 50 years for women without CRF’s to 22% at the age of 80 years with a parental history of
hip fracture (approximately 100-fold range). In men, the probabilities were lower, as was the range (0.1 to 11% in the examples
above). For a major osteoporotic fracture the probabilities ranged from 3.5% to 31% in women, and from 2.8% to 15% in men
in the example above. The presence of one or more risk factors increased probabilities in an incremental manner. The differences
in probabilities between men and women were comparable at any given T-score and age, except in the elderly where probabilities
were higher in women than in men due to the higher mortality of the latter.
Conclusion The models provide a framework which enhances the assessment of fracture risk in both men and women by the integration of
clinical risk factors alone and/or in combination with BMD.

Summary The short-term association between wrist-fracture history and future fracture has not been simultaneously compared between
younger and older postmenopausal women. This 3-year follow-up study of 158,940 women showed a similar future fracture risk
in younger and older women with wrist-fracture history.
Introduction We examined the association between prior wrist fracture and future osteoporosis-related fractures within 3 years in younger
and older postmenopausal women.
Methods In the National Osteoporosis Risk Assessment (NORA) study, 158,940 postmenopausal women, aged 50-98 (median 63) years, provided
information on fracture history since age 45, and responded to follow-up surveys 1 or 3 years later when new fractures were
queried. Cox regression models were used to obtain relative risk (RR) and 95% confidence interval (CI) estimates.
Results Of the 158,940 participants, 8,665 reported a history of wrist fracture at baseline; 4,316 women reported at least one new
fracture within three years. The RR for any subsequent clinical fracture, adjusted for covariates and baseline BMD T-score,
was 2.4 (2.0, 2.9) for younger and 2.1 (1.9, 2.3) for older women. A prior wrist fracture increased the risk of a future wrist
fracture about 3-fold and doubled the risk of any osteoporotic fracture.
Conclusions Prior wrist fracture strongly predicts three-year risk of any future osteoporotic fracture for older and younger postmenopausal
women, independent of baseline BMD and common osteoporosis risk factors. More consideration should be given to evaluating
and managing osteoporosis in younger and older women with a history of wrist fracture, independent of their BMD. 相似文献
