目的:探讨高频超声在手部肌腱断裂缝合术中的应用价值。方法选择2009年3月~2013年6月在东阳市横店集团医院手外科就诊并术前进行超声定位的98例手部肌腱断裂患者为实验组,2014年1~6月就诊未行超声定位的48例患者为对照组,分别记录两组患者手术时间,术后高频超声探查肌腱修复效果和评估术后手指康复情况,并进行统计分析。结果两组最终完成全部超声监测检查的为实验组76例、对照组40例。实验组平均手术时间明显少于对照组[(30±20)min比(60±30)min],差异有高度统计学意义(P<0.01);实验组术后腱性连接率高于对照组[(72.4豫,55/76)比(52.5豫,21/40)],实验组瘢痕形成率低于对照组[(27.6%,21/76)比(47.5%,19/40)],差异均有统计学意义(P<0.05);实验组TAM手部功能康复评估良好率高于对照组[(86.8%,66/76)比(67.5%,27/40)],差异有统计学意义(P<0.05)。结论采用高频超声可明显降低手术时间和术后瘢痕连接,术后高频超声持续监测肌腱修复水平可明显提高术后手部功能康复。 相似文献

In this study, the silica fume replacement rate, fly ash replacement rate, and curing temperature were regarded as the independent variables, and the compressive and flexural strengths were regarded as the response values. The response surface method was used to construct the response surface polynomial regression model and obtain the optimal preparation parameters of a steel slag cement-based gel slurry (SCGS). The univariate and multivariate effects on the SCGS’s strength were investigated via analysis of variance and a three-dimensional surface model, and the hydration products and strength development law were characterized via scanning electron microscopy and X-ray diffraction. The actual compressive strengths at 3 and 28 d of age were 31.78 and 53.94 MPa, respectively, which were close to the predicted values (32.59 and 55.81 MPa, respectively), demonstrating that the optimized strengths were accurate and reliable. Further, the hydration reaction rate of SiO2 in the silica fume and the physical filling effect of the inert components of fly ash and steel slag under the optimal parameters were the key factors for the early strength of the material. Moreover, continuous C3S hydration in steel slag and the continuous excitation of the volcanic ash properties of fly ash were important factors for the later strength. 相似文献

The thermomechanical interaction of the tool with the chip in the most loaded secondary cutting zone depends on the contact length of the tool rake face with the chip. Experimental studies of the dependency of the contact length on the cutting speed, the undeformed chip thickness, and the tool rake angle, performed by the optical method, are used for comparison with the contact length obtained by the FE modeling of the orthogonal cutting process. To determine the parameters of the constitutive Johnson–Cook equation, which serves as a material model of the FE cutting model that has a predominant influence on the contact length, a software-implemented algorithm was developed. This algorithm is based on determining the generalized parameters of the constitutive equation through finding the intersection of these parameter sets. The plurality intersection of the parameter sets of the constitutive equation is determined by means of the design of experiments and refined by subsequent multiple iterations. The comparison of the contact length values, obtained by simulating the cutting process using the generalized parameters of the constitutive equation as a material model with their experimental values, does not exceed 12% for a wide range of cutting speeds and depths of cut, as well as for the tool rake angle. 相似文献

目的:观察及探讨卵巢癌患者卵巢血流参数及组织微小RNA(MiR)、组织激肽释放酶(KLK)的变化情况。方法:选取本院收治的66例卵巢癌患者为观察组,另以同期的66例卵巢良性肿瘤患者为对照组,然后将两组的卵巢血流参数及组织MiR、KLK表达情况进行比较,比较观察组中不同分期卵巢癌患者的上述检测项目。结果:观察组的卵巢血流收缩期峰值流速(PSV)高于对照组,阻力指数(RI)及搏动指数(PI)均低于对照组,组织MiR表达低于对照组,KLK则高于对照组,且观察组中不同分期卵巢癌者的上述检测结果之间比较,差异均具有统计学意义(P均〈0.05)。结论:卵巢癌患者卵巢血流参数及组织MiR、KLK的变化较大,且上述指标对于卵巢癌分期也有较高的诊断价值。 相似文献

【摘要】 目的 建立含有随机序列与miniCMV融合的启动子,驱动抗生素耐药基因的表达;用于筛选和寻找不同处理条件下的反应性元件。方法 设计针对Puromycin抗性基因,即嘌呤霉素N 乙酰转移酶基因(puromycin N acetyl transferase gene, PAC)的引物,扩增目的条带并通过PacI+Pme1限制性内切酶克隆至目的载体pWPI;然后,设计并合成接头序列+20N随机序列+ miniCMV的融合片段,通过PCR的方法得到双链序列,并进行酶切,通过Cla1+PacI克隆入前述pWPI PAC载体。克隆得到的载体组合即为慢病毒反应元件筛选报告载体文库。在此基础上,我们将报告载体文库转染细胞,并进行超声照射。超声照射剂量为10min,100mW/cm2。每天一次,一共照射3天。每次超声照射后,对细胞进行puromycin处理。4天后,收集超声照射存活细胞,提取DNA,PCR扩增含有20N的序列并进行测序鉴定。结果 我们成功设计了一套文库构建方案,初步构建了20N随机序列+ miniCMV +Puromycin抗性基因的文库。将该文库转染细胞,超声照射后可以增加转染有对超声反应克隆细胞中抗性基因表达;我们通过获取Puromycin处理后的存活细胞,发现了潜在的对超声反应的调控元件。结论 我们构建的20N随机序列+ miniCMV +Puromycin抗性基因文库,可以用于筛选对超声等特定条件有表达调控反应的顺式反应元件;进而为该元件控制基因表达提供依据。 相似文献

Human gait is a procedure of learning and maturation; the steady, normal, adult gait pattern is considered to be established
after the age of 11 years. This study presents the results accumulated for a group of 20 able-bodied children (10 male) aged
9–10 (mean age 9.5, SD=0.5), who were tested with a portable system using telemetry for temporal factors. These results can
serve as reference for the assessment of possible orthopedic disorders in children with similar ages. For the parameter of
single support, a systematic difference between the two legs was found for runs with normal speed, which opposes to the symmetric
gait pattern of a mature able-bodied population; this difference seemed to disappear in fast gait. However, for the double
support, insignificant differences between the two legs were observed in both normal and fast gaits, which opposes to other
findings for a similar group of children. Last, the single step duration was significantly different between the two feet
only in normal gait.

Accidental ingestion of magnetic foreign body in a pediatric patient: A potentially fatal attraction
Foreign body ingestion is one of the most common pediatric emergencies. As part of their cognitive development, infants and toddlers are extremely curious and constantly explore their surroundings through their senses, namely taste. The ubiquity of toys containing magnetic elements consecutively meant an increase in the cases of children ingesting said magnets. While most ingested foreign bodies, including a single magnet, will spontaneously traverse the gastrointestinal tract without problems, some may give rise to grave and potentially life-threatening complications; the latter is often seen in the presence of 2 or more magnets or paramagnetic material. The diagnosis of ingestion of magnetic foreign bodies remains a challenge, given its often ambiguous history and presentation; nonetheless, their abundance, gravity, and preventability of their complications alone should render physicians vigilant and keep a low threshold of suspicion. 相似文献
