Health insurance and private providers: A study of the civil servants' medical benefit scheme in bangkok,Thailand

Compulsory health insurance schemes have received attention recently as a means of creating a reliable extra-budgetary source of health care funding. Yet, the full implications of such schemes in a developing country context are unclear; in particular, the impact on and relationship with private sector providers has infrequently been explored. This study examines patterns of public and private sector utilization under the Civil Servants' Medical Benefit Scheme (CSMBS) in Bangkok, Thailand. The CSMBS currently provides limited reimbursement for private inpatient care, but recent proposals suggest increasing benefit levels for care sought in the private sector. The study shows that despite high level of cost recovery in the public sector, charges were much lower than those in the private sector. Different patterns of diagnosis in the two sectors were found with private hospitals tending to treat a less complex case mix. Within the private sector, there was a limited tendency to specialize in certain types of care. It is concluded that under the current payment mechanism of fee-for-service reimbursement, measures to enhance access to private sector care should be approached with caution. In the long run, the scheme should merge with the recently established social security scheme.     

屈膝过程中的胫股运动特性分析

本文采用三维位移测量方法，测试１０例离体人膝标本的胫股关节三维运动特性。发现屈膝过程中，胫股运动也具有典型的三维运动特点。其中在屈膝初期的３０°内，胫骨内旋最明显并伴有胫骨内翻现象，平均内旋８°，内翻６－７ｍｍ，膝关节完全伸直时胫股扣锁使膝关节稳定，屈曲时胫股“解锁”使膝关节松弛，具重要的临床意义，同时提示，在解释传统的胫股扣锁现象时不应遗漏伴随发生的胫骨内／外翻现象。     

论神经在腧穴针感形成中作用

腧穴针感在针刺防治疾病过程中发挥着重要作用。本文从针刺作用的感受器,腧穴针感的外周传入神经,以及针感与中枢神经的关系,诸方面,列举大量实验结果和理论,阐明针感形成的物质基础是神经系统。     

润滑油抗泡机理的研究 Ⅱ.助泡剂对泡沫倾向的影响及抗泡剂配伍性能

考察了各类添加剂在不同浓度、温度及不同基础油中的泡沫倾向,并测定了这些条件下体系的表面张力。从表面化学角度解释了添加剂对泡沫性质的影响,并通过对抗泡剂配伍性能的研究,验证了消泡机制:抗泡剂须具备高的表面活性才能吸附于泡膜表面,降低泡膜的表面粘弹性,改变膜的流变性能,从而起消泡作用。     

Analysis of a decreased Na+ conductance by tumor necrosis factor in identified neurons of Aplysia kurodai.

The ionic mechanisms of the effect of extracellularly ejected recombinant human tumor necrosis factor-alpha (rhTNF-alpha) on the membrane of identified neurons R9 and R10 of Aplysia kurodai was investigated with conventional voltage-clamp, micropressure ejection, and ion substitution techniques. Micropressure-ejected rhTNF caused a marked hyperpolarization in the unclamped neuron. Clamping the same neuron at it resting potential level (-60 mV) and reejecting rhTNF-alpha with the same dose produced a slow outward current [Io (TNF)] associated with a decrease in input membrane conductance. Io (TNF) was decreased by depolarization and increased by hyperpolarization. The extrapolated reversal potential of Io (TNF) was approximately +10 mV. Ion substitution and pharmacological experiments suggest that Io (TNF) in identified neurons R9 and R10 of A. kurodai is due to a decreased Na+ conductance but not due to an activation of the Na(+)-K+ pump. Our results demonstrate that the immunomodulator TNF can act directly on the nervous system as well as on the immune system.     

病理性近视眼的研究进展

通过复习近年来相关文献,对近视眼发生发展机制与形觉剥夺之间的关系进行综合评述。认为近视眼的发生及眼轴延长与形觉剥夺有一定关系,深入研究其相关机制,对近视眼的早期防治具有重要意义。     

浅析《内经》之闭经证治

收集了《内经》有关闭经方面的相关阐述从而探讨闭经的病因病机、诊断与治疗,并详细记述了《内经》对血滞经闭和血枯经闭这两类型经闭的不同治疗法度。     

通过原生质体融合获得青霉素高产菌株

采用两株分别带有不同抗药性标记的产黄青霉原生质体以1:1相混合,用30％的聚乙二醇(MW6000)原生质体融合处理。将混和物涂布在补给有两种药物同时存在的高渗性分离培养基上,培养后获得异核体菌落。洗下异核体分生孢于继续分离在补给有两种药物同时存在的分离培养基上,得到了同时带有两种抗性标记的重组体。纯化后,进行抗性稳定性和摇瓶效价的测定,将一株具有双重抗性、摇瓶发酵效价较高的菌株作出发菌株,经诱变因子处理后,获得了一株43u7c—57菌株,在摇瓶中效价比生产菌株平均提高4.94％,该菌株在40吨发酵罐试生产后,发酵效价平均提高4.4％,产量则平均增加12.15％。     

膨胀型聚氨酯防火涂料阻燃机理的研究

用自制的新型成炭剂和聚磷酸铵复配作为聚氨酯防火涂料的阻燃体系。采用热失重分析、扫描电镜、红外光谱和元素分析等手段对膨胀型聚氨酯防火涂料的阻燃机理进行了较为深入的研究．     

Pulsatility and resistance indices in intrarenal arteries of normal adults

In order to establish the normal range of values of Pulsatility (PI) and Resistance (RI) Indices in the intrarenal vasculature, a study of 50 healthy volunteers (23 males, 27 females), divided into five groups of 10 according to age, was performed with Duplex Doppler ultrasound. Both kidneys were examined in all individuals and, in 12, indices were also compared between upper and lower poles of both kidneys. In addition, repeat examinations were performed in nine subjects on three different days, in order to assess the reproducibility of the method. No differences were found in the mean values of both indices between males and females, upper and lower poles, right and left kidneys. A statistically significant increase (p < 0.01, unpaired t-test) was demonstrated when the oldest age group (7th decade) was compared to the youngest age group (3rd decade). The method appeared remarkably reproducible for RI (4.2–7%), with wider variation in the PI (9.5–22.7%).
