目的 探讨体外循环 (CPB)中应用乌司他丁 (UTI)对机体肾脏的影响。方法 择期CPB下心内直视术 40例 (ASAⅡ~Ⅲ级 )分为 2组 :乌司他丁组 (U组 ,n =2 0 ) ,于麻醉后、CPB开始前将UTI 2 0万U溶于 2 0ml 0 9%生理盐水中静脉注射 (约 10min) ;若CPB时间超过 4h ,可在CPB开始 4h后追加 1次 ,剂量方法同前。对照组 (C组 ,n =2 0 )不用UTI。分别于麻醉前及CPB结束 3h后检测血Bun、Cr及尿NAG、RBP、α1-MG、γ -GTP ,统计尿量并计算排尿量 /输液量。结果 麻醉前 :两组病人间血Bun、Cr和尿NAG、RBP、α1-MG、γ -GTP及排尿量均无显著性差异 (P >0 0 5 )。CPB结束后 :⑴两组病人其尿γ -GTP、NAG均分别高于麻醉前( P <0 0 5 ) ,但U组二者均分别低于C组 ( P <0 0 5 )。⑵两组病人其尿RBP、α1-MG均分别明显高于麻醉前 ( P <0 0 1) ,但U组二者分别低于C组 (P <0 0 5 )。⑶两组病人其排尿量均明显多于麻醉前 (P <0 0 1) ,但U组高于C组 (P <0 0 5 )。⑷U组病人其排尿量 /输液量高于C组 (P <0 0 5 )。结论 UTI对CPB下心内直视病人的肾脏功能有较强的保护作用 相似文献

以Wistar雄性大鼠为受体,建立慢性肾功能衰竭动物模型,将鼠婴肾组织声多点植入受体双侧后肢皮下和筋膜下。结果表明,30天后移植物的体积由1mm^3增至4mm^3大小,表面血管网丰富;光镜下见肾小球、肾小管结构正常。促红细胞生成素(EPO)着色颗粒主要分布在肾小球区,移植组着色程度明显增高。血红蛋白和4促红细胞生成素随移植的时间延长而逐渐升高,实验结果提示,此方法有可能为治疗慢性肾功能衰竭性贫血提 相似文献

The precise cause of allograft dysfunction after renal transplantation often cannot be established by non-invasive means. In clinical practice, radionuclide scans form an integral part of the clinician's armamentarium in the assessment of these patients [1, 2]. Unfortunately, in the clinical setting more than one pathological process may be responsible for the impaired function, making it difficult to correlate the scan appearances with the pathology. Extracorporeal shock wave lithotripsy (ESWL) is standard therapy for urolithiasis. With comparable technical principles, various
lithotripters have been developed and are in routine use. Renal pelvic stones, calyceal stones, ureteral stones, and other
special forms can be treated with varying results. Currently, the so-called clinically insignificant residual fragments and
the recurrence of calculi are under discussion. Whereas the side effects of ESWL are well known, studies comparing ESWL with
other endourological procedures are still lacking.

It is documented that alkaline phosphatase (AP) plays an important role in bone mineralization. Considering that TN-AP is
expressed in periodontal ligament fibroblasts, renal epithelial cells, and vascular endothelial cells, and that TN-AP is both
a calcium-/phosphate-binding protein and a phosphohydrolytic enzyme, we hypothesize that membrane-bound AP also plays an important
role in the initiation of physiological and pathological mineralizations in tissues other than bone and cartilage. To test
this hypothesis, nonosteoblast cell lines, including a fibroblast line, a renal epithelial line, and a capillary endothelial
line, were stably transfected to express high levels of rat bone AP on their cell surfaces. These rat bone AP-expressing cells
were then cultured on filter membranes in the presence or absence of β-glycerol phosphate. von Kossa staining for calcium
phosphate and transmission electron microscopy with electron diffraction analysis for minerals were employed to investigate
the effect of membrane AP on extracellular calcium phosphate mineralization. Our results indicated that AP expression on these
nonosteoblast-like cell surfaces have induced extracellular hydroxyapatite (HAP) mineralization. Our findings support the
concept that membrane-bound AP contributes to extracellular apatitic mineralization by mechanisms that do not necessarily
involve its hydrolase activity. They also suggest that AP might be important for the initiation of pathological mineralization
in nonosteogenic tissues.

The modifying potential of two nephrotoxic agents, harman and norharman, on N-ethyl-N-hydroxyethylnitrosamine (EHEN)-induced renal and hepatic carcinogenesis was investigated in male F344/DuCrj rats. Animals were given 0.1% EHEN in their drinking water for the first 2 weeks as an initiator. Subsequently, starting 3 weeks from the commencement, they were fed diet containing these compounds at concentrations of 1000, S00 or 0 ppm until week 26, and then killed for light microscopic examination. The mean numbers of renal tubular cell hyperplasias/cm2 and those of tumors/cm2 in rats given harman and norharman at 1000 ppm after initiation, but not at 500 ppm, were significantly increased as compared to the control values. However, neither compound modified liver carcinogenesis. It is concluded that harman and norharman show enhancing effects on rat kidney carcinogenesis, when ingested at dose levels which cause renal tubular damage. 相似文献

目的:探讨选择性肾动脉栓塞治疗肾损伤的优越性及其临床应用。材料与方法:同时使用两种栓塞材料——可吸收的明胶海棉条和美国COOK公司产38-3-5弹簧栓子,治疗了两例肾剌伤患者,两例均为男性,年龄分别为20和35岁。结果:两例患者肾刺伤后均经受过保守治疗,治疗时间分别为14和22天,但未奏效。经超选择性肾内动脉分支栓塞、取得了十分满意的止血效果,后随访3~4年,患者一切感觉正常。结论:选择性肾动脉栓塞治疗肾刀刺伤后肾出血是一种安全、可靠、有效的方法。 相似文献

This report of the North American Pediatric Transplant Cooperative Study summarizes data contributed by 57 participating centers on 754 children with 761 transplants from 1 January 1989 to 16 February 1989. Data collection was initiated in October 1987 and follow-up of all patients is ongoing. Transplant frequency increased with age; 24% of the patients were less than 5 years, with 7% being under 2 years. Common frequent diagnoses were: aplastic/dysplastic kidneys (18%), obstructive uropathy (16%), and focal segmental glomerulosclerosis (12%). Preemptive transplant, i.e., transplantation without prior maintenance dialysis, was performed in 21% of the patients. Dialytic modalities pretransplant were peritoneal dialysis in 42% and hemodialysis in 25%. Bilateral nephrectomy was reported in 29%. Live-donor sources accounted for 42% of the transplants. Among cadaveric donors, 41% of the donors were under 11 years old. During the first post-transplant month, maintenance therapy was used similarly for live-donor and cadaver source transplants, with prednisone, cyclosporine, and azathioprine used in 93%, 83%, and 81%, respectively. Triple therapy with prednisone, cyclosporine, and azathioprine was used in 78%, 75%, and 75% of functioning cadaver source transplants at 6 months, 12 months, and 18 months as opposed to 60%, 63%, and 54% for live-donor procedures, with single-drug therapy being uncommon. Rehospitalization during months 1–5 occurred in 62% of the patients, with treatment of rejection and infection being the main causes. Additionally, 9% were hospitalized for hypertension. During months 6–12 and 12–17, 30% and 28% of the patients with functioning grafts were rehospitalized. Times to first rejection differed significantly for cadaver and live-donor transplants. The median time to the first rejection was 36 days for cadaver transplants and 156 days for live-donor transplants. Overall, 57% of treated rejections were completely reversible although the complete reversal rate decreased to 37% for four or more rejections. One hundred and fifty-two graft failures had occurred at the time of writing, with a 1-year graft survival estimate of 0.88 for live-donor and 0.71 for cadaver source transplants. In addition to donor source, recipient age is a significant prognostic factor for graft survival. Among cadaver donors, decreasing donor age is associated with a decreasing probability of graft survival. Thirty-five deaths have occurred; 16 attributed to infection and 19 to other causes. Abstract: Pyridoxalated-hemoglobin-polyoxyethylene conjugate (PHP), which is made from out-dated human red blood cells by two major chemical modifications, namely pyridoxalation and conjugation with polyoxyethylene (POE), is currently under development as a physiological oxygen carrier. This study assessed the effects of PHP-88 solution, which contains 8% (wt/vol) each of hemoglobin (Hb) and maltose, on renal function when it was infused 3 times every other day into the intact circulation of 8 dogs (5 dogs for the PHP group and 3 for the control group; 20 ml/kg for the first infusion, and 10 ml/kg each for the second and third infusions, at the rate of 2.5 ml/h/kg). Serial determinations of glomerular filtration rate (GFR) and renal plasma flow (RPF) were carried out pre- and postinfusion for up to 3 months along with measurements of blood and urine analyses, urine output rate, fractional excretion of sodium (FES), and free water clearance (CH2O). The results showed that plasma colloid osmotic pressure (COP) elevated an average of 3.3 mm Hg (p = 0.0085), and GFR and RPF tended to increase by 13% (NS) and 38% (NS), respectively, immediately after the third infusion with PHP solution. Urine output rate increased during and after the infusion, and FES and CH2O also increased for 24 h after the infusion in both groups. Blood urea nitrogen, serum creatinine, and serum Na+ concentrations were not affected greatly by the infusions, but hematocrit was decreased by 8% in the PHP group, indicating approximately a 42% expansion of plasma volume. These changes were observed to return to their preinfusion levels by 1 week postinfusion. Renal histology of the PHP group obtained at 2 weeks postinfusion revealed vacuole formation in the proximal tubules which was not associated with any pathologic changes indicative of cell death or regeneration. In 4 out of 5 dogs at 3 months postinfusion (necropsy), the vacuoles were not present. Though urinary N-acetyl β-glucosaminidase (NAG) activity had significantly increased after infusion, it returned to the preinfusion level by 1 month postinfusion. No detrimental effect of vacuoles on the assessed renal tubular functions was confirmed in the present study. The result demonstrated that multiple infusions of PHP solutions were well tolerated in normal dogs, and the observed effects were conceived predominantly attributable to the physiological response of the kidneys to an oncotic load into the circulation, which produced plasma volume expansion. 相似文献
