目的:观察情景互动结合器械辅助核心肌群训练对改善偏瘫患者步行功能的疗效。方法:将45例存在步行功能障碍的脑卒中后偏瘫患者随机分成对照组(A组)、器械辅助核心训练组(B组)和情景互动结合器械辅助核心训练组(C组)各15例。所有患者均给予常规神经内科药物治疗,A组接受常规的运动疗法(包含常规核心肌群训练),B组接受常规运动疗法和器械辅助核心肌群训练,C组接受常规运动疗法和情景互动结合器械辅助核心肌群训练,3组均每日训练45min,每周治疗6d,共4周。治疗前后采用下肢Fugl-Meyer运动功能量表(Fugl-Meyer assessment,FMA)、10米最大步行速度测试和Holden步行功能测试对患者的步行功能进行评估。结果:治疗前,3组患者下肢FMA、10MWT和Holden步行功能分级相比差异无统计学意义。治疗4周后,3组患者下肢FMA、10MWT和Holden步行功能分级均较治疗前明显提高(P<0.05);B、C组的下肢FMA、10MWT和Holden步行功能分级提高均优于A组(P<0.05);C组各项评分提高程度更优于B组(P<0.05)。结论:对于脑卒中后偏瘫患者,在常规运动疗法的基础上,情景互动结合器械辅助核心训练比起单纯器械辅助核心肌群训练能更加有效地改善患者步行功能,值得临床推广应用。 相似文献

目的:探讨个体化的桥式运动强化训练对脑卒中偏瘫患者肢体运动功能、平衡能力和ADL能力的影响。方法:对80例发病2-12周内的脑卒中偏瘫患者,随机分成常规运动治疗组(A组)和个体化桥式运动强化训练组(B组),两组的常规内科治疗相同,分别于入组时、治疗8周末,采用简化Fugl-Meyer运动功能评分、Fugl-Meyer平衡功能评分和改良Barthel指数进行评定。结果:治疗8周后B组的各项评分明显高于A组(P<0.05)。结论:个体化桥式运动强化训练较常规运动治疗能更显著地改善脑卒中偏瘫患者肢体运动功能、平衡能力和日常生活活动能力。 相似文献

Purpose:
Cerebral hemiatrophy is a common childhood disease. It clinically manifests with seizures, hemiparesis and mental retardation.Materials and Methods:
In this prospective study, previously untreated patients with seizures and cerebral hemiatrophy were recruited. Cerebral hemiatrophy was diagnosed on the basis of hemispheric ratio. Patients with acquired hemiconvulsion, hemiplegia, and epilepsy (HHE) syndrome were included in group A. Group B included patients with congenital HHE syndrome. Patients were followed up for 6 months for seizure recurrence.Results:
Out of 42 patients 26 were in group A and 16 were in group B. After 6 months, there was significant reduction in seizure frequency (P < 0.0001) in both the groups. At least 50% reduction in seizure frequency was noted in all the patients. Complete seizure freedom was observed in 15 (35.7%) patients. Seizure recurrences were significantly higher (P = 0.008) in group A. On univariate analysis, predictors of seizure recurrences were history of febrile seizures (P = 0.013), hippocampal sclerosis (P = 0.001), thalamic atrophy (P = 0.001), basal ganglia atrophy (P = 0.001), cerebellar atrophy (P = 0.01), ventricular dilatation (P = 0.001), epileptiform discharges at presentation (P = 0.023), complex partial seizures (P = 0.006) and status epilepticus (P = 0.02). On multivariate analysis, hemispheric ratio was the only significant factor for seizure recurrence.Conclusion:
Patients with congenital hemiatrophy had better seizure control than that in patients with HHE syndrome. 相似文献996.

ObjectiveThe aim was to find a sensitive method to highlight the remodeling of the brain’s bioelectric activity in post-stroke repair.MethodsFifteen mild upper limb paretic stroke patients and age-matched healthy controls were included. Repeated trials of finger tapping around the 10th and 100th days after stroke onset were recorded with a 128-channel EEG. Power spectra and Inter Trial Coherence (ITC) calculations were synchronized to tappings. ITC was correlated with motor performance.ResultsITC, in low frequency bands, designates the motor related bioelectric activity in channel space in both healthy subjects and patients. Ten days after stroke onset, delta-theta ITC was severely reduced compared to baseline, while three months later ITC reorganized partially over the ipsilesional central-parietal areas reflecting the improvement of motor networks. Decreased ITC in the central-parietal area remained significant compared to controls. Delta band ITC over the dorsolateral-prefrontal cortex correlates with the performance on Nine Hole Peg Test. At post-recovery, non-paretic hand tappings show significantly decreased delta-theta ITC over the supplementary motor area, which reflects network remodeling.ConclusionsInter Trial Coherence is a useful measure of brain reorganization during stroke recovery.SignificanceDelta- theta ITC is a sensitive indicator of impaired motor execution. 相似文献

目的 探讨高频超声诊断卒中患者偏瘫侧肩关节病变,分析偏瘫侧肩痛的可能病因,为进一步治疗提供依据.方法 选择首次发病的卒中偏瘫患者93例,进行肩关节高频超声检查,分为肩痛组和无肩痛组,比较两组肩关节病变情况.同时应用改良Ashwonh评分及Brunnstrom评分分别评定上肢肌张力及其运动功能.结果 93例中,超声发现异常75例,占80.7%,主要病变包括肩袖损伤(25例),滑囊炎、滑囊积液(31例),肩关节半脱位(28例),肱二头肌长头腱及腱鞘病变(50例).两组比较,在肩袖损伤,肩关节半脱位,滑囊炎、滑囊积液,肱二头肌长头腱及腱鞘积液的发病率上有统计学差异(x2=8.67,P<0.01;x2=6.99,P<0.01;x2 =13.44,P<0.01;x2 =3.87,P<0.05),两组在年龄、性别、偏瘫侧、中风类型、病程、上肢肌张力和上肢运动功能评分等均无统计学差异.结论 高频超声可用于诊断卒中患者偏瘫侧肩关节的病变,为指导康复治疗提供依据. 相似文献

Mutations in ATP1A3 have been found to cause rapid‐onset dystonia Parkinsonism, alternating hemiplegia of childhood, epileptic encephalopathy and other syndromes. We report a four‐year, nine‐month‐old boy with episodes of frequent and recurrent status epilepticus, who first began having generalized tonic‐clonic seizures at four months of age. Development was normal until the age of four months, and markedly slowed down after the onset of seizures. Between the age of seven months and two and a half years, the patient had recurrent attacks of unilateral and bilateral hemiplegia. At the age of 21 months, after a febrile illness with status epilepticus, he regressed and developed continuous severe dystonia and bradykinesia with superimposed intermittent painful dystonic spasms. Extensive neurological and genetic workup revealed a de novo p.V589F ATP1A3 mutation (NM_152296.5:c.1765G>T, NC_000019.9:g.42482344C>A). This is a novel mutation associated with a novel phenotype that shares features with epileptic encephalopathy, alternating hemiplegia of childhood, and rapid‐onset dystonia Parkinsonism. 相似文献

ABSTRACT Background: Pro-inflammatory cytokines may be associated with post-stroke depression (PSD); however, results from different studies are inconsistent. Objectives: To investigate whether pro-inflammatory cytokines are associated with the development of PSD in acute stroke. Methods: PubMed, Embase, and Web of science were searched for relevant literature. Meta-analyzes were performed to determine whether the baseline blood concentrations of pro-inflammatory cytokines differed between acute stroke patients with and without depression. Sensitivity analyzes and regression analyzes were conducted to explore sources of heterogeneity. Results: We included 889 acute stroke patients from eight original studies, 312 of whom developed PSD and 577 did not. The serum concentrations of interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α) were higher in the PSD group, compared with the non-PSD group (IL-6: SMD = 1.26, 95% CI = [0.55, 1.97], P < 0.001; TNF-α: SMD = 0.61, 95% CI = [0.13, 1.10], P = 0.010). Conclusions: This study indicates IL-6 and TNF-α as potential biomarkers of PSD in the acute stage of stroke and provides theoretical support for the early prevention and treatment of PSD. 相似文献
