Denial of Human Rights: We Must Change the Paradigm of Dementia Care

In far too many instances treatment of persons with dementia has reflected a fundamental denial of basic human rights. At times, these individuals are treated worse than the treatment of animals when the five basic freedoms of animals, described by Pachana in her editorial, are implemented. A number of such examples of dehumanizing (and “de-animalizing”) persons with dementia are presented. A case is made for the position that this is the direct result of the “medicalization” of dementia and “Alzheimer Disease.” This has led to the disenfranchisement of persons with dementia and their caregivers regarding the treatment of dementia, while medical “expertise” has led to a paradigm of learned helplessness while waiting for “the cure.” While the medicalization of dementia has been a financial success in terms of funding failed researcher to find a cure, it has been a catastrophe for the quality of life of persons with dementia and their caregivers. It is time to take control of the treatment of dementia back, and especially to listen to the voices of persons with dementia. It is time to take action NOW – to become disruptive to the current paradigm. The emperor and his cure have no clothes. We deserve better. We must make this change in paradigm our mission, to demand it, and to accept nothing less. Power to the people.     

多力视角下社会资本参与基层卫生服务的动力机制及优化策略研究

社会办医作为我国基层卫生服务的重要组成部分,可以弥补基层卫生机构发展总量与质量不足等问题,对于推进整个基层卫生服务高质量发展也是有着重大裨益。文章从物理学的"力"出发,运用支持力,推力,引力,阻力,摩擦力五种"力"探讨社会资本参与基层卫生服务的动力形成机制,并构建动力斜坡图,针对存在问题,提出加大支持力和引力,合理利用推力,减少摩擦力,消除阻力等优化建议,以期助力社会资本参与基层卫生服务建设平稳进行。     

临床实习生对患者隐私的认知及保护情况调查

目的了解临床实习生对患者隐私的认知以及在临床实践中对患者隐私的保护情况,探讨患者隐私教育及临床实习中存在的问题,分析成因并提出加强患者隐私教育、引导实习生合理获取患者隐私的具体建议,以改善临床医学教育过程中存在的薄弱环节。方法随机选取海军军医大学等5所医学院校的572名临床医学实习生为调查对象,发放调查问卷。主要内容包括实习生隐私教育评分、对于隐私保护法及隐私具体范围的知晓情况、临床实践中是否泄露隐私、隐私与临床诊疗等方面,调查结果采用SPSS 21.0软件进行分析。结果27.6%实习生曾发生过泄露患者隐私的情况,58.9%实习生不了解隐私相关法律法规及隐私具体范围,95.4%实习生认为出于诊疗目的而获取患者隐私信息很有必要,但仍有56.6%实习生曾回避涉及患者隐私的医疗活动。结论临床医学实习生的教育中患者隐私教育相对欠缺并有待进一步加强。实习生应当自觉保护患者隐私,维护患者利益,并树立对于患者隐私的合理认知,从患者隐私中提取有价值的信息。     

北京地方志医学资料挖掘浅析

地方志作为"地方性的百科全书",因其全面性而往往为专业研究如医学研究所忽略,而其中却不乏丰富的医学资料。通过对北京地方志中的医学资料进行挖掘,发现涉及太医院、官办医药机构、社会福利机构、医家、流行病、土产药物、药王庙等内容,并对相关文献进行分析,以展现北京地方志医学资料情况,为后期进一步研究奠定基础。     

社区老年患者拔牙的临床特点分析

目的：通过对近两年社区卫生服务中心327例老年病人拔牙临床观察，探讨基层社区老年病人的特点。方法：通过心理、精神和技术方面的配合，安全顺利地完成拔牙术。结果：除5例病人术后感头晕外，其余均顺利完成且病人满意。结论：社区卫生服务中心除具备熟练的技术外，医患之间心理沟通是必不可少的。     

深低温对全脑缺血性损伤的保护作用

国际上把低温划分为轻度低温(33～35℃),中度低温 (28～32℃),深度低温(17～27℃)和超深度低温(2～16℃)。低温已经被大量动物试验和临床实践证实具有脑保护作用,但同时也存在心律失常,凝血功能障碍,免疫抑制等全身多系统副作用,产且温度越低,副作用越明显。日前国内外已经有大量的试验证实深低温对全脑缺血的脑保护作用,它能明显增加脑组织对缺血的耐受性,本文将就深低温对全脑缺血性损     

Framing the child at risk

Risk Assessment Reports are one form of accounting practice which delineate the space of risk considered in a child abuse neglect investigation. The paper analyses two Risk Assessment Reports, contrasting what is reported, what is directly observed and what is hypothesized and implied by child protection practitioners during the assessment of risk. From an analysis of these two reports (each 4 – 5 pages long), the paper identifies the rhetorical strategies and their realizations used by two practitioners in framing the child at risk. The paper problematizes the tension between the institutional need to constrain the exercise of professional judgement through Risk Assessment Models, and the extent to which practitioners actually localize the framing of risk to specific areas of investigation.

原北京医科大学(现为北京大学医学部)从20世纪90年代初开始,积极寻求与美国中华医学基金会的合作以促进医学伦理学的学科发展和国际化水平的提高。十几年来,北京大学医学部作为基地和依托极大地支持和引领了我国医学伦理学的教学改革与发展,增强了相关人员的医学伦理意识以及医学伦理学理论对实践的指导作用,促进了伦理学知识和能力的培训,提升了我国生物医学科研伦理的实力,奠定了医学伦理学继续教育的基础,使医学伦理学实现了系统性和跨越式的发展。
