Objective. Highly sensitive and specific assays of cardiac troponins I and T are the preferred biomarkers in diagnosing myocardial infarction (MI). Assays of cardiac troponin I (cTnI) have been improved with the addition of antibodies against the cTnI molecule and may have increased sensitivity. We hypothesized that a cTnI assay with modern antibody configuration will exhibit equal or better sensitivity in the setting of acute MI compared to cTnT and other markers of myocardial necrosis. Material and methods. We investigated release kinetics of cTnI (Abbott ADV, Abbott Diagnostics), cTnT (Roche Diagnostics), CKMBmass, myoglobin and heart fatty acid binding protein (H‐FABP) in 23 patients admitted with acute ST‐segment elevation MI undergoing primary percutaneous coronary intervention. Calibrators for the Abbott ADV cTnI assay are traceable to the United States National Institute of Standards and Technology (NIST) reference material for cTnI. Eleven blood samples were drawn from each patient in the period from admission to 24?h. Biomarkers of necrosis showed marked increases in relative concentrations, especially within the first 2?h after admission. Results. From 30?min after admission onwards, cTnI exhibited significantly higher relative concentrations compared to cTnT, CKMBmass, Myoglobin and H‐FABP (p<0.05). Conclusions. The NIST standardized Abbott TnI ADV assay appears to be more sensitive than cTnT and other biomarkers in the early phase of MI.  相似文献

目的探讨心肌损伤标志物对诊断新生儿缺氧缺血性脑病(HIE)患者心肌损伤的临床意义。方法对不同程度HIE患儿和健康新生JL(对照组)分别测定血清cTnI、CK—MB和Mb浓度。结果3组HIE患儿心肌损伤标志物均增高,其中以重度患儿最高,中度患儿次之,轻度患儿增高较少。组间比较差异均有统计学意义(P〈0.05)。HIE患儿出生第1天均以Mb异常率最高,3d后Mb均持续下降,阳性持续时间短,而CK—MB和cTnI却升至峰值,并维持在较高水平达1周以上。结论HIE患儿脑损伤越重,心肌损伤也越重;联合检测心肌损伤标志物有利于心肌损伤的早期诊断、程度评估及预后判断。  相似文献

Effects of iron deficiency and exercise on myoglobin in rats   总被引:5,自引:0,他引:5  
Summary The effects of iron deficiency and endurance training on muscle myoglobin (Mb), body weights, and blood lactic acid concentration were studied in rats. Fifty animals were divided into four groups: anemic trained (AT), normal trained (NT), anemic sedentary (AS), and normal sedentary (NS). Following 5 weeks of dietary control, the mean hemoglobin values for the AT and AS rats were 0.013±0.002 mmol·l–1 (8.7±1.4 g·dl–1) and 0.014±0.003 mmol·l–1 (9.2±1.7 g·dl–1) respectively, and did not significantly change throughout the study. AT and NT rats were run on a motor driven treadmill 4 days/week for 6 weeks up to a pre-established time of 90 min. Following the training, body weights of the AT (157±13 g) and NT (153±13 g) rats were lower than their respective sedentary groups AS (172±9 g) and NS (176±15 g). Resting blood lactic acid concentration following training was lower in both trained groups, AT (3.3±2.0 mM) and NT (2.3±1.9 mM) compared to AS (8.2±2.6 mM) and NS (3.8±1.6 mM). Training increased Mb concentration in hearts of both the anemic and normal trained groups (AT, 0.66±0.13 mg·g–1; NT, 0.95±0.08 mg·g–1) compared to the sedentary groups (AS, 0.44±0.08 mg·g–1; NS, 0.70±0.13 mg·g–1). Only the AT rats showed an increase in skeletal muscle Mb. This study provides evidence that myoglobin may limit aerobic metabolism.  相似文献

目的探讨心肌肌钙蛋白I(cTnI)、肌红蛋白(Mb)在急性心肌梗塞早期诊断中的价值。方法对胸痛发作后4~6h的27例急性心肌梗塞(AMI)患者,25例健康者,28例非急性心肌梗塞患者的血清心肌肌钙蛋白I(cTnI)和肌红蛋白(Mb)进行测定。结果AMI组血清cTnI值明显高于健康组及其它对照组,差异有显著性(P<005)AMI组Mb值显著高于正常值,除骨骼肌损伤组Mb有明显开高外,其它对照组测定值均在正常范围以内,与AMI组有明显差异(P<001)。cTnI及Mb的灵敏性明显高于CK-MB。结论cTnI是诊断AMI较理想的指标,同时测Mb可对AMI的早期诊断提供一种较CK-MB更为经济、快速、可靠的生化指标,具有临床应用价值。  相似文献

Haemoglobin and myoglobin are haem proteins that play a key role as they help transport oxygen around the body. However, because of their chemical structure, these molecules can exert harmful effects when they are released massively into the bloodstream, as reported in certain pathological conditions associated with rhabdomyolysis or intravascular haemolysis. Once in the plasma, these haem proteins can be filtered and can accumulate in the kidney, where they become cytotoxic, particularly for the tubular epithelium, inducing acute kidney failure and chronic kidney disease. In this review, we will analyse the different pathological contexts that lead to the renal accumulation of these haem proteins, their relation to both acute and chronic loss of renal function, the pathophysiological mechanisms that cause adverse effects and the defence systems that counteract such actions. Finally, we will describe the different treatments currently used and present new therapeutic options based on the identification of new cellular and molecular targets, with particular emphasis on the numerous clinical trials that are currently ongoing.  相似文献

目的 评价联合快速检测心肌损伤标记物在急性心肌梗死 (AMI)中的早期诊断价值。方法 采用美国PMB公司的综合诊断试剂盒快速测定 4 7例AMI患者在不同时期的CK MB、Mb、TnI,同时用生化法测定CRP和CK MB ,进行统计学比较。结果  4种标记物早期 (≤ 6h)诊断灵敏性以血清Mb最高 ,CK MB最低。在AMI发病 3~ 7天 ,TnI阳性率非常显著高于其它 3种 (P <0 0 0 5 )。早期快速测试法测定CK MB的阳性率显著高于酶法 (P <0 0 5 ) ,而早期CRP阳性率与酶法CK MB相比亦有非常显著的差异 (P <0 0 1 )。结论 联合快速检测心肌损伤标记物在AMI早期诊断中具有重要临床价值  相似文献

目的:探讨肌红蛋白(Myoglobin,Mb)在早期心肌梗死诊断中的价值。方法:对86例心肌梗死患者在发病6h内检测Mb、心电图、CK-MB。结果:心电图特异性改变为55.81%,CK-MB阳性率为13.95%,而Mb阳性率为74.42%,心肌梗死检出率明显高于其他两种方法,差异有统计学意义(P<0.05)。结论:Mb能早期诊断AMI,对AMI的早期治疗及预后均有重要意义,值得临床推广应用。  相似文献

Globins are respiratory proteins involved in oxygen metabolism, which is a critical factor in tumor growth and progression. The status of neuroglobin and myoglobin is largely unknown in human malignancies, including lung cancer. The aim of this study was to explore mRNA expression profiles, potential regulatory mechanisms and clinicopathological associations of neuroglobin and myoglobin in non-small cell lung cancer (NSCLC). We screened 208 surgically resected NSCLC specimens and a panel of lung normal and cancer cell lines. The mRNA expression of neuroglobin, myoglobin and hypoxia markers (HIF1α and VEGFa) was measured with qRTPCR, while neuroglobin promoter methylation was assessed with Pyrosequencing. Neuroglobin and myoglobin were upregulated in the tumor samples compared to normal tissue (p = 1.3 × 10−22 and p = 1.9 × 10−9, respectively). Neuroglobin was more frequently overexpressed in squamous cell carcinomas (SqCCL) than adenocarcinomas. Overexpression of myoglobin was more profound in adenocarcinomas, which correlated with poor survival (p = 0.013). Neuroglobin promoter was hypermethylated in 30.8% of NSCLC cases, which correlated with neuroglobin mRNA downregulation. The epigenetic regulation of neuroglobin was confirmed by treating lung cell lines with 5′azadeoxycytidine and/or trichostatin A. Expression of both genes correlated with the expression of HIF1α (neuroglobin: p = 3.8 × 10−5, myoglobin: p = 1.1 × 10−11). Myoglobin expression was also associated to that of VEGFa (p = 2.1 × 10−7). Hypoxia-dependent upregulation of both globins was validated in vitro. In summary, neuroglobin and myoglobin overexpression in NSCLC is associated with histological subtype, hypoxia and, in case of neuroglobin - epigenetic regulation. Myoglobin expression may have potential significance in the prognostication of lung adenocarcinomas.  相似文献

A sensitive method for the determination of picogram level dibutyl phthalate (DBP) in wine by flow-injection chemiluminescence (FI–CL) analysis is presented for the first time, which was based on the quenching effect of DBP on the luminol–myoglobin (Mb) CL system. The decrement of CL intensity was linearly proportional to the logarithm of DBP concentration in the range of 0.1–100 pg mL−1 with the detection limit of 0.03 pg mL−1 (3σ). At a flow rate of 2.0 mL min−1, a complete determination of DBP including sampling and washing could be accomplished in 0.5 min, giving the maximum sample throughput of 120 h−1. The proposed method was successfully applied to the determination of DBP in wine, human serum and urine samples with the relative standard deviations (RSDs) of less than 3.0% (n = 5). The molecule docking results showed that DBP interacted with the amino acid residues near the heme moiety of Mb. The possible CL mechanism of luminol–Mb–DBP reaction should be that the binding of Mb with DBP forming a 1:1 complex (binding constant K = 1.55 × 104 L mol−1) led to the conformational change of Mb and resulted in the quenching of CL intensity.  相似文献
