对71例肾移植术后患者进行了氨苄青霉素与环孢菌素的相互作用研究。患者在开始服用氨苄青霉素之前、之中、之后的肾功能以及环孢菌素全血浓度测定结果表明,三项指标均无显著改变。另一组肾移植术后患者37例进行的诺氟沙星与环孢菌素相互作用研究,结果与氨苄青霉素的相互作用类似。提示氨苄青霉素、诺氟沙星可安全地与环孢菌素合用,而不影响肾功能。  相似文献

The excitatory transmitters glutamate and aspartate become toxic whenever their extracellular levels are increased because of neuronal, glial and endothelial impairment. Taurine, a volume-regulating amino acid, is released upon excitotoxin-induced cell swelling. Our aim was to investigate if glutamate and aspartate in cerebrospinal fluid (CSF) reveal neuropathology in neurological patients, and if taurine unmasks glutamate-mediated toxicity. Glutamate and aspartate are doubled in viral meningitis, acute multiple sclerosis (MS) and myelopathy compared with control subjects and patients with peripheral facial nerve palsy. These levels do not coincide with a disturbed blood–brain barrier, as estimated by the albumin ratio, are independent of their precursors (glutamine, asparagine) and are not associated with cell lysis. Taurine is significantly increased in meninigitis, acute MS, and myelopathy, suggesting glutamate-mediated toxicity. Analysis of transmitters in lumbar CSF can be used to identify patients with cerebral and spinal pathology who might benefit from specific receptor-modulating agents.  相似文献

Intravenously injected polystyrene microspheres with functional amino groups (AP-MSs, 0.2,1.0, and 4.0 urn in diameter) were cleared from the blood very rapidly. The calculated half-lives for 0.2-, 1.0-, and 4.0-μm AP-MSs were about 55,60, and 50s; no significant differences were found with 106,107, and 108 microspheres/rat. Loading experiments showed that the liver, spleen, lung, kidney, and heart had a very high capacity to take up AP-MSs. The AP-MSs were distributed mainly to the liver, lung, and spleen, whereas other organs contained less than 1 % of injected AP-MSs. In terms of numbers of AP-MSs per gram of tissue, the highest contents were found in spleen, liver, and lung for 0.2-, 1.0-, and 4.0-um AP-MSs, respectively. There was indication of redistribution of particles from one organ to another during the first 6 h after injection. Chondroitin sulfate A (Chon) and hyaluronic acid (Hya) adsorbed or covalently linked to AP-MSs increased uptake in the liver, with Chon AP-MSs (adsorbed or linked) showing the best effect: about 25% increase compared with unadsorbed 1-μm AP-MSs. Experiments with separated cells in vitro demonstrated that 1 um AP-MSs, intravenously injected, associated only with Kupffer cells. When the microspheres were adsorbed with Chon, there was also association with liver endothelial cells. This finding indicates that conjugation of microspheres with ligands for endothelial receptors may be a useful method for directing microspheres to specific organs, even if the receptors are not by themselves phagocytic  相似文献

目的 观察环孢素血药浓度与肾移植效果间关系。方法 对97例接受同种异体肾脏移植术受者术后8周内206例次环孢素血药浓度监测结果进行回顾性分析,按照受者术后的临床表现、生化指标将其分为术后正常组、急性排斥组、急性毒性组,采用荧光偏振免疫测定法,以单克隆抗体试剂测定环孢素血药浓度。结果 术后正常组62例移植肾功能良好,环孢素的给药剂量为52±1.9 mg/kg·d,171例次环孢素血药浓度的平均值为309.85±131.69μg/L;术后急性排斥组 26例,距发生排斥反应最近一次的环孢素血药浓度平均为165.80±123.13μg/L,环孢素的给药剂量为 4.8±1.6mg/kg·d;术后急性毒性组9例,距发生毒性反应最近一次的环孢素血药浓度平均为556.51±102.50μg/L,环孢素的给药剂量为6.2±1.0mg/kg·d。三组之间环孢素的给药剂量无显著性差异(P>0.05),但环孢素血药浓度却相差很大,两两之间有显著性差异(正常组与排斥组 P< 0.05;正常组与毒性组P< 0.05;排斥组与毒性组P< 0.01)。结论 环孢素浓度较低时,出现排斥反应的可能性较大;而环孢素浓度较高时,发生毒性反应的机会较多。  相似文献

[目的]了解泰州市市级各医院的消毒剂使用现状,为指导临床合理、规范使用消毒剂提供依据和指南。[方法]以泰州市5所市级医院为研究对象,全面调查临床使用消毒剂的种类及其使用中消毒剂的有效成份、年消耗量、消毒方法知晓率、染菌量等背景资料。[结果]5所医院使用的消毒剂种类基本相似,主要是含氯消毒剂等高中水平消毒剂,低水平消毒剂使用量很少,消毒剂年消耗量巨大。医院工作人员消毒知识知晓率低,消毒剂使用不合理,影响消毒效果。使用中消毒剂检测合格率为51.4%;其中消毒剂有效成份合格率71.2%;染菌量指标合格率90.9%。消毒剂染菌量合格率与消毒剂配制浓度、消毒时间、消毒方法知晓率之间呈正相关,相关密切程度:消毒时间〉消毒方法〉消毒剂配制浓度;消毒剂总合格率与消毒剂有效成份合格率之间呈正相关。[结论]医院消毒剂使用不合理,可致医院菌株对常用消毒剂产生抗药性。  相似文献

Ambulatory blood pressure monitoring was applied in 27 pediatric patients aged 6.3 – 24.3 (median 15.0) years who had been transplanted 1.5 – 8.4 years previously. Daytime values were compared with the mean of 10 concomitant casual blood pressure recordings. At the time of the study, antihypertensive drugs were given to 17 patients. Inulin clearance ranged from 18 to 116 (median 66) ml/min per 1.73 m2. Ambulatory blood pressure monitoring confirmed hypertension or normotension determined by casual blood pressure measurements in 63% of patients. The physiological nocturnal dip in blood pressure was attenuated or reversed in 8 of 27 patients. It was reduced in all 3 patients with renal artery stenosis of the graft, in 3 of 4 patients with chronic rejection, in the only patient with recurrent focal segmental glomerulosclerosis, and in 1 of 6 patients with past acute rejection. The dipping was not related to inulin clearance. In conclusion, casual blood pressure measurements do not accurately reflect blood pressure in pediatric patients transplanted more than 1.5 years previously. A reduced nocturnal dip in blood pressure may indicate an underlying renovascular or renoparenchymal pathology.
