长脉冲Nd:YAG激光非侵入性治疗皮肤浅表静脉畸形 总被引:5,自引:0,他引:5
目的前瞻性研究长脉冲1064nm Nd:YAG激光非侵入性治疗皮肤浅表静脉畸形的疗效。方法22例不同部位皮肤浅表静脉畸形患者接受长脉冲1064nm Nd:YAG激光治疗,6mm直径光斑,7~8ms双脉宽,脉冲间隔20ms,140~150J/cm2能量,接触式冷却,治疗间隔3周至1个月,治疗后1、6个月随访,评价病灶清除率分为五级。结果22例患者完成治疗,最多治疗5次。随访6个月,96.3%浅表静脉畸形获得76%~100%的清除,37%获得病灶的完全清除,未见复发。治疗部位无一例发生紫癜,治疗后8例(38.1%)出现红斑,1d至1个月消退。无色素改变和形成。结论长脉冲1064nm Nd:YAG激光能有效安全地治疗皮肤浅表静脉畸形,选择性地清除浅表血管,并使用接触冷却系统保护表皮,减少紫癜发生及色素改变和瘢痕的形成,达到理想的美容效果。 相似文献

Vitiligo is a skin disease with a worldwide prevalence ranging from 0.5% to 4%. Conservative therapies include photochemotherapy, phototherapy with UVB radiation (broadband UVB 290–320 nm, narrow band UVB 311 nm), systemic steroids and pseudocatalase. Modern therapeutic options include treatment with topical immunomodulators (tacrolimus, pimecrolimus), analogues of vitamin D3, excimer laser and surgery/transplantation. Our analysis compares these therapies for vitiligo and the evidence levels supporting their effectiveness. Conclusions: The face and neck respond best to all therapeutic approaches, while the acral areas are least responsive. For generalized vitiligo, phototherapy with UVB radiation is most effective with the fewest side effects; PUVA is the second best choice.Topical corticosteroids are the preferred drugs for localized vitiligo. They may be replaced by topical immunomodulators which display comparable effectiveness and fewer side effects.The effectiveness of vitamin D analogues is controversial with limited data. Surgical therapy can be very successful, but requires an experienced surgeon and is very demanding of time and facilities, thus limiting its widespread use. L-phenylalanine therapy appears effective on the face but enjoys neither widespread use nor extensive data support. No single therapy for vitiligo can be regarded as the most effective as the success of each treatment modality depends on the type and location of vitiligo. 相似文献

目的探讨经尿道钬激光膀胱肿瘤切除术的临床疗效和安全性。方法对20例膀胱肿瘤(Ta~T2a期)经尿道钬激光膀胱肿瘤切除,其中17例为初发肿瘤,3例为复发肿瘤。激光功率15~40W。小的肿瘤(直径〈1.0cm,窄蒂)直接汽化,大的肿瘤(直径〉1.0cm,宽带)自瘤体基底部切除,再将其周围1~2cm正常组织汽化、烧灼。结果20例膀胱肿瘤均一次切除,手术时间10~70min(直径〉1.0cm,宽蒂),平均30min。术中无闭孔神经反射、膀胱穿孔及水中毒发生,无输血。术后保留导尿时间1~5d,平均3d。16例随访3个月,4例随访半年,平均3.6月,均未见肿瘤复发。结论经尿道钬激光膀胱肿瘤切除疗效确切、安全性高。 相似文献

目的探讨将整形美容的方法引入CO2激光包皮环切术中治疗包皮过长及包茎的疗效。方法在行常规CO2激光包皮环切术中,将整形美容的方法贯穿始终。结果1624例中,1593例术后未发生并发症,伤口愈合良好,外形美观;7例术后6h内发生出血,24例术后9~20d切口裂开。结论将整形美容的方法引入CO2激光包皮环切术中,伤口愈合良好,外形美观,疗效满意。 相似文献

The purpose of this study was to present our experience with combined use of CO2 laser and cold instrumentation for Reinke’s edema surgery and to evaluate 1-year follow-up results of the technique in a
series of professional voice users. Fifteen patients with Reinke’s edema who underwent microlaryngoscopic surgery were included.
Videolaryngostroboscopy, perceptual and acoustic voice analyses were performed before and after surgery. During the 1-year
follow-up, no recurrence of Reinke’s edema was encountered. Significant postoperative improvement was obtained in the quality
of voice, in terms of GRBAS scores, Fo, jitter, shimmer and NHR. No evidence of laryngeal cancer was found on the histological
examinations. Combined use of CO2 laser and cold instrumentation provides a reliable and safe method for Reinke’s edema surgery, and cessation of smoking,
voice rest and control of the laryngopharyngeal reflux contribute to the success of surgery. We consider that the removal
of redundant mucosa of the vocal fold reduces the risk of the recurrence of Reinke’s edema and provides better quality of
voice. However, it does not imply that our method is superior to others’, but this procedure constitutes an effective treatment
of choice for Reinke’s edema patients, including professional voice users. 相似文献

Clinical investigations of laser photobioactivation, or biostimulation, might be differently designed and more fruitful if knowledge of basic biochemical mechanisms were better understood. In this investigation, biochemical events identified as responses to 904 nm irradiation included increased ascorbic acid uptake by fibroblasts. These cells also showed increased hydroxyproline formation, and this was increased several-fold by the addition of proline to the medium. Maximum biochemical responses were observed at a pulse frequency of 67 Hz and a pulse width of 150 nsec with an energy density of approximately 7 mJ/cm2 per exposure. Elements in the mitochondrial cytochrome system are proposed as the radiation absorbing chromophore(s). 我院应用He-Ne激光治疗551例声带小结,取得较好疗效。为研究作用机理,我们进行了免疫功能测定、测温试验、He-Ne激光照射前后的间接喉镜检查、显微结构研究、抑菌试验和过氧化脂质测定。结果提示He-Ne激光照射治疗声带小结的机理为照射后喉部温度升高、血管扩张、血循环加快,从而增强局部营养,促进喉部粘膜生长、消除组织水肿,加速炎性渗出物和毒素的排泄。而He-Ne激光本身没有杀菌和抑菌作用。 相似文献

We studied retrospectively 12 eyes with a solitary choroidal hemangioma in 12 patients over a period of 13 years. A peculiar, even hyperfluorescence of the tumor within the retinal venous phases occurred in all 12 cases. In all but four patients, laser therapy was performed to reduce subretinal fluid and partially destroy the tumor. Two of the three eyes without extensive retinal detachment or cystoid macular edema at the initial visit regained a final vision of 20/30 or better. The remaining 5 patients with the two major complications had a final vision of 20/80 or worse due to degenerative retinal changes. We believe that laser treatment is definitely advisable in the early stages of a circumscribed choroidal hemangioma.The study was performed at the Institute of Ophthalmology, University of Nijmegen. 相似文献
