观察血浆置换、内科治疗对中晚期重型乙型病毒性肝炎生存率的影响,探索中晚期重型肝炎的治疗方法。方法观察在内科治疗基础上给予血浆置换患者的肝功能、并发症及疾病的转归,与同期仅予内科综合治疗的患者相比较,对相应的临床资料进行统计学分析,从而了解两种治疗方法的疗效。结果血浆置换组36例,18例好转、18例死亡,内科治疗组32例,13例好转、19例死亡,两组之间患者生存情况无统计学差异;肝功能指标(ALT、AST、SB、ALB、TC、ChE和凝血酶原时间(PT)也无统计学差异。结论与内科治疗相比较,在其基础上给予血浆置换并不能提高中晚期重型病毒性肝炎生存率;中晚期重型肝炎患者的预后决定于其肝功能衰竭的程度。  相似文献

采用柱上聚合的方法制备血红蛋白溶液,解决传统血红蛋白聚合过程中由于戊二醛的活性过高导致平均分子量大、产物分子量分布宽的问题。该方法利用阳离子交换剂对修饰度(聚合度)小的血红蛋白吸附能力大的原理,使其在柱上富积,同时加入戊二醛进行聚合反应。结果表明该方法能比较有效地缩小聚合血红蛋白的分子量分布。  相似文献

Cells with abnormally high rates of sister-chromatid exchange (SCE) are uniquely characteristic of Bloom's syndrome (BS). However, in one in five persons a minor population of cells with a low-SCE phenotype circulates in the blood. The origin and significance of the low-SCE cells in BS have never been understood, although they are assumed to arise by somatic mutation. In the present investigation, the enigmatic high-SCE/low-SCE mosaicism was investigated by comparing the incidence in several subpopulations of persons in the Bloom's Syndrome Registry who exhibit the two types of cells, and a striking negative correlation emerged: in persons with BS whose parents share a common ancestor, the case in approximately half of registered persons, low-SCE cells are found only rarely; conversely, the mosaicism occurs almost exclusively in persons with BS whose parents are not known to share a common ancestor. Because those who share a common ancestor are predominantly homozygous-by-descent at the mutated BS locus, the negative correlation is interpreted to mean that the emergence of low-SCE cells in BS in some way depends on the pre-existence of compound heterozygosity. A corollary to this is that BS is genetically heterogeneous.  相似文献

Summary To investigate the effect of hyperthyroidism on the pattern and time course of O2 uptake ( O2) following the transition from rest to exercise, six patients and six healthy subjects performed cycle exercise at an average work rate (WR) of 18 and 20 W respectively. Cardiorespiratory variables were measured breath-by-breath. The patients also performed a progressively increasing WR test (1-min increments) to the limit of tolerance. Two patients repeated the studies when euthyroid. Resting and exercise steady-state (SS) O2 (ml·kg–1·min–1) were higher in the patients than control (5.8, SD 0.9 vs 4.0, SD 0.3 and 12.1, SD 1.5 vs 10.2, SD 1.0 respectively). The increase in O2 during the first 20 s exercise (phase I) was lower in the patients (mean 89 ml·min, SD 30) compared to the control (265 ml·min–1, SD 90), while the difference in half time of the subsequent (phase 11) increase to the SS O2 (patient 26 s, SD 8; controls 17 s, SD 8) were not significant (P = 0.06). The OZ cost per WR increment ( O2/WR) in ml·min–1·–1, measured during the incremental period (mean 10.9; range 8.3–12.2), was always within two standard deviations of the normal value (10.3, SD 1). In the two patients who repeated the tests, both the increment of O2 from rest to SS during constant WR exercise and the O2/WRs during the progressive exercise were higher in the hyperthyroid state than during the euthyroid state. While both resting and exercise O2 are increased in the hyperthyroid patients, the O2 cost of a given increment of WR is within the normal range. However, a small reduction in the O2 requirement to perform exercise following treatment of the hyperthyroid state suggests a subtle change O2 cost of muscle work in this disease.  相似文献

The possibility of involvement of a Na–Ca exchange mechanism in the contractile responses induced by a reduction of external Na concentration ([Na]0) has been studied in isolated guinea-pig aorta. Low-Na (11.9 mM) solution (Lisubstituted) produced a contraction in ouabain-treated muscles in the presence of phentolamine (10–6 M). The magnitude of the contraction was dependent on the duration of the pretreatment with ouabain (2×10–5 M). Ca-free solution, but not verapamil (10–6 M), abolished the contraction induced by low-Na solution. The muscles were loaded with various amounts of Na by incubating the tissue with ouabain and varying [Na]0 (11.9–148.7 mM) in the absence of Ca. The magnitude of the contractions induced in these muscles by low-Na solution containing Ca (2.5 mM) was dependent on the cellular Na content. Loss of cellular Na into low-Na solution followed a single exponential time course and the rate coefficient of Na-loss in the presence of external Ca was about twice as great as in the absence of Ca. Cellular45Ca uptake in low-Na solution was significantly greater in Na-loaded tissues (pretreated with ouabain for 3 h) than in normal tissues. The45Ca uptake in low-Na solution was not inhibited by verapamil. These results suggest that the contraction induced by low-Na solution is caused by a Ca influx which is dependent on internal Na (a Na–Ca exchange mechanism).  相似文献

Caffeine actions on currents induced by calcium-overload in Purkinje fibers   总被引:2,自引:0,他引:2  
The ionic events underlying the changes induced by caffeine in calcium-overloaded Purkinje fibers were studied by means of a voltage-clamp technique. The following results were obtained. In fibers exposed to strophanthidin (5 X 10(-7) M), a depolarizing clamp of suitable magnitude or duration is followed by an oscillatory current (Ios) often superimposed on a decaying inward tail current (the "tail current"). Caffeine (9 mM) abolishes Ios and increases the tail current. Caffeine has similar actions when calcium overload is induced by increasing [Ca]0 or decreasing [Na]0. The magnitude of the tail current is reduced by decreasing [Ca]0. The tail current appears with repolarizations to -40 mV or more negative values as Ios appears in the absence of caffeine. As with Ios the tail current can be triggered twice (during and after a test clamp of suitable characteristics), becomes more inward with repeated clamps and becomes larger with larger or longer conditioning clamps. During the recovery from caffeine exposure, the tail current decreases gradually as Ios returns progressively. It is concluded that both Ios and tail current are caused by calcium overload but are affected in opposite direction by caffeine, apparently because caffeine decreases the calcium overload in the sarcoplasmic reticulum (abolition of Ios) and increases the calcium to be extruded from the sarcoplasm (increase in the tail current).  相似文献

The distribution in pregnant C57BL mice (day 18 of gestation) of intravenously administered cadmium (Cd) chloride and mercury (Hg) chloride (0.75 mol/kg b.w.) was studied, with or without previous dithiocarbamate pretreatment. Diethyldithiocarbamate (DEDTC), disulfiram, or thiram (2×1 mmol/kg b.w.) or vehicle (gelatine) alone, were given by gavage 2 h before and immediately after injection of the metals. The mice were sacrificed 4 and 24 h later and subjected to autoradiography or impulse counting of excised organs.All the dithiocarbamates increased the concentration of both Cd and Hg in brain and most other maternal organs. While DEDTC and thiram, in that order, strongly increased Cd concentrations in whole fetuses (around 17-fold at 4 h) and all fetal organs measured, disulfiram caused a decrease in fetal Cd concentrations. For Hg, all the dithiocarbamates substantially decreased fetal levels. Disulfiram, for example, decreased Hg levels by a factor of 5. The 24 h values confirmed those at 4 h both elements, although the differences between control and treatment groups were less pronounced.Although the results suggest the formation of lipid-soluble metal-dithiocarbamate complexes in vivo (e.g., increased concentration in brain), this does not necessarily lead to increased fetal levels of the metals. The increased levels of Cd after thiram and DEDTC pretreatment, however, indicate a risk for higher Cd fetotoxicity. It is likely that Cd is released in fetal cells following metabolism of the dithiocarbamate moiety of the complex.  相似文献
