目的 研究主、被动最大张口度(AMMO、PMMO)与颞下颌关节强直严重程度的关系。方法 选取28只健康雄性绵羊随机分为实验组和对照组,每组各14只。实验组绵羊双侧颞下颌关节模拟髁突矢状骨折,其中左侧翼外肌被切断以阻断其功能;对照组绵羊未进行手术。于术前及术后12、24周对所有绵羊体重、AMMO、PMMO、颞下颌关节形态学特点进行测量评估。结果 实验组只有右侧保留翼外肌功能的颞下颌关节发生了骨强直。术后12、24周,实验组绵羊AMMO、PMMO、极限距离均显著低于对照组,差异均有统计学意义(均P < 0.05)。实验组绵羊AMMO和PMMO与骨融合区宽度、长度、面积及钙化程度均呈负相关(均P < 0.05),其中骨融合区面积为主要影响因素(术后12、24周相关系数r分别为-0.94、-0.95)。结论 颞下颌关节强直动物模型中阻断翼外肌功能可阻止骨强直的发生;对于早期髁突矢状骨折,可通过牙合垫或牙合板进行张口训练,进而阻断翼外肌功能。当颞下颌关节发生骨强直时,骨融合区面积越大,张口受限越明显。 相似文献

Ankylosis may be defined as the fusion of joint surfaces. Temporomandibular joint (TMJ) ankylosis is a condition that may
cause chewing, digestion, speech, esthetic and psychological disorders. It is a devastating disorder resulting in inability
to open the mouth. As a result of this, General anaesthesia, is very difficult to administer because laryngeal inlet is not
directly visualized. Even the blind nasal intubation is difficult because of small mandible and tongue fall following relaxation.
There are various techniques to overcome these challenges. At times these techniques fail and tracheostomy has to be done.
All the risks associated with difficult intubation, and general anaesthesia can be avoided if the surgery is done under conscious
sedation. Conscious sedation, a simple but safe and effective method of anaesthesia is described here, which allows successful

We describe a case of a 19-year-old young man with oligoarthritis type of juvenile idiopathic arthritis, who presented with several month duration of lower neck pain and progressive muscular weakness of all four limbs. X-rays of the cervical spine demonstrated spontaneous apophyseal joint fusion from the occipital condyle to C6 and from C7 to Th2 with marked instability between C6 and C7. Surgical intervention began with anterolateral approach to the cervical spine performing decompression, insertion of cage and anterior vertebral plate and screws, followed by posterior approach and fixation. Care was taken to restore sagittal balance. The condition was successfully operatively managed with multisegmental, both column fixation and fusion, resulting in pain cessation and resolution of myelopathy. Postoperatively, minor swallowing difficulties were noted, which ceased after three days. Patient was able to move around in a wheelchair on the sixth postoperative day. Stiff neck collar was advised for three months postoperatively with neck pain slowly decreasing in the course of first postoperative month.On the follow-up visit six months after the surgery patient exhibited no signs of spastic tetraparesis, X-rays of the cervical spine revealed solid bony fusion at single mobile segment C6-C7. He was able to gaze horizontally while sitting in a wheelchair. Signs of myelopathy with stiff neck and single movable segment raised concerns about intubation, but were successfully managed using awake fiber-optic intubation. Avoidance of tracheostomy enabled us to perform an anterolateral approach without increasing the risk of wound infection. Regarding surgical procedure, the same principles are obeyed as in management of fracture in ankylosing spondylitis or Mb. Forestrier. 相似文献

Abstract – The present clinical study investigated the outcome of intentional replantation using resection of the ankylosed sites of the root, extraoral endodontic treatment using titanium posts and Emdogain® for periodontal healing following trauma-related ankylosis. During an evaluation period of 6 years, 16 ankylosed teeth affected by replacement resorption were treated as described. Evaluation parameters before treatment and during the follow-up period included Periotest® scores, percussion sound and periapical radiographs. All findings were compared to those of the adjacent teeth. In a second accident, one tooth was lost after 7 months and was excluded as a dropout. Ankylosis did not recur in seven replanted teeth, which were observed for an average of 52.3 months (range: 24–68 months). Ankylosis recurred in eight teeth after an average period of 12 months (range: 4–26 months). An infraocclusion, normal or only slightly reduced Periotest® scores and normal percussion sound were preoperatively found in six of seven successfully replanted teeth, which corresponded to a relatively small area of ankylosis. The majority of the teeth showing recurrent ankylosis preoperatively presented with normal position, negative Periotest® scores and a high percussion sound which corresponded to an extended area of ankylosis. Statistically significant relationship between preoperative findings and the treatment outcome ( P = 0.031) have become apparent. The results indicate that the treatment of minor areas of ankylosis by intentional replantation, resection of the ankylosed sites and Emdogain® appeared to prevent or delay the recurrence of ankylosis in 7 of 15 teeth. 相似文献

Abstract – The purpose of the study was to evaluate periodontal healing after replantation of intentionally rotated teeth with healthy and denuded root surfaces. Ten teeth with hopeless prognoses because of extensive alveolar bone loss and deep pockets extending to the apexes only on one or two surfaces, but with a healthy periodontal apparatus on the other surfaces, were selected. A mucogingival flap was elevated and the teeth were extracted carefully so as not to damage the healthy remnant of the periodontal membrane remaining on the root surfaces. Thorough extra‐oral debridement was performed on the contaminated root surface and the remnant was carefully conserved. Retrofilling was also done to eliminate an endodontic cause of attachment loss. The teeth were horizontally rotated and then replanted so that the healthy root would face the connective tissue at the initially periodontally involved sites, and the root planed surfaces, which had been periodontally involved, would face the surfaces of the alveolar sockets at initially healthy sites. The teeth were splinted with adjacent teeth. Clinical parameters and radiographic examination were performed pre‐operatively, and at 3 or 6 months, 1, 2, and 3 years postoperatively. One tooth was extracted 1.5 years postoperatively due to reduced support and the treatment strategy of the patient. The other nine teeth were well maintained without any discomfort for the whole maintenance period of 3 years. In all teeth, areas that previously had no pocketing but were now against denuded root surfaces maintained the previous pocket depth readings. In areas where a deep pocket was present but now had a root surface with healthy periodontium, a distinct decrease of mean pocket depth was observed at the first reexamination 3 months postoperatively. The mean radiographic alveolar bone level increased from 0.3% to 45.3% in 2 years and was thereafter maintained for the entire observation period. Loss of periodontal space and possible root resorption were observed in only one case without other ankylotic symptoms. These results suggest that the healing without ankylosis of an extensive denuded root surface may occur by mechanisms other than the maintenance of a viable periodontal ligament on the root surface. 相似文献

Congenital infiltrating lipomatosis of the face (CIL-F) is a rare lipomatous lesion with diffuse fatty infiltration of tissues and hyperplasia of underlying bone. We report clinical and CT findings in an unusual case of CIL-F presenting with progressive hemifacial asymmetry, manifesting as severely restricted mouth opening owing to exophytic temporomandibular joint ankylosis. The role of imaging in diagnosis is presented with a review of the literature. Differential diagnosis of CIL-F and its exclusion as a cause of hemifacial hyperplasia are also discussed. 相似文献

The aim of the article is fourfold; firstly, to detect the aetiology of torticollis in patients with Müllerian duct/renal aplasia-cervicothoracic somite dysplasia syndrome; secondly, spine pathology in Müllerian duct/renal aplasia-cervicothoracic somite dysplasia syndrome varies considerably from one patient to another and there are remarkable differences in severity and localization; thirdly, mismanagement of congenital spine pathology is a frequent cause of morbid/fatal outcome; and fourthly, the application of prophylactic surgical treatment to balance the growth of the spine at an early stage is mandatory. Reformatted CT scans helped in exploring the craniocervical and the entire spine in these patients. The reason behind torticollis ranged between aplasia of the posterior arch of the atlas, assimilation of the atlas and extensive fusion of the lower cervical vertebrae (bilateral failure of segmentation) in four patients; in one patient, in addition to the hypoplastic posterior arch of the atlas, we observed ossification of the anterior and the posterior longitudinal spinal ligaments giving rise to a block vertebrae-like suggestive of early senile ankylosing vertebral hyperostosis (Forestier disease). Scoliosis at different spine levels was attributable to variable spine defects. Pelvic ultrasound showed the classical renal agenesis in four patients; whereas in one patient, the MRI showed pelvic cake kidney (renal fused ectopia) associated with ovarian, uterine and vaginal abnormalities. This is the first exploratory study on the craniocervical and the entire spine in a group of patients with MURCS association. 相似文献

目的:回顾我院应用牵张成骨序列治疗颞下颌关节强直及继发颌面部畸形的效果.方法:40例关节强直患者,平均年龄24.5岁(9~53岁),其中单侧强直11例,双侧29例,伴发OSAHS者27例.所有患者一期手术牵张成骨,二期手术拆除牵张器并同期行颞下颌关节成形术,同期或三期行正颌手术改善面部外形.通过术前术后最大张口度、面型及打鼾症状改善情况评估治疗效果.结果:关节成形术后40例患者张口度基本恢复正常,面型均得到显著改善,打鼾症状全部消失.经过4~72个月(平均20.5个月)的随访,4例患者关节强直复发.结论:应用一期牵张成骨二期关节成形治疗颞下颌关节强直及继发畸形能够精确控制面型和气道的改变. 相似文献

目的评价超声引导下持续收肌管置阻滞镇痛在膝关节僵直手法松解术后早期康复治疗中的应用效果。方法选择全膝关节置换术(TKA)术后关节僵直的患者36例,年龄45~75岁,体质指数18~25 kg/m2,ASAⅡ~Ⅲ级。所有患者均在静脉麻醉下由同一外科医生行手法松解术,而后在超声引导下行神经置管持续阻滞镇痛,置管成功后连接电子自控性输注泵行术后镇痛。根据神经阻滞镇痛方式不同,分为两组:持续收肌管阻滞镇痛组(A组)及持续股神经阻滞镇痛组(F组),每组18例。记录两组术后72 h内不同的观察点持续主动及被动运动功能锻炼时VAS评分、补救镇痛药物使用次数、术后12、24、36、48、72 h的膝关节活动度(range of motion,ROM)、住院时间及患者满意度等。观察镇痛期间恶心、呕吐、导管阻塞、渗液、脱管等并发症的发生情况。结果两组术后72 h镇痛期间不同时间点VAS评分、镇痛药物使用总次数及膝关节活动度比较差异无统计学意义(P>0.05);A组股四头肌力测试评分高于F组(P<0.05),住院时间短于F组(P<0.05),患者满意度高于F组(P<0.05),阻塞、渗液等置管并发症发生率低于F组(P<0.05)。结论膝关节僵直手法松解术后超声引导下持续收肌管阻滞镇痛与持续股神经阻滞镇痛均可获得较好的镇痛效果,但持续收肌管阻滞镇痛可有效保持股四头肌肌力,有利于早期主动功能锻炼,同时,置管的并发症发生率低,患者的满意度较高,为膝关节僵直手法松解术后康复治疗理想镇痛选择方式之一。 相似文献
