Wrinkled Edges on Soft Contact Lenses

ABSTRACT Wrinkling of the edges of some ultrathin soft contact lenses is reported, possibly as a result of partial dehydration in combination with heat disinfection and chemical preservatives. Wrinkling has been observed after a short period of wear, and generally does not cause any patient discomfort.     

The upper urinary tract following a ureteroneocystostomy (UNC) in animal experiments: I. Functional findings

Summary A ureteroneocystostomy (UNC) was carried out on 32 dogs. A standardised measurement of flow across the ureterovesical anastomosis revealed a significant obstruction on the day of the operation itself and on the first 5 days after the operation. On the 1st, 7th and 28th day after the operation there was no significant reduction in either tubulosecretory or glomerular function on the operated and non-operated kidneys.     

Cholesterol, weight, height, Quetelet's index, and colon cancer recurrence

The association of low serum cholesterol with colon cancer mortality suggests that low serum cholesterol promotes colon cancer recurrence. We compared cumulative 5-year recurrence-free rates of 279 colon cancer patients in relation to serum cholesterol, weight, height, and Quetelet's index. The median value for each variable was used to divide patients into those above the median, or at the median and below. Patients with median and lower serum cholesterol exhibited an 11% lower disease-free rate at 5 years. Patients above median weight were at significantly increased risk of recurrence in both sexes (76 vs 54%, z = 3.0026, p = 0.003). Progressively decreasing weight was noted with advancing stage in males but not in females. Women above median Quetelet's index were also at significantly greater risk of recurrence (74 vs 52%, z = 2.6109, p = 0.009). Patients above median height were at insignificantly increased risk of recurrence. This study indicates that body weight is a significant prognostic factor for patients with colon cancer.     

Wirksamkeitskontrollen der Desinfektionsmaßnahmen bei Candida-bedingter Prothesenstomatitis

Zusammenfassung:  Bei 36 pfegebedürftigen Totalprothesenträgern mit Prothesenstomatitis wurden in der Erstuntersuchung Candida albicans und Candida spec. quantitativ auf der Schleimhaut und auf der Prothesenoberfläche besrimmt. Nach Hygienemaßnahmen über 14 Tage hinweg konnte eine Keimreduktion um 85–95% nachgewiesen werden. Die Kontroll-untersuchung nach 4 Wochen zeigte einen Wiederanstieg der Keimzahlen um 40–100%. Pflegebedürftige Personen müssen daher im Rahmen der gerostomatologischen Betreuung regeimäßig auf Hefebesiedlung der Schleimhäute kontrolliert werden, um wirksame Bekämpfungsmaßnahmen durchsetzen zu können.
Summary:  In 36 persons needing geriatric care and wearing full dentures with a denture stomatitis, Candida albicans and Candida spec. were detected in the first examination on the mucosa and the fitting surface of the denture. After hygienic measures lasting 14 days an 85–95% reduction in fungi was evident. A control examination after 4 weeks showed a 40–100% increase in fungi. Patients, therefore, should be regularly checked with respect to mucosal yeast colonization, within the framework of geriatric patient care. This would ensure effective therapeutic measures.

RILMENIDINE ACTS AS A PARTIAL AGONIST AT PREJUNCTIONAL α2-ADRENOCEPTORS IN GUINEA-PIG ATRIA

1. The present study was carried out to determine whether rilmenidine, a recently introduced antihypertensive agent which acts on alpha 2-adrenoceptors, has partial agonist activity on prejunctional alpha 2-adrenoceptors in guinea-pig atria. 2. Isolated preparations of guinea-pig atria were incubated with [3H]-noradrenaline and the efflux of radioactivity induced by stimulation of intramural sympathetic nerves was used as an index of release of transmitter noradrenaline. 3. Rilmenidine (1 mumol/l) inhibited noradrenaline release evoked by short trains (five, 20 and 50 pulses) of sympathetic nerve stimulation and this inhibitory effect of rilmenidine was antagonized by the alpha 2-adrenoceptor antagonists, idazoxan (0.1 and 0.3 mumol/l) and rauwolscine (0.3 mumol/l) whereas it was not affected by the alpha 1-adrenoceptor antagonist prazosin (0.1 mumol/l). 4. On the other hand, rilmenidine (1 mumol/l) enhanced noradrenaline release evoked by long trains (150 and 300 pulses) of stimulation and this effect was also abolished by idazoxan (0.1 mumol/l). 5. These findings suggest that the effects of rilmenidine on transmitter release depend on the degree of auto-inhibition: when the concentration of noradrenaline in the biophase of the prejunctional alpha 2-adrenoceptors is low, rilmenidine acts as an agonist, but when the concentration is high it acts as an antagonist. Thus, rilmenidine, like clonidine, is a partial agonist on prejunctional alpha 2-adrenoceptors in guinea-pig atria.     

Magnetoencephalography/Magnetic Source Imaging in the Assessment of Patients with Epilepsy

Summary: Magnetoencephalographic (MEG) dipole source localization is a particularly promising new tool for noninvasive presurgical evaluation of epileptogenic foci. It is potentially more accurate than EEG localization techniques because magnetic fields are not attenuated or distorted by the skull and scalp, which allows cerebral sources to be modeled more simply. MEG spike and seizure sources are routinely co-registered with the patient's brain MRI for clinical interpretation. This has been called magnetic source imaging. Numerous studies have shown that MEG localization of foci agreed with lesion position, depth electrode and ECoG data, PET and MRI findings, and surgical success. The recent development of whole head sensor arrays has greatly enhanced the ease with which epileptiform magnetic fields can be recorded and analyzed.

Effect of Naloxone Infusion on the Rate of Epileptiform Discharges in Patients with Complex Partial Seizures

In experimental studies, endogenous opioids have shown protective effects on seizure recurrence and facilitatory effects on postictal inhibition that were reversed by the opioid antagonist, naloxone. We evaluated the effect of all-night continuous infusion of 10 mg naloxone on the rate of focal interictal epileptiform discharges (FIEDs) during sleep in eight men with complex partial seizures (CPS) during 2 consecutive nights. Patients with abundant FIEDs during the control night showed a mean increase of 39% in the rate of FIEDs per unit of time during the naloxone infusion night. During the naloxone infusion night, mean nocturnal plasma prolactin (PRL) concentrations in this group of patients showed significant elevation, which was correlated with increased density of FIEDs. All-night infusion of naloxone failed to show any effect on the remaining three patients with minimal or no FIEDs during the control night. Mean nocturnal plasma PRL concentrations in this group of patients was significantly lower than in the former group. Our data support the notion that, in response to interictal or ictal discharges, endogenous opioid peptides may exert an inhibitory action that is reversible by administration of naloxone.     

De novo direct tandem duplication of a small segment of the short arm of chromosome 7 (p21.22→22.1)

We report a boy with duplication of a small segment of the short arm of chromosome 7 (46, XY, dir dup (7) (p21.2→22.1). The boy presented with supravalvular pulmonary stenosis, atrial septum defect and mental retardation. The origin of the additional material from chromosome 7 was confirmed by fluorescence in situ hybridization. This technique in combination with the use of single-copy DNA probes may in future help to delineate the phenotype/karyotype correlation.
