  目的  了解江苏省南京市1956 — 2021年尘肺患者的生存状况及可能的影响因素,进一步探究其疾病负担水平,为制定新时代尘肺防治措施提供科学依据。  方法  于2022年10月整群抽取江苏省职业病防治信息管理系统中1956年1月1日 — 2021年12月31日南京市上报的3456例尘肺确诊患者进行随访调查,采用寿命表法和Kaplan-Meier法进行生存分析,应用Cox比例风险回归模型对生存时间的相关因素进行分析,并采用伤残调整寿命年(DALY)、因早死造成健康生命年的损失(YLL)和因伤残造成的健康寿命年的损失(YLD)综合分析尘肺的疾病负担。  结果  南京市1956 — 2021年累计报告尘肺病例3333例,以男性(89.3%)、初次诊断分期为I期(89.3%)、矽肺(72.1%)为主。生存分析结果显示,截至2021年12月31日,南京市尘肺患者中位数生存时间为30.20(95%CI = 29.53~30.86)年。多因素Cox比例风险回归分析结果显示,诊断年龄 ≥ 30岁和初次诊断分期为III期是南京市尘肺患者存活的危险因素,女性和制造业是南京市尘肺患者存活的保护因素。疾病负担分析结果显示,1956 — 2021年南京市尘肺共造成累计DALY损失27529.49人年,例均DALY损失8.26年;累计YLD损失23440.46人年,例均YLD损失7.03年;累计YLL损失4089.03人年,例均YLL损失2.53年。南京市1956 — 2021年因尘肺造成的例均YLD、例均YLL和例均DALY总体均呈下降趋势(χ2趋势 = 776.437、206.153和1040.571,均P < 0.001),YLD和DALY在1991 — 2000年造成的损失最大,分别为10042.02人年和10619.59人年,YLL则随时间的推移逐渐变大,2011 — 2021年造成的损失最大为1956.35人年。  结论  南京市1956 — 2021年尘肺的防控形势整体可控,但造成的疾病负担不容忽视,建议相关部门加强对重点人群、重点病种及重点行业的防控。  相似文献

采用反相HPLC法测定滴耳液中洛美沙星及其杂质的含量。采用TSk gel~(TM)C_(18)色谱柱,乙腈-0.1%三乙胺溶液(16:84,以磷酸调节pH为2.5)作流动相,检测器波长287nm。含量测定采用内标法,以诺氟沙星为内标,杂质检查采用面积归一化法。实验结果表明,该方法对杂质的分离度好,含量测定的平均回收率为99.85%(RSD=0.41%,n=5)。  相似文献

Coumarins have been shown to reduce blood glucose levels and improve insulin sensitivity in other studies. The purpose of this study was to investigate the effects of scopoletin, which is a type of coumarin family, on glucose uptake in 3T3-L1 cells to test the hypothesis that scopoletin exerts an antidiabetic function on adipocytes. Scopoletin significantly increased glucose uptake, which was associated with increased expression of the plasma membrane glucose transporter type 4 (PM-GLUT4) in 3T3-L1 adipocytes. This increase in PM-GLUT4 expression was promoted by phosphorylation of protein kinase B, activation of phosphatidylinositol-3-kinase (PI3K), and enhanced intracellular glucose uptake. Scopoletin also promoted phosphorylation of adenosine monophosphate–activated protein kinase (AMPK) and enhanced PM-GLUT4 expression. Scopoletin-induced glucose uptake in 3T3-L1 adipocytes was inhibited by treatment with the PI3K inhibitor wortmannin and the AMPK inhibitor compound C. These results suggest that scopoletin has an antidiabetic effect by stimulating GLUT4 translocation to the PM through activation of the PI3K and AMPK pathways in 3T3-L1 adipocytes, thereby upregulating glucose uptake.  相似文献

Objective Many metals, some of which have been classified as environmental endocrine disruptors,are used extensively in everyday consumer products and are ubiquitous in our living environment. In the present study, we aimed to explore the associations between the prevalence risk of type 2 diabetes and plasma levels of 20 trace elements as well as those of heavy metals in a Han Chinese population.Methods We conducted a case-control study to investigate the associations between plasma concentrations of 20 metals and diabetes in Jiangsu province. A total of 122 newly diagnosed cases of type 2 diabetes and 429 matched controls were recruited from community physical examinations in Suzhou City of Jiangsu Province. Plasma metal levels were measured by inductively-coupled plasma mass spectrometry.Results After adjusting for confounders, plasma vanadium, chromium, manganese, copper, zinc,arsenic, selenium, strontium, palladium, cadmium, cesium, and barium were associated with diabetes risk (P < 0.05). The adjusted OR increased with increasing concentration of vanadium, manganese,copper, zinc, and cesium.Conclusion Many metals, including manganese, copper, zinc, arsenic, selenium, and cadmium in plasma, are associated with the morbidity of diabetes. Monitoring of environmental metal levels and further studies are urgently needed.  相似文献

The protection from endocrine disruptors is a high regulatory priority. Key issues are the characterization of in vivo assays, and the identification of reference chemicals to validate alternative methods. In this exploration, publicly available databases for in vivo assays for endocrine disruption were collected and compared: Rodent Uterotrophic, Rodent Repeated Dose 28-day Oral Toxicity, 21-Day Fish, and Daphnia magna reproduction assays. Only the Uterotrophic and 21-Day Fish assays results correlated with each other. The in vivo assays data were viewed in relation to the Adverse Outcome Pathway, using as a probe 18 ToxCast in vitro assays for the ER pathway. These are the same data at the basis of the EPA agonist ToxERscore model, whose good predictivity was confirmed. The multivariate comparison of the in vitro/in vivo assays suggests that the interaction with receptors is a major determinant of in vivo results, and is the critical basis for building predictive computational models. In agreement with the above, this work also shows that it is possible to build predictive models for the Uterotrophic and 21-Day Fish assays using a limited selection of Toxcast assays.  相似文献

Contaminated soils have caused serious harm to human health and the ecological environment due to the high toxicity of organic and inorganic pollutants, which has attracted extensive attention in recent years. Because of its low cost, simple operation and high efficiency, soil washing technology is widely used to permanently remove various pollutants in contaminated soils and is considered to be the most promising remediation technology. This review summarized the recent developments in the field of soil washing technology and discusses the application of conventional washing agents, advanced emerging washing agents, the recycling of washing effluents and the combination of soil washing and other remediation technologies. Overall, the findings provide a comprehensive understanding of soil washing technology and suggest some potential improvements from a scientific and practical point of view.
