Fundus-first laparoscopic cholecystectomy   总被引:1,自引:1,他引:0  
Removal of the gallbladder with commencement of dissection at the fundus is well recognized as a safe technique during difficult open cholecystectomy because it minimizes the risks of damage to the structures in or around Calot's triangle. We report here the routine employment of liver retractors and fundus-first dissection during laparoscopic cholecystectomy (LC) as an alternative to techniques previously described.Retraction of the liver and fundus-first dissection was used in 53 patients who underwent laparoscopic cholecytectomy. There were 16 male and 37 female patients. Seven were operations performed during an acute admission and 20 had moderate or severe adhesions involving the gallbladder. Thirteen patients had a preexisting abdominal incision.The procedure was successful in 52 patients (98%), but in one patient it was converted to open operation because of dense adhesions. Median duration of operation was 90 min (range 35–240 min). There was no mortality and two complications (persistent right upper quadrant pain for 2 weeks after operation and bile leakage from the gallbladder bed).The facility to retract the liver and carry out a fundus-first dissection extends techniques developed for open surgery into the laparoscopic arena. BACKGROUND: Increasingly, laparoscopy is being used in critically ill patients in whom there is a question of intra-abdominal sepsis. We examine the cardiopulmonary effects of laparoscopy in a porcine model of adult respiratory distress syndrome (ARDS). METHODS: Domestic pigs (n = 12) underwent saline lung lavage and subsequent surgical abdominal exploration using either laparoscopy or conventional laparotomy. Hemodynamic and respiratory measurements were obtained. RESULTS: After pulmonary lavage, the two groups developed similarly diminished arterial pO2 (P <0.001), a worsened pulmonary shunt (P <0.001), and an increased alveolar-arterial oxygen gradient (P <0.001). The pulmonary compliance was significantly decreased in the animals undergoing laparoscopy (versus laparotomy, P <0.05). The mean pulmonary arterial pressure did not differ between the groups. The laparoscopic group had a higher pCO2 (not significant) and was more acidotic (P <0.05) than the laparotomy group. The laparoscopic animals had an increased heart rate (P <0.05), cardiac index (P <0.01), and oxygen delivery (P <0.005) as compared with the laparotomy group. CONCLUSIONS: During laparoscopy, animals with ARDS demonstrate further compromise in pulmonary physiologic parameters but overall cardio-respiratory function is preserved.  相似文献

PURPOSE: This study was aimed at comparing the effects of a neutral liquid and a neutral gas used as intraamniotic media on umbilical blood flow, O2 delivery, blood pressure, acid-base status, and electrolytes in the fetus at escalating intraamniotic pressures. METHODS: Eight fetal lambs underwent invasive monitoring of common umbilical blood flow, blood pressure, blood gases, sodium, and hematocrit, as intraamniotic pressure was raised from 0 to 30 mm Hg. The animals were divided equally in two groups depending on the intraamniotic medium used (group I, warmed saline and group II, air). Maternal systemic blood pressure, O2 saturation, and temperature were kept constant. RESULTS: In each group, a threshold level of intraamniotic pressure was evident, above which there was a significant decrease in the common umbilical artery blood flow, with concomitant fetal hypoxemia and hypercarbia. This intraamniotic pressure threshold was 20 mm Hg in group I (saline), but only 15 mm Hg in group II (air). CONCLUSIONS: Although both a neutral liquid and a neutral gas can safely be used as intraamniotic media, a neutral liquid medium allows for a wider range of safe intrauterine working pressure (0 to 20 mm Hg), as compared with a neutral gas (0-15 mm Hg).  相似文献

Traditional approaches to thoracic metastases and spinal cord compression have been well worked out and validated in the literature. Anterior decompression is clearly superior to laminectomy; vertebrectomy and reconstruction are indicated for sagittal collapse, instability, and pain; and surgical decompression is necessary in cases of bony impingement. The role of endoscopic and minimally invasive techniques in treatment of metastatic disease is evolving. Dr. Lieberman advocates the use of thoracoscopic anterior approaches as the principal application in these patients, whereas Dr. McLain has found that endoscopic assistance has vastly improved his results with posterolateral decompression. The two authors weigh the relative advantages and disadvantages of these approaches for the selected patient with metastatic thoracic disease.  相似文献

In the era of open cholecystectomy, common bile duct stones were approached by traditional choledocholithotomy. Retained or recurrent stones discovered after cholecystectomy were approached by endoscopic extraction techniques or repeat surgery. With the advent of laparoscopic cholecystectomy, the approach to choledocholithiasis became more problematic as techniques for laparoscopic extraction were rudimentary. Preoperative endoscopic retrograde cholangiopancreatography rapidly became an adjunct to laparoscopic cholecystectomy when common duct stones were likely. Experience, however, revealed that many of these procedures were unnecessary. With developing sophistication of laparoscopic techniques, a variety of approaches to common duct stones developed. These included: transcystic extraction, direct laparoscopic choledocholithotomy, intraoperative endoscopic retrograde cholangiopancreatography, antegrade sphincterotomy, and transcystic placement of a common duct stent with subsequent endoscopic sphincterotomy and stone extraction. It is the purpose of this article to define the current role of each of these methods in the laparoscopic approach to choledocholithiasis.  相似文献

PURPOSE: Laparoscopic techniques have enabled less invasive surgery in pediatric urology. We report our experience with laparoscopic nephrectomy, nephroureterectomy and heminephroureterectomy in 26 children, and evaluate our series to establish the safety and efficacy of such procedures for benign disease. MATERIALS AND METHODS: Laparoscopic procedures were done in 15 girls and 11 boys 4 months to 11 years old (mean age 37) during a 23-month period. Laparascopic surgery consisted of nephrectomy in 14 (1 horseshoe kidney), nephroureterectomy in 6 and laparoscopic heminephroureterectomy in 6 cases. RESULTS: Mean overall operative time for the laparoscopic procedures was 165 minutes (range 43 to 355). Blood loss was less than 5 cc in all cases and there were no intraoperative complications in our series. Overall postoperative analgesic administration during hospitalization was 0.20 mg./kg. morphine sulfate and 19 mg./kg. acetaminophen as well as 0.9 mg./kg. codeine in 6 patients who did not receive morphine. Three children were discharged home on the day of surgery, while 17, 3, 1 and 2 were discharged home on postoperative days 1, 2, 4 and 5, respectively. Cosmetic results were excellent in all cases. CONCLUSIONS: Nephrectomy, nephroureterectomy and heminephroureterectomy may be performed for benign disease in children using laparoscopy with minimal morbidity, minimal postoperative discomfort, improved cosmesis and a short hospital stay. It may even be done as an outpatient procedure.  相似文献

BACKGROUND: Open thoracic sympathectomy has been the established option for patients with essential hyperhidrosis. Recently, video-assisted endoscopic sympathectomy has provided a simple, safe, reliable, and cost-effective alternative to the earlier technique. With advances in instrumentation, performing the procedure through 2-mm and 3-mm needlescopic ports is now possible. The authors evaluate the effectiveness of so-called needlescopic thoracic sympathectomy for the treatment of primary hyperhidrosis. METHODS: Thirty five consecutive patients with a mean age of 24 years, including 23 men and 12 women, underwent bilateral needlescopic thoracic sympathectomies at the National University Hospital of Singapore. RESULTS: The mean operative duration was 56 minutes, and the mean hospital stay was 1.2 days. In no patient did Horner's syndrome or significant pneumothorax develop. The rate of success, defined as completely dry hands, was 97%. Two patients had unilateral recurrences that responded well to repeat needlescopic sympathectomies. We performed a total of 72 sympathectomies. CONCLUSIONS: Our study demonstrates that the use of miniature port access sites produces excellent medical and cosmetic results and is associated with a short hospital stay and low risk of complications.  相似文献
