目的探讨肝组织学、丙氨酸氨基转移酶(ALT)等因素与慢性乙型肝炎抗病毒疗效的相关性。
方法回顾性分析首都医科大学附属北京地坛医院2005至2010年住院行肝组织活检后抗病毒治疗的慢性HBV感染者共81例,采集患者抗病毒治疗基线人口学、生化学、病毒学及肝组织学结果并收集患者抗病毒治疗随访过程中病毒学指标动态下降情况,分析此类患者抗病毒治疗应答相关因素。
结果共纳入患者81例,其中男性60例;平均年龄36.5岁。肝组织学活检提示,肝脏炎症和纤维化≥ G2S2者54例(66.7%);肝脏炎症和纤维化< G2S2者27例(33.3%);治疗24周病毒学应答患者63例(77.8%)。肝组织学炎症或纤维化等级、ALT水平及患者平均年龄在抗病毒治疗应答组与无应答组差异均具有统计学意义(P均<0.05)。抗病毒药物类型[干扰素/核苷(酸)类似物]与患者病毒学应答无显著相关。
结论ALT持续正常或轻度异常(< 2 × ULN)的慢性乙型肝炎患者抗病毒疗效与患者肝组织学炎症/纤维化程度及ALT水平显著相关,建议此类患者行肝组织活检以评价抗病毒指征及预测抗病毒疗效。 相似文献

目的 探讨指背腱膜的构成特点及临床意义。方法 在30只成人尸体手标本上,通过模拟手术,对指背腱膜的构成特点进行了应用解剖学观测。结果 指背腱膜的构成复杂,其中的外侧腱束、外侧束、外侧腱和中央束在手指畸形的发生中有着特殊的临床意义。外侧腱束主要形成伸近节指间关节的功能解剖机制;其与外侧束形成的密切纤维联系而具有协同外侧腱的功能;该腱束与屈肌腱鞘有联系又使其具有平衡屈肌和伸肌肌力的作用。中央束在指背腱膜的整体协调、畸形的发生及矫正过程中起着重要的功能。结论 通过对指背腱膜的解剖研究,可以解释因指屈、伸肌腱平衡被破坏后所形成的畸形。 相似文献

Objective To explore the clinical characteristics and treatment outcomes of early-onset peritoneal dialysis-associated peritonitis (EOP). Methods Clinical data of patients with peritoneal dialysis-associated peritonitis (PDAP) from 2013 to 2018 in four tertiary hospitals of Jilin province were collected retrospectively. According to whether the dialysis time of the first PDAP was ≤12 months or not, the subjects were divided into EOP group (≤12 months) and late-onset PDAP (LOP) group (>12 months) , and clinical data, pathogenic bacteria, treatment outcomes of PDAP and prognosis of two groups were compared. Results A total of 575 patients were included, including 314 patients in the EOP group, with age of (56.53±15.57) years and 152 females (48.4%), and 261 patients in the LOP group, with age of (56.61±14.42) years old and 144 females (55.2%). Compared with LOP group, the proportion of pathogenic bacteria culture-negative in EOP group was higher and the proportion of streptococcal infection was lower (both P<0.05). The initial treatment efficiency and cure rate of EOP group were higher than that of LOP group, while the extubation rate was lower than that of LOP group (all P<0.05). Multivariate logistic analysis indicated that the cure rate of EOP was 79% higher than that of LOP (OR=1.79, 95%CI 1.13-2.82, P=0.012), and the extubation rate of EOP was 68% lower than that of LOP (OR=0.32, 95%CI 0.15-0.66, P=0.002). Kaplan-Meier survival curve showed that the cumulative rates of multiple PDAP, technical failure, all-cause death, and composite end points (technical failure or all-cause death) in EOP group were higher than those in LOP group (P≤0.001). After correcting for confounding factors by multivariate Cox proportional hazard regression, the risk of multiple PDAP, technical failure, all-cause death, and composite endpoint (technical failure or all-cause death) in EOP group was 2.02 times (HR=2.02, 95%CI 1.26-3.24, P=0.004), 2.53 times (HR=2.53, 95%CI 1.58-4.05, P<0.001), 2.66 times (HR=2.66, 95%CI 1.70-4.16, P<0.001) and 2.48 times (HR=2.48, 95%CI 1.78-3.43, P<0.001) of LOP group respectively. 目的 探讨体外循环和非体外循环下冠状动脉旁路移植术中患者脑氧饱和度(rScO2)的变化趋势。
方法 前瞻性选取2018 年2 月—2019 年6 月在上海市第一人民医院择期行冠状动脉旁路移植术的105 例患
者。其中,56 例接受非体外循环冠状动脉旁路移植术(OPCAB),49 例患者接受体外循环冠状动脉旁路移植
术(CCABG)。术中分别选择T0 ~ T9 10 个时间点检测rScO2,另外记录术后7 d 内术后认知功能障碍(POCD)
发生率。结果 CCABG 组患者手术时间长于OPCAB 组(P <0.05),术中最低鼻咽温和红细胞比容(HCT)
低于OPCAB 组(P <0.05),术中最高血糖(Glu)和血乳酸(Lac)高于OPCAB 组(P <0.05);但是两组术
中最低rScO2 比较,差异无统计学意义(P >0.05)。两组rScO2 在不同时间、组间、变化趋势上有差别(P <0.05)。
CCABG 组rScO2 与鼻咽温、Lac 呈正相关(r =0.229 和0.283,P =0.005 和0.002),而OPCAB 组rScO2 只
与Lac 呈正相关(r =0.214,P =0.010)。OPCAB 组和CCABG 组POCD 发生率分别为39.29%(22/56)和
32.65%(16/49),且CCABG 组POCD 患者最低rScO2 低于非POCD 患者(P <0.05)。结论 虽然OPCAB
与CCABG 术中患者rScO2 变化趋势有差异,但是rScO2 可能不是影响术后早期POCD 发生的主要原因。 相似文献

Objective:To investigate the effect of MCP-1 on mesenchymal stem cells(MSCs) homing to injured myocardium in a rat myocardial infarction(MI) model. Methods:Rat myocardial infarction model was established by permanent left anterior descending branch ligation. Mesenchymal stem cells from donor rats were cultured in IMDM and labeled with BrdU. The Rats were divided into two groups. Monocyte chemotactic protein I(MCP-1) expression were measured by in situ hybridization and immunohistochemistry in the sham operated or infarcted hearts at 1, 2, 4, 7, 14 and 28 days post operation in MCP-1 detection group. The rats were injected with MCP-1, anti-MCP-1 antibody or saline 4 days after myocardial infarction in intervention group. Then, a total of 5 × 10^6 cells in 2.5 ml of PBS were injected through the tail vein. The number of the labeled MSCs in the infarcted hearts was counted 3 days post injection. Cardiac function and blood vessel density were assessed 28 days post injection. Results:Self-generating MCP-1 expression was increased at the first day, peaked at the 7^th day and decreased thereafter post MI and remained unchanged in sham operated hearts. The MSCs enrichment in the host hearts were more abundant in the MI groups than that in the non-MI group(P= 0.000), the MSCs enrichment in the host hearts were more abundant in the MCP-1 injected group than that in the anti-MCP-1 antibody and saline injected groups (P = 0.000). Cardiac function was improved more in MCP-1 injected group than anti-MCP-1 antibody and saline injected groups(P= 0.000). Neovascularization in MCP-1 injected group significantly increased compared with that of other groups(P = 0.000). Conclusion: Myocardial MCP-1 expression was increased only in the early phase post MI. MCP-1 may enhance MSCs homing to the injured heart and improve cardiac function by promoting neovascularization. 相似文献
