BACKGROUND: The success of arterial switch operations for transposition of the great arteries largely depends on faultless coronary translocation and subsequent sufficient myocardial perfusion. However, in patients with complex coronary artery anatomy, coronary translocation is often difficult to perform by conventional surgical techniques alone. Therefore we developed the "bay window" technique as a useful adjunct in patients with complex coronary arteries undergoing concomitant coronary translocation and arterial switch operation. Early and midterm results of this technique are described. METHODS: Between September 2001 and February 2002, 4 patients with transposition of the great arteries with complex coronary arteries underwent arterial switch operation. The ages of the patients at the time of operation ranged from 8 to 52 days. Great arterial relationships were anteroposterior in 2 patients, right-oblique in 1, and side-by-side in 1. One patient also had ventricular septal defect. Coronary arterial patterns were as follows: absent left main trunk in 1 patient, short left main trunk in 1, and short right main trunk in 1. Both coronary arterial orifices were resected as a tall U-shaped cuff. The inferior half of the coronary cuff was sewn into a J-shaped incision on the pulmonary stump. The superior half of the coronary cuff was folded down inside to form a bay window channel. RESULTS: No coronary events occurred (ie, inclusive of coronary stenosis, myocardial infarction, and coronary death). Postoperative echocardiogram demonstrated normal ventricular wall motions in all 4 patients. Impact of carotid atherosclerosis on long-term mortality in chronic hemodialysis patients 总被引:5,自引:0,他引:5
BACKGROUND: Cardiovascular event is the major cause of mortality in patients on maintenance hemodialysis. We prospectively tested the predictive values of atherosclerotic parameters for all-cause and cardiovascular outcomes in 219 hemodialysis patients (age, 58 +/- 13 years; time on hemodialysis, 13 +/- 7 years; male/female, 144/75). METHODS: We measured blood homocysteine (Hcy), ultrasound carotid artery intima media thickness (IMT) and % aortic wall calcification at L2/3 region [% of calcification index in the abdominal aortic wall (%ACI)] by computed tomography (CT) scan, and followed all patients for 5 years. RESULTS: During the follow-up periods, 54 patients (25%) died, 40 (74%) of them of cardiovascular causes. IMT was significantly higher in patients who expired (0.75 +/- 0.02 mm) than in those who survived (0.62 +/- 0.01 mm). IMT was significantly correlated with age (r = 0.47, P < 0.01) and %ACI (r = 0.27, P < 0.01). The survival rate during the observation was significantly lower in the final IMT third (58%) than in the first (90%) and the middle IMT third (80%) (P < 0.01). Multivariate Cox proportional hazards analysis revealed that diabetes and IMT became independent determinants of all-cause and cardiovascular death. Adjusted hazards ratios of all-cause and cardiovascular mortality for an increase of 0.1 mm in IMT were 1.31 (95% CI, 1.07 to 1.59) and 1.41 (95% CI, 1.12 to 1.76). In contrast, %ACI at abdominal aorta and blood Hcy did not affect their 5-year mortality. CONCLUSION: These findings suggested that measurement of carotid artery IMT is useful for predicting long-term mortality in patients receiving maintenance hemodialysis. 相似文献

This study clarified the effects of long-term hypoxia and hypoxic exercise on monoamines in the whole brain, and in four specific regions of the rat brain. The male Wistar rat progenitors (P1 group) were randomly assigned to three groups: hypoxia (16.0% oxygen) and exercise (MHE-P1), hypoxia and sedentary (MHS-P1), normoxia and sedentary (MNS-P1). The male children of P1 (the first generation of hypoxic rats; F1) were randomly divided into two groups: hypoxia and exercise (MHE-F1) and hypoxic sedentary (MHS-F1). The monoamines of whole brain were measured in P1 males, and monoamines of cerebellum, frontal lobe, striatum and hippocampus were measured in F1 males. The monoamine levels of MHE-P1 were significantly lower than those of MHS-P1 and MNS-P1. No significant difference was found in monoamine levels between MHS-P1 and MNS-P1. Epinephrine, norepinephrine, and dopamine levels of the MHE-F1 group significantly decreased in the frontal lobe, cerebellum and striatum, compare with the other groups (hypoxic and sedentary; normoxic and sedentary, respectively). These monoamines in the hippocampus were not influenced by the hypoxia or hypoxic exercise conditions. This study suggests that long-term hypoxic exercise decreased monoamine levels in whole brain, and that sensitivity to hypoxia and hypoxic exercise differed according to brain region. 相似文献

The effect of toluene inhalation on oxidative damage in rat organs was examined. Male Wistar rats was inhaled toluene (1500 ppm for 4 h a day) for 7 days. Quantitatively and immunohistochemically, oxidative DNA damage, lipid peroxide (LPO) and superoxide dismutase (SOD) were examined. As a marker of the oxidative DNA damage, 8-hydroxy-2'-deoxyguanosine (8-OH-dG) immunoreactivity increased in the lung, liver and kidney. The amount of 8-OH-dG also increased in liver and kidney significantly. In the testis, the amount of 8-H-dG did not increase, however 8-OH-dG immunoreactivity enhanced in the spermatogonia. SOD immunoreactivity increased in the lung, liver and kidney. However, 4-hydroxy-nonenal immunoreactivity and the amount of LPO did not change in each organ. Thus, oxidative damage by toluene is mainly DNA damage, especially, the oxidative DNA damage observed in the lung, liver and kidney for the increase of the immunoreactivity and amount of 8-OH-dG. 相似文献

Summary.
Background: Trigeminal neuralgia is usually treated by the padding method using Teflon felt. However this can not be done in certain
cases in whom a large tortuous vertebrobasilar artery compresses the fifth nerve. The transposition method using the sling
may be an alternative method. But this method is not an easy procedure and requires a relatively large craniotomy. Two cases
were treated by a new and simpler effective technique.
Clinical presentation: Two cases of the trigeminal neruralgia were treated. The first case was a 71 year-old male and the second case was a 63 year-old
male. The history of the medical treatments were similar and both cases had had trigeminal nerve blocks and were prescribed
carbamazepin. However, the pain control was insufficient in both cases. In both cases, three dimensional computerized tomography
showed the large tortuous right vertebral artery ran just behind the clivus and compressed the right trigeminal nerve. In
the second case past history showed a recent hypertensive cerebellar hemorrhage.
Technique and results: A right suboccipital craniotomy were performed in both cases. In both cases, the right vertebral artery compressed the trigeminal
nerve in a rostral direction. The sling technique with nylon sutures was tried in both cases but failed during surgery. Then,
the bone fixation stainless plate was cut to 10 cm in length and pre-shaped with pliers. After being shaped, the distal end
of the plate was inserted between the vertebral artery and fifth nerve and the proximal end of the plate was fixed to the
skull by screw. The fifth nerve was completely isolated from the artery as they were in direct contact. After surgery, the
pain disappeared completely during the follow-up of one and a half year in the first case and 9 months in the second case.
Conclusion: The plate can be bent and curved with plier to suit each individual case. This technique is easily applied even when the
slings or other isolation technique is not available and appeared to achieve the mechanically stronger reposition and fixation
of a very large and tortuous artery away from the trigeminal nerve.

Tumor necrosis factor (TNF)-alpha inhibits insulin action; however, the precise mechanisms are unknown. It was reported that TNF-alpha could increase mitochondrial reactive oxygen species (ROS) production, and apoptosis signal-regulating kinase 1 (ASK1) was reported to be required for TNF-alpha-induced apoptosis. Here, we examined roles of mitochondrial ROS and ASK1 in TNF-alpha-induced impaired insulin signaling in cultured human hepatoma (Huh7) cells. Using reduced MitoTracker Red probe, we confirmed that TNF-alpha increased mitochondrial ROS production, which was suppressed by overexpression of either uncoupling protein-1 (UCP)-1 or manganese superoxide dismutase (MnSOD). TNF-alpha significantly activated ASK1, increased serine phosphorylation of insulin receptor substrate (IRS)-1, and decreased insulin-stimulated tyrosine phosphorylation of IRS-1 and serine phosphorylation of Akt, and all of these effects were inhibited by overexpression of either UCP-1 or MnSOD. Similar to TNF-alpha, overexpression of wild-type ASK1 increased serine phosphorylation of IRS-1 and decreased insulin-stimulated tyrosine phosphorylation of IRS-1, whereas overexpression of dominant-negative ASK1 ameliorated these TNF-alpha-induced events. In addition, TNF-alpha activated c-jun NH(2)-terminal kinases (JNKs), and this observation was partially inhibited by overexpression of UCP-1, MnSOD, or dominant-negative ASK1. These results suggest that TNF-alpha increases mitochondrial ROS and activates ASK1 in Huh7 cells and that these TNF-alpha-induced phenomena contribute, at least in part, to impaired insulin signaling. 相似文献
