Synthetic gastrin as a dual adrenergic blocker

A gastrin-like peptide, pentagastrin, was found to exhibit an- as well as-adrenergic blockade on various isolated preparations in vitro. The block appears to be competitive in nature. However, since the concentrations required to produce the block were well above the physiologic or hormonal range, the significance of the blockade is not clear at present. If natural gastrin is also found to exhibit such a block, it may suggest a possible secondary involvement of autonomic cardiovascular regulation in Zollinger-Ellison Syndrome, which is usually accompanied by hypotension.The author wishes to thank Dr. Eugene D. Starr-Edwards valve thrombosis detected preoperatively by transesophageal echocardiography

A 31-year-old woman developed increasing signs of congestive heart failure 15 years following placement of a mitral Starr-Edwards mechanical valve. A preoperative transesophageal echocardiogram (TEE) demonstrated a large, obstructive valvular thrombus and the patient underwent successful reoperative valve replacement.     

Liver abscess caused by Clostridium bifermentans following blunt abdominal trauma.

A case of hepatic abscess and subsequent septicemia caused by Clostridium bifermentans is described. The abscess manifested itself on the third day after blunt trauma to the torso. The patient had nausea, vomiting, fever, evidence of hepatic dysfunction, and subphrenic gas. This case illustrates the association of hepatic abscess and blunt trauma to the torso.

Free fatty acids and the binding of sulfonamides in the serum of genetically obese Zucker rat

The purpose of this study was to evaluate the serum chemistry profiles and the degree of protein binding of six sulfonamides in the sera of genetically obese Zucker rats and their lean litter mates. Free fatty acid concentrations were significantly elevated in the serum of obese rats. Obese rat serum had a mean fraction unbound for sulfanilamide, sulfadiazine, sulfapyridine, sulfamerazine, sulfisomidine, and sulfisoxazole which was significantly higher than that found in the lean rat serum. Altered serum chemistry and/or pathological states may play a significant role in altering drug protein interactions.
