Solitary fibrous tumor is a spindle cell neoplasm rarely arising in the thyroid gland. We present a 78-year-old man with the diagnosis of solitary fibrous tumor of the thyroid gland resected by subtotal thyroidectomy. Fine needle aspiration cytology via ultrasound guidance demonstrated a hypocellular aspirate that revealed follicular epithelial cells with mild nuclear atypia and scattered spindle cells with bland nuclei. Histologically, the patternless proliferation of spindle cells was seen among collagenous bundles, accompanied by hemangiopericytomatous vessels, and variously dilated follicles with mild atypical cells having slightly enlarged nuclei, indicating adenomatous goiter. The neoplastic spindle cells showed diffuse immunoreactivity to CD34, bcl-2, CD99 and vimentin, but were negative for cytokeratins, calcitonin, TTF-1 and CD5. Although solitary fibrous tumor arising in thyroid gland is rare, this tumor should be included in the differential diagnosis of thyroid spindle cell tumors and also that of adenomatous. 相似文献

Epithelial-to-mesenchymal transition (EMT), an essential developmental program, is involved in tumor progression. ΔNp63, a homolog of p53, is associated with the EMT program, but the detailed mechanism remains to be elucidated. In this study, we investigated the role of ΔNp63 in EMT during progression of oral squamous cell carcinoma (OSCC). Five OSCC cell lines and specimens from 78 patients with OSCC were used. The expressions of ΔNp63, p63α, p63β and epithelial markers (cytokeratins 5 and 14) was detected in the OSCC cells, but not in SQUU-B cells (high metastatic potential). E-cadherin was expressed in all OSCC cells. Mesenchymal markers were strongly expressed in the SQUU-B cells. Knockdown of endogenous ΔNp63 in HSC-2 cells induced morphological changes to the spindle shape, decreased the expression of epithelial markers, increased the expression of mesenchymal markers, increased migration and reduced proliferation. By contrast, SQUU-B cells overexpressing ΔNp63β showed changed their morphology from stromal cell-like to epithelial cells. However, E-cadherin expression was not affected by ΔNp63 knockdown or overexpression. Immunohistochemical staining revealed that cancer cells expressing vimentin were found at the invasive front in the OSCC specimens. The intensity of ΔNp63 expression was also decreased in these cells. Interestingly, the vimentin positivity or decreased intensity of ΔNp63 was positively associated with metastases and poor prognosis in the OSCC patients. These results indicated that ΔNp63 downregulation in cancer cells induces a mesenchymal phenotype that is related to tumor progression of OSCC. 相似文献

Many families of centromeric repetitive DNA sequences isolated from Struthioniformes, Galliformes, Falconiformes, and Passeriformes are localized primarily to microchromosomes. However, it is unclear whether chromosome size-correlated homogenization is a common characteristic of centromeric repetitive sequences in Aves. New World and Old World quails have the typical avian karyotype comprising chromosomes of two distinct sizes, and C-positive heterochromatin is distributed in centromeric regions of most autosomes and the whole W chromosome. We isolated six types of centromeric repetitive sequences from three New World quail species (Colinus virginianus, CVI; Callipepla californica, CCA; and Callipepla squamata, CSQ; Odontophoridae) and one Old World quail species (Alectoris chukar, ACH; Phasianidae), and characterized the sequences by nucleotide sequencing, chromosome in situ hybridization, and filter hybridization. The 385-bp CVI-MspI, 591-bp CCA-BamHI, 582-bp CSQ-BamHI, and 366-bp ACH-Sau3AI fragments exhibited tandem arrays of the monomer unit, and the 224-bp CVI-HaeIII and 135-bp CCA-HaeIII fragments were composed of minisatellite-like and microsatellite-like repeats, respectively. ACH-Sau3AI was a homolog of the chicken nuclear membrane repeat sequence, whose homologs are common in Phasianidae. CVI-MspI, CCA-BamHI, and CSQ-BamHI showed high homology and were specific to the Odontophoridae. CVI-MspI was localized to microchromosomes, whereas CVI-HaeIII, CCA-BamHI, and CSQ-BamHI were mapped to almost all chromosomes. CCA-HaeIII was localized to five pairs of macrochromosomes and most microchromosomes. ACH-Sau3AI was distributed in three pairs of macrochromosomes and all microchromosomes. Centromeric repetitive sequences may be homogenized in chromosome size-correlated and -uncorrelated manners in New World quails, although there may be a mechanism that causes homogenization of centromeric repetitive sequences primarily between microchromosomes, which is commonly observed in phasianid birds. 相似文献

Although many surgical modalities for spontaneous osteonecrosis of the knee (SONK) of the medial femoral condyle have been reported, few reports have described these treatment options from the etiological point of view. Recently, osteochondral autografting has gained popularity for use in small cartilage injuries. The aims of this study were to characterize the SONK lesion histopathologically and to report on preliminary clinical results of autogenous osteochondral grafting for SONK.Six patients with SONK of the medial femoral condyle underwent osteochondral autografting. Average age was 54.2 years (range, 50–57 years). Using Koshino's classification, three patients' lesions were classified as stage III and three as stage IV. Classical histological investigation of the lesions was performed in all cases. All the patients achieved favorable pain relief after osteochondral autografts. The mean duration of follow-up was 27.7 months (range, 23–45 months). An increase in the average Lysholm score was found, ranging from 54.7 preoperatively to 92.3 postoperatively.Histological investigation of the lesions revealed articular bone plate fracture with enchondral ossification, reactive cartilage tissue formation, and proliferation of fibrous tissue. An area of osteonecrosis was observed in detached or fragmented osteochondral lesions.Osteochondral autografting was performed on six patients for the SONK and the short-term clinical results were favorable. Small pulmonary nodules in patients with sarcoma are problematic, because it is difficult to distinguish such small metastatic nodules from benign. The purpose of this study was to establish management guidelines for such small pulmonary nodules in patients with sarcoma. Pulmonary nodules were detected in 70 of 206 patients with sarcoma. About 55 patients were classified as having pulmonary metastasis. Seventeen of these 55 patients with pulmonary metastases were excluded from the imaging review because they did not undergo the required imaging examination. This study reviewed 38 patients with metastatic nodules and 15 patients with benign nodules. A statistically significant relationship was observed between the size of the nodules and final clinical decision. The patients with pulmonary nodules which did not exceed 5 mm in size showed significantly better cumulative overall survival rate after the detection of pulmonary nodules than those with larger nodules (5-years: 58.4 vs. 20.4%). There was no significant difference in the overall survival rate between the patients with smaller pulmonary benign lesions which did not exceed 5 mm in size and those with a normal chest CT (5-years: 92.3 vs. 85.3%). The only factor to diagnose in a metastatic pulmonary lesion is the size of the nodules. If the nodule remains ≦5 mm in size for more than 6 months, the nodule will be a benign lesion. On the contrary, if the nodule becomes larger than 5 mm within 6 months, a surgical excision of the nodules is recommended. 相似文献

upper lobe emphysema and a shift of the mediastinal structures to the right. Two-dimensional computed tomography (CT) exhibited
left upper lobe emphysema and right upper lobe atelectasis. Three-dimensional (3D) spiral CT angiography showed a bilateral
tracheal bronchus. The left tracheal bronchus branch was compressed between the descending aorta and the ductus arteriosus.
After a right arteriopulmonary shunt operation, the patient’s respiratory condition improved dramatically, with spontaneous
closure of the ductus arteriosus. Subsequently, 3D-CT clearly exhibited the disappearance of tracheal compression. This combination
of bilateral tracheal bronchus and congenital heart anomaly is extremely rare. The 3D-CT is a powerful noninvasive means for

BACKGROUND: Although some laboratory findings are known to be indicators of the risk of giant coronary aneurysm formation among Kawasaki disease patients, an appropriate cut-off point to predict aneurysm formation is not clear. METHODS: One hundred and five patients with giant coronary aneurysms were selected from the 15th and 16th nationwide surveys of Kawasaki disease in Japan. A total of 2936 patients without Kawasaki disease were recruited from a single hospital as a control group. Odds ratios were calculated for six laboratory data with specific values as cut-off points. Receiver operating characteristic (ROC) curves were observed to determine the most appropriate laboratory tests and cut-off points. RESULTS: Hematocrit, leukocyte count, neutrophil proportion, and hemoglobin had one or more peaks of odds ratio for specific cut-off points, but they did not have a clear cut-off point for the predictor according to the receiver operating characteristic curves. Alanine aminotransferase (ALT) increased the risk of giant coronary aneurysms continuously so no clearly appropriate cut-off point was identified. Serum sodium concentration of 135 mEq/L had a peak of odds ratio, and those with <135 mEq/L had the highest odds ratio (4.78). This value seemed appropriate with a sensitivity of 78% and specificity of 57%, although the predictive positive value was as small as 5%. CONCLUSION: The author's propose that a serum sodium concentration of <135 mEq/L at the patient's first visit to hospital may be a predictor of giant coronary aneurysms due to Kawasaki disease. 相似文献
