We interviewed 51 blood donors in four major US metropolitan areas subsequently found to have had antibodies to human T-cell lymphotropic virus (anti-HTLV) in late 1984-early 1985. Sixteen donors (31%) reported that they or a sexual contact had a history of blood transfusion. Twelve donors (24%) reported that they or a sexual contact used intravenous drugs. Ten donors (20%) were blacks born in the southeastern US. Four of the male donors (15%) reported homosexual contact. The most common characteristic was an association with Japan or the Caribbean basin (61%). These results show a broader variation of epidemiologic backgrounds than anticipated. 相似文献

The ability of polyester white cell-reduction blood filters to prevent the growth of Yersinia enterocolitica in units of donated blood was studied. Sixteen units of freshly drawn blood were inoculated with 10, 50, 100, or 150 colony-forming units (CFU) per mL of a clinical isolate of Y. enterocolitica (serotype O:3). The units were subsequently fractionated into red cell concentrate and resuspended in AS-1 or AS-3 solution. One-half of the red cell concentrates in each solution were filtered within 15 hours of phlebotomy and stored for 42 days. The remaining units served as unfiltered controls. Bacterial growth was monitored by weekly cultures and, on the last storage day, by the presence of endotoxin and the formation of methemoglobin. One hundred twelve primary cultures (560 plates) were performed. Units collected in AS-1 and filtered remained sterile when initially inoculated with 50 CFU or less. Filtered units spiked with 100 CFU or less and collected in AS-3 remained sterile throughout their shelf life. All unfiltered units supported bacterial growth and the formation of endotoxin and methemoglobin. The filtration of freshly donated blood proves to limit the growth of Y. enterocolitica in red cell components. 相似文献

By using human bone marrow cells enriched for early progenitors by selective immunoadsorption and plated at low cell density (10(3) to 10(4) cells/mL/9.6 cm2) in semisolid methylcellulose culture, we have analyzed the cooperative effects of human colony-stimulating factor 1 (CSF-1), granulocyte-macrophage-CSF (GM-CSF), interleukin-1 alpha (IL-1 alpha), and gibbon as well as human recombinant IL-3 on the formation of monocytic colonies. CSF-1 alone stimulated mature monocytic colony formation by human CFU-M. However, in the presence of IL-3 and erythropoietin, CSF-1 stimulated maximal immature monocytic colony formation at low concentrations and inhibited the formation of granulomonocytic, erythrocytic, and mixed colonies. Cultures with CSF-1 and IL-3 contained more immature monocytic colonies than did cultures with CSF-1 alone. IL-1 alpha alone had little effect. However, IL-1 alpha in combination with optimal concentrations of either CSF-1, GM- CSF, or IL-3 increased the number of colonies containing immature or mature monocytic colonies. 相似文献

OBJECTIVE: This study compared the progression of joint damage in patients
with early active severe rheumatoid arthritis (RA) treated with cyclosporin
or parenteral gold. METHODS: In this open, randomized, multicentre study
with a blinded radiological endpoint, 375 patients who had suffered from
active severe RA for <3 yr were randomized to be treated for 18 months
with low-dose cyclosporin or parenteral gold. The groups were stratified
with regard to corticosteroid use. Primary efficacy variables were numbers
of erosions, erosion score and the Larsen-Dale joint damage score. RESULTS:
Joint damage progressed at similar rates in both treatment arms. In both
groups, patients receiving corticosteroids had less X-ray progression.
Rheumatoid factor positivity, high swollen joint count, high erythrocyte
sedimentation rate and pre-existing X-ray abnormalities predicted
progression of joint damage. Although numbers of serious adverse events
were similar, more gold patients (n = 65) than cyclosporin patients (n =
45) withdrew from study medication because of adverse events. CONCLUSION:
Cyclosporin was comparable to parenteral gold in retarding progression of
joint damage and was better tolerated in terms of adherence to therapy. The
open label design should be kept in mind when assessing this difference.

Hemoglobin and ferritin iron content have been followed during differentiation in tissue cultures of murine erythroleukemia cells (MELC) using the techniques of Mossbauer spectroscopy and electron microscopy. In undifferentiated cells grown without DMSO, only iron stored in ferritin was detected. The amount of iron in a cell grown in the presence of iron citrate is approximately 1.2 X 10(-14) g, whereas in a cell grown in the presence of transferrin the amount is approximately 0.28 X 10(-14) g. These quantities do not depend on the iron concentration in the nutrition medium in a range from 0.3 to 2.0 microgram Fe/ml and are the same for growth times between 8 hr and 7 days. Cells grown with DMSO contain, in addition to ferritin, increasing concentrations of hemoglobin. Chase experiments prove that ferritin iron participates in hemoglobin synthesis. The amount of ferritin iron reaches saturation within less than 8 hr in MELC grown with or without DMSO. In differentiating cells grown with iron citrate there is a decrease with time in ferritin iron content concomitant with the increase in hemoglobin. Cells grown with transferrin incorporate additional amounts of iron, which are approximately equal to the amounts used for hemoglobin synthesis maintaining a constant ferritin iron level. In the electron microscope, iron is seen only as ferritin within lysosomes. The density of the ferritin in lysosomes correlates with the ferritin iron concentrations determined by Mossbauer spectroscopy. 相似文献

A 73-yr-old woman with a 4 yr history of rheumatoid arthritis presented
with the clinical features of congestive cardiac failure. She had a good
early response to standard therapy although she subsequently developed
recurrent biventricular failure. The preservation of good ventricular
function on echocardiography in the face of clinical evidence of myocardial
insufficiency raised the possibility of constrictive pericarditis, which
was confirmed on cardiac catheterization. Constrictive pericarditis should
be considered in patients with rheumatoid arthritis who develop unexplained
cardiac failure. Early diagnosis requires a high index of suspicion and
cardiac catheterization may be necessary to confirm the diagnosis. Medical
treatment is largely ineffective and pericardiectomy should be considered.

目的应用全反式维甲酸和康莱特注射液逆转兔颊黏膜癌前病变,探寻癌前病变化学干预方法。方法在已制备的兔鳞状上皮癌前病变模型上,分别予以贴附全反式维甲酸(Alltransretinoic Acid,ATRA)药膜和注射康莱特注射液,采用病理组织学及免疫组化观察病变变化。结果药物治疗12周后,空白对照组16只家兔中2只逆转,6只病情稳定,8只恶化;药物组中,12只逆转,15只稳定,5只恶化;免疫组化显示:随病变的逆转试验进行,PCNA、Bcl-2蛋白表达下降明显(29/32,26/32)。结论全反式维甲酸和康莱特注射液可逆转家兔颊黏膜鳞状上皮癌前病变,有可能成为新的化学阻断干预药物。 相似文献

Endophthalmitis is a serious post-traumatic ocular complication that can lead to loss of vision. We report a case of acute post-traumatic endophthalmitis following a penetrating injury caused by an unusual organism, Brevibacterium casei . The patient was successfully treated with intravitreal antibiotics like ceftazidime and vancomycin, along with topical cefazolin and tobramycin. Brevibacterium casei can be added to the list of rare bacteria causing endophthalmitis and should be kept in mind by clinicians as a potential source of pathology. 相似文献

目的探讨不同方式营养支持治疗对重症急性胰腺炎的影响,评估最佳营养方式。方法回顾性分析天津西青医院2008年8月至2013年1月收治的30例重症急性胰腺炎患者的临床资料,随机分为两组,即早期肠内营养+肠外营养治疗(PN+EN)组及完全胃肠外营养治疗(TPN)组,每组15例。分析两组患者治疗后的血、尿淀粉酶,血白细胞及中性粒细胞,并发症发生率及肠道功能恢复状况、住院时间等。结果 PN+EN组较TPN组患者血、尿淀粉酶,血白细胞及中性粒细胞降低明显增快,且并发症的发生率低、肠道功能恢复快、住院时间短,更缩短了患者的自然病程,降低了患者的住院费用。结论营养支持治疗对重症急性胰腺炎具有重要的临床作用,尤其是早期肠内营养治疗结合肠外营养能够明显缩短患者的自然病程,显著降低各种并发症,疗效优于TPN。 相似文献
