Leighton's theory of sentiments: explorations and speculations on barriers to interpersonal forgiveness

In My Name Is Legion (Leighton, 1959), the basic assumption of Alexander H. Leighton's theory of sentiment is as follows: 'A given personality exists more or less continuously throughout life in the act of striving, and interference with that striving has consequences which in turn often lead to psychiatric disorder' (p. 136). This commentary focuses first on selected sources of interference to interpersonal forgiveness and then elaborates a research agenda that aims to expand our knowledge of barriers to interpersonal forgiveness.     

Anticipating the Future and Change

As in all of medicine, there are basic assumptions underlying the present system of dental education and delivery. These basic assumptions are facing challenges due in part to the marked reduction in the incidence of caries and the development of more effective therapies. This current period of time is described as an era of true change, rather than the mere modification of existing technology. Changes are going to occur in a number of areas. The move will be from government to private, from small to large, from generalist to specialist. Since change is inevitable, one can learn about and prepare for the stress of change, the changes happening now, and how to adapt to change. Although many changes are beyond the individual's control, their occurrence may be beneficial.     

Is there a role for sentinel lymph node biopsy in the management of sarcoma?

Is there a role for sentinel lymph node (SLN) biopsy in the management of sarcoma? Sentinel node biopsy has dramatically changed the management of melanoma and breast cancer, helping surgeons avoid radical lymphadenectomies in node negative patients who would previously have undergone a more morbid operation with little benefit, or remained pathologically unstaged. Many investigators have explored the use of lymphatic mapping for malignancies other than breast cancer or melanoma. Lymphatic mapping and sentinel node biopsy has not been investigated in the management of sarcomas, which is not surprising given that the majority of sarcomas spread by local extension or hematogenously. Regional lymph node metastases are rare; developing in about 3-10% of patients with localized disease. However, among certain subtypes of high-grade sarcomas there is a propensity for regional lymph node metastases. These include rhabdomyosarcoma, epithelioid sarcoma, clear cell sarcoma, synovial sarcoma, and vascular sarcomas. It is in these particular subtypes that there may be a benefit to SLN biopsy.     

An association between invasive breast cancer and familial idiopathic hyperparathyroidism: a case series and review of the literature

The possibility of an association between primary hyperparathyroidism and breast cancer has been postulated. We report here a sibship with three premenopausal breast cancers and hyperparathyroidism, and no detectable BRCA or MEN1 gene mutations. We explore genetic and molecular rationales for an association between these metabolic and neoplastic processes.
