Overall progeroid aspect of the Louis-Bar syndrome: pigmentary and keratotic phenomena]

A patient with ataxia-teleangiectasia is presented who developed numerous actinic keratoses and pigmented spots predominating the clinical picture with only few teleangiectasis. Premature ageing of the skin is perhaps the reason for the marginal dermatologic symptoms of this disease.

Haptoglobin- und Coeruloplasmin-Bestimmungen beim experimentellen Lathyrismus der Ratte

Zusammenfassung Bei Ratten wurde experimentell durch -Aminopropionitril Lathyrismus erzeugt. Die beiden Serum-Proteine Haptoglobin und Coeruloplasmin zeigten hinsichtlich ihrer Serum-Spiegelvor undnach der Behandlung keine statistisch signifikanten Unterschiede. Die Peroxydase- und Oxydase-Aktivität des Ratten-Serums wird demnach durch diese Behandlung nicht verändert. Da der Kohlenhydrat-Anteil des Haptoglobins aus der Grundsubstanz stammen soll, ist demnach durch -Aminopropionitril keine Veränderung dieses Anteils festzustellen. Das in der Leber synthetisierte Coeruloplasmin erleidet ebenfalls — zumindest quantitativ — keine Veränderungen.

Summary Lathyrism was produced in rats by feeding -aminopropionitril. Haptoglobin and Coeruloplasmin serum levels showed no significant difference before and after treatment. Encymatic activity of Haptoglobin and Coeruloplasmin therefore appears not to be changed in lathyric rat. Thus the assumption that the carbohydrate part of Haptoglobin originates from the groundsubstance could not be confirmed by these assays.

Retinoids in the treatment of skin aging: an overview of clinical efficacy and safety

Aging of skin is an intricate biological process consisting of two types. While intrinsic or chronological aging is an inevitable process, photoaging involves the premature aging of skin occurring due to cumulative exposure to ultraviolet radiation. Chronological and photoaging both have clinically differentiable manifestations. Various natural and synthetic retinoids have been explored for the treatment of aging and many of them have shown histological and clinical improvement, but most of the studies have been carried out in patients presenting with photoaged skin. Amongst the retinoids, tretinoin possibly is the most potent and certainly the most widely investigated retinoid for photoaging therapy. Although retinoids show promise in the treatment of skin aging, irritant reactions such as burning, scaling or dermatitis associated with retinoid therapy limit their acceptance by patients. This problem is more prominent with tretinoin and tazarotene whereas other retinoids mainly represented by retinaldehyde and retinol are considerably less irritating. In order to minimize these side effects, various novel drug delivery systems have been developed. In particular, nanoparticles have shown a good potential in improving the stability, tolerability and efficacy of retinoids like tretinoin and retinol. However, more elaborate clinical studies are required to confirm their advantage in the delivery of topical retinoids.     

Octreotide dependency and headache: a case report

We report on headache induced by a somatostatin octapeptide analog (octreotide) used for the treatment of acromegaly, This "rebound" headache has severe tension-type characteristics and occurs every 6–8 h. It resolves dramatically within minutes with octreotide administration. This is the first report of headache developing under treatment with octreotide.     

Etiology and pathogenesis of Attention-deficit Hyperactivity Disorder (ADHD): significance of prematurity and perinatal hypoxic-haemodynamic encephalopathy

Attention-deficit Hyperactivity Disorder (ADHD), defined as a disorder of awareness with impul-sivity, has lately been characterized as a dysfunction of the striatum (neostriatum = globus pallidus + putamen). This structure is in a unique position for contextual analysis and samples information from almost the entire cortex through its spiny neurons. The etiology is heterogeneous, with genetic as well as lesional factors. Among the latter, pre- and perinatal events are prominent. Advances in the understanding of the role of fetal circulatory insufficiency with loss of autoregulation and systemic hypotension have drawn attention to the vulnerability of watershed regions, including the striatum. Not only circulatory facts are important for this selectivity, however. The anatomical characteristics, with convergent glutaminergic afferent synaptic transmission from almost the entire cortex contribute to the vulnerability in ischemia-induced liberation of glutamate: The striatum becomes the victim of its virtue. Repeated hypoxic-ischemic events are particularly common in prematurity, a fact which seems to explain the high incidence of ADHD in this patient group. The magnitude of the problem is increasing with the increased survival rate among premature infants.

At the age of 4 weeks a suckling developed discoid erythema with scales and a few pustules at the face. Previously, his father having contact with cattle as a farmer had fallen ill from tinea barbae. In both cases Trichophyton verrucosum could be detected by culture. Dermatophytosis in sucklings has recently more often been reported about in the literature. This, however, does not apply to Tr. verrucosum infections.
