特殊染色和免疫组织化学染色在肾上皮性肿瘤诊断中的应用

目的 探讨肾上皮性肿瘤的诊断及鉴别诊断。方法 对91例具有较完整病理资料的肾上皮性肿瘤进行了常规组织学观察,Mowy胶状铁染色及CD10、细胞角蛋白(CK)7、波形蛋白免疫组织化学染色。结果 91例肾上皮性肿瘤中肾透明细胞癌78例,占86％;肾乳头状癌8例,占9％;肾嫌色细胞癌4例,占4％;肾嗜酸细胞腺瘤1例,占1％。78例肾透明细胞癌中CD10、波形蛋白阳性分别为63例(81％)和69例(88％),主要表现为细胞膜阳性。74／78肾透明细胞癌CK7呈阴性,17／17 Mowy胶状铁染色阴性或呈灶状粗颗粒。肾嫌色细胞癌4例,CK7均呈胞膜阳性;Mowy胶状铁染色表现为胞质内蓝色细网状;4例CD10肿瘤细胞膜均阴性,波形蛋白阴性。肾嗜酸细胞腺癌1例CD10,CK7,波形蛋白,Mowy胶状铁染色均为阴性或不特异。结论 在常规诊断肾上皮性肿瘤有困难时免疫组织化学CD10,CK7,波形蛋白染色以及Mowy胶状铁染色对鉴别诊断有帮助。     

身心因素与肠易激综合征患者生活质量的相关研究

目的调查肠易激综合征(IBS)患者生活质量状况并评估躯体及心理因素对它的影响。方法采用肠易激综合征生活质量量表(IBS-QOL)、临床症状问卷、艾森克人格问卷简式(EPQ-RSC)、简明心境问卷简式(POMS-SF)、医学应对问卷(MCMQ)对120例IBS患者进行测查,将IBS患者的心理健康水平(人格、情绪、应对)及躯体症状(肠道症状、肠外症状、睡眠障碍)与IBS患者生活质量进行相关分析。结果IBS患者IBS-QOL的8个因子和总分均显著降低(P<0.05),其中健康忧虑和饮食限制降低最为明显,除IBS症状外,肠外症状、睡眠障碍、精神心理异常在IBS患者中也很常见。心理因素中EPQ-RSC神经质(N)分、POMS中焦虑、抑郁等多项因子分及总分、MCMQ屈服积分与IBS-QOL总分呈负相关;躯体因素中IBS症状、肠外症状、睡眠障碍积分与IBS-QOL总分呈负相关。结论肠易激综合征患者的生活质量明显降低,除躯体症状外,心理功能失调也是决定患者生活质量的重要因素。     

Antigenic variation of the Epstein–Barr virus nuclear antigen EBNA1 as revealed by monoclonal antibodies

The Epstein–Barr virus (EBV) nuclear antigen EBNA1 plays an essential role in the replication of EBV episomes in latently infected cells and is the only viral protein that is consistently expressed in all programs of latent EBV gene expression. In this study, four monoclonal antibodies (MoAbs) directed to a region (amino acid residues 442–530) of EBNA1 were generated. Competitive enzyme-linked immunosorbent assay (ELISA) experiments using biotinylated MoAbs showed that they recognized distinct epitopes. Reactivity of these MoAbs with various laboratory EBV strains and field EBV isolates was shown to be heterogeneous in that EBNA1 from certain strains (isolates) was recognized and that from others was not. All four MoAbs showed such heterogeneous reactivity, and moreover, each MoAb showed a distinct spectrum of reactivity with these EBV strains (isolates). These results demonstrate an extensive structural variation in this region of EBNA1 as predicted by previous sequencing studies. These MoAbs will be useful as probes to dissect this structural heterogeneity of EBNA1.     

A novel locus for autosomal dominant nonsyndromic hearing loss identified at 5q31.1-32 in a Chinese pedigree

 Hearing impairment is an extremely heterogeneous disorder. A total of 35 loci and 17 related genes for autosomal dominant nonsyndromic hearing loss have been identified. In a Chinese pedigree characterized by autosomal dominant inheritance with bilateral, postlingual, progressive, and sensorineural nonsyndromic hearing impairment, the putative disease gene locus was localized to chromosome 5q31.1-32 by a genome-wide scan. Fine mapping indicated that the disease gene was located within an 8.8-cM region between markers D5S2056 and D5S638, with a maximum two-point logarithm of differences (LOD) score of 6.89 (θ = 0) at D5S2017. By the candidate gene approach, mutation screening of the DIAPH1 and POU4F3 genes at 5q31 was performed. No mutation was found, suggesting that this is a novel deafness locus, which has been named DFNA42. Received: May 8, 2002 / Accepted: October 1, 2002     

丹参和黄芪治疗气虚血瘀型心力衰竭的作用

本文报道91例气虚血瘀型心力衰竭(心衰)病人选用丹参和黄芪治疗作用。16例作气囊漂浮导管监测,发现黄芪明显增加心排出量、每搏量及心脏指数,而丹参无此作用,提示黄芪有正性肌力作用;30例给黄芪加党参,治疗后心功能分级及某些异常的血液流变学指标均有改善;44例先丹参,继之丹参加黄芪,治疗后心功能分级有明显改善,且能改善异常的血液流变学指标,明显抑制血小板聚集功能。提示丹参具有活血化瘀改善血液流变学及抑制血小板的聚集性,黄芪有补气强心改善心功能的作用。     

Persistent effect of a low dose of preadministered diethylnitrosamine on the induction of enzyme-altered foci in rat liver

The effect of a low dose of preadministered diethylnitrosamine(DEN) on the induction of enzyme-altered foci in the liversof male full-grown Fischer 344 rats was studied. As a pretreatment,DEN at a dose of 10 mg/kg body wt was injected i.p. At varioustimes after DEN pretreatment a complete initiation, consistingof administration of the same dose of DEN by the same routein rats subjected to partial hepatectomy (PH), was performed,followed by application of selection pressure. Enzyme-alteredfoci stained with -glutamyltrans-peptidase (-GTP) and glutathioneS-transferase placental form (GST-P) were then assayed. Decreasesin the numbers and areas of foci in the rats which receivedsaline + PH 14 or 28 days after DEN pretreatment were observedin comparison with rats which received saline + PH immediatelyafter DEN. On the other hand, the numbers and areas of fociwere not decreased in rats which received the complete initiation,consisting of DEN + PH, at various times after DEN pretreatmentwhen compared with rats which received these at the same timeas the DEN pretreatment. This persistent effect of DEN pretreatmenton the complete initiation lasted up to 182 days after the timeof DEN pretreatment. In this experiment, GST-P was found tobe a more sensitive marker for the detection of putative preneoplasticliver-cell foci than -GTP.     

新政府会计制度下公立医院会计核算的难点及对策

2019年1月1日公立医院正式开始实施新政府会计制度,通过研究公立医院在新制度下会计核算的衔接问题以及适应问题,找出公立医院会计核算将面临的难点,如固定资产及其累计折旧核算方式、应付职工薪酬、科教项目核算等,并给出相应的解决对策,如利用信息化手段对应付职工薪酬进行资金分配、利用成本配比处理医院科教项目核算难点等,帮助公立医院能够在改革中进步,提升公立医院会计核算管理水平。     

止消通脉宁对糖尿病微血管合并症患者心功能影响的临床研究

为了探讨中药复方止消通脉宁对有微血管合并症的亚临床期糖尿病心脏病患者心功能的影响 ,我们选择了 63例 2型有微血管合并症的糖尿病患者 ,分为对照组 (2 0例 )和治疗组 (4 3例 ) ,分别使用常规疗法和加用止消通脉宁的治疗方法 ,疗程 4周 ,着重观察心功能、眼底和 2 4h尿白蛋白定量的改变。结果 :止消通脉宁可以明显改善心功能、减少尿白蛋白定量 ,而常规治疗对照组则无明显作用。表明止消通脉宁干预治疗可以改善有微血管合并症的亚临床期糖尿病心脏病患者心功能的异常 ,从而预防和治疗糖尿病心脏病。     

乳腺髓样癌的超声检查与组织病理的相关性分析

目的:探讨超声检查在诊断乳腺典型髓样癌中的临床意义。方法:按严格的病理诊断学标准将病例重新分类为乳腺典型髓样癌和非典型髓样癌,比较两者间声像学特征的异同。结果:32例中有13侧重新诊断为乳腺典型髓样癌;另外19例为非典型髓样癌。13例典型髓样癌患者中的10例边界整齐光滑;全部19例非典型髓样癌病人没有一例边界光滑,或者边界呈锯齿状(16例),或者边界光滑伴局部不规则(3例)。非典型与典型髓样癌间肿瘤边界的规则性有明显的差异。结论:如果超声检查提示肿瘤边界呈锯齿状或肿瘤边界光滑伴局部不规则,应行整个肿瘤的病理评估防止典型髓样癌的过诊断。
