We compared the growth of 183 children with short stature (≤ 2SD) and 73 children of
normal height at age six who were visiting the Tanaka Growth Clinic. We classified these
short children as suffering from either idiopathic short stature (ISS, n = 119), GH
deficiency (GHD, n = 33) or small-for-gestational-age short stature (SGASS, n = 31) on the
basis of subsequent test results and other factors. We also conducted a retrospective
study of changes in their height, wt and nutritional intake over time. The mean changes in
height SD score from birth to 6 yr were –0.24 SD in normal height children with a normal
birth length and +2.27 SD in normal height children with a low birth length. In short
children, these changes were –1.93 SD for children with ISS, –2.41 SD for those with GHD
and +0.58 for those with SGASS. The mean changes from birth to 6 mo were –0.84 SD, −1.03
SD and +0.38 SD in children with ISS, GHD and SGASS, respectively. The mean change in
height SD score from birth to age 1 yr was –1.07 SD, –1.44 SD and +0.35 SD, respectively.
The decrease in height SD score from birth to 6 mo accounted for 43.5% of the decrease in
height SD score from birth to 6 yr in children with ISS and it accounted for 42.6% of the
decrease in children with GHD. Only 19% of short children bottle-fed well, and 53% fed
poorly, as opposed to 56% and 16% of normal height children who fed well and poorly,
respectively. Post weaning, only 22% of short children ate well, and 56% fed poorly, as
opposed to 53% and 17% of normal height children who fed well and poorly, respectively.
These findings demonstrated that growth failure started from early infancy in ISS and GHD
children. It was suggested that poor nutritional intake in infancy and early childhood was

We describe herein a case of unilateral pulmonary agenesis (PA) with oesophageal atresia (EA)/tracheoesophageal fistula (TEF) that was diagnosed prenatally and repaired by esophagoesophagostomy with stable postoperative course. The patient was born at 34 weeks gestation, after ultrasonography at 22 weeks gestation showed possible right-sided diaphragmatic eventration or PA and EA was subsequently suspected due to hydramnios. The initial X-ray showed mediastinal shift to the right, and coil up sign of the nasogastric tube, without intracardiac anomaly. Immediately after the diagnosis of EA/TEF and unilateral PA on day 0, the patient was intubated in the operating room, and a gastrostomy tube was placed. After pulmonary status stabilized, at 4 days old, EA/TEF was repaired through a thoracotomy in the right 4th intercostal space. The right main bronchus was noted to continue into the distal oesophagus; this fistula was ligated and divided, and a single-layer esophagoesophagostomy was performed under mild tension with one vertebral gap. The neonate was maintained on mechanical ventilation and gradually weaned to extubation at 7 days old. The postoperative course was uneventful, with the exception of prolonged jaundice that emerged at 3 months old. Laparoscopic cholangiography at that time excluded biliary atresia, and jaundice resolved spontaneously. Chronic myelogenous leukemia (CML) begins with an indolent chronic phase but inevitably progresses to a fatal blast crisis. Although the Philadelphia chromosome, which generates p210(bcr/abl), is a unique chromosomal abnormality in the chronic phase, additional chromosomal abnormalities are frequently detected in the blast crisis, suggesting that superimposed genetic events are responsible for disease progression. To investigate whether loss of p53 plays a role in the evolution of CML, we crossmated p210(bcr/abl)-transgenic (BCR/ABL(tg/-)) mice with p53-heterozygous (p53(+/-)) mice and generated p210(bcr/abl)-transgenic, p53-heterozygous (BCR/ABL(tg/-)p53(+/-)) mice, in which a somatic alteration in the residual normal p53 allele directly abrogates p53 function. The BCR/ABL(tg/-)p53(+/-) mice died in a short period compared with their wild-type (BCR/ABL(-/-)p53(+/+)), p53 heterozygous (BCR/ABL(-/-)p53(+/-)), and p210(bcr/abl) transgenic (BCR/ABL(tg/-)p53(+/+)) litter mates. They had rapid proliferation of blast cells, which was preceded by subclinical or clinical signs of a myeloproliferative disorder resembling human CML. The blast cells were clonal in origin and expressed p210(bcr/abl) with an increased kinase activity. Interestingly, the residual normal p53 allele was frequently and preferentially lost in the tumor tissues, implying that a certain mechanism facilitating the loss of p53 allele exists in p210(bcr/abl)-expressing hematopoietic cells. Our study presents in vivo evidence that acquired loss of p53 contributes to the blastic transformation of p210(bcr/abl)-expressing hematopoietic cells and provides insights into the molecular mechanism for blast crisis of human CML. A total of 2651 consecutive native Japanese women who underwent a glucose challenge test (GCT) were retrospectively investigated. GCT was performed between 24 and 27 weeks of gestation; each subject received a 50 g oral glucose load without regard to the fasting or fed state, followed by a determination of 1 h venous plasma glucose level. Women demonstrating GCT exceeding 130 mg/dl received a 75 g, 2 h oral glucose tolerance test to determine whether or not they had gestational diabetes mellitus (GDM). All women with GDM were treated with a strict diabetic protocol including insulin therapy. Forty-nine (1.8%) women were diagnosed to have GDM. The receiver-operator characteristic curve identified a GCT finding above 140 mg/dl as the cutoff value for detecting GDM, which showed a sensitivity and specificity of 96 and 76%, respectively. Our results suggest that the cutoff value of a 50 g GCT is 140 mg/dl to identify pregnancies with GDM in a Japanese population. 相似文献

To prevent or remedy musculoskeletal conditions, the relationship between obesity and the
characteristics of muscle mass and strength need to be clarified. [Subjects and Methods] A
total of 259 Japanese males aged 30–64 years were classified into 4 groups according to
the Japanese obesity criteria. Body composition was evaluated, and handgrip strength and
knee extensor strength were measured for the upper and lower extremities, respectively.
Physical performance was evaluated with a jump test. [Results] Obesity was positively
correlated with skeletal muscle mass index, percentage of whole-body fat, and leg muscle
strength and negatively correlated with the percentage of muscle mass index, body
weight-normalized handgrip strength, and knee extensor strength, and the jump test
results. [Conclusion] Weight loss may be a better approach than increasing muscle mass and

Background: In thoracoabdominal aneurysm (TAAA) repair, our technical modification of visceral reconstruction using longer cut pre-sewn side branches has provided good surgical outcomes. Here, we assessed the long-term durability and patency of revascularized branches using computed tomography (CT) to confirm the validity of our approach.Methods: Early and late CT evaluations were performed in 11 TAAA patients (males: 5; mean age: 60.6 years) using the Coselli graft to evaluate the position of main graft and the diverging pattern and patency of side branches. Seven of 11 were sutured in an extra-anatomical fashion using longer cut side branches.Results: In Anatomical (n = 4) and Extra-anatomical (n = 7) groups, the early patency of side branches was not significantly different. Although the late patency of right renal artery (RA) was 100% in both groups, the one of left RA was 60% in Extra-anatomical, while 100% in Anatomical. Furthermore, the main graft in Extra-anatomical was significantly posterior and leftward to the spine with left RA side branch diverging at an acute angle.Conclusions: When a pre-sewn branched graft designed for TAAA is used, the graft should be sutured in a fashion similar to normal patient anatomy to minimize the possibility of kinking of RA side branch for the patency. 相似文献
