Fate control has been often misconceptualized as a superstitious belief and overlooked in health psychology. It is not known how this cultural belief might impact the well-being of Chinese people living with HIV. This study examined the protective role of fate control for well-being and the potential mediation effect of resilience. Participants in this study were rural patients who contracted HIV via commercial blood donation. In this cross-sectional survey, 250 participants completed measures of fate control, well-being, and resilience. The results showed that fate control and resilience were positively associated with well-being. Resilience mediated the association between fate control and well-being. Our findings provide insight into the adaptive function of fate control as a cognitive defensive mechanism and highlight the need to incorporate this cultural belief in developing culturally sensitive intervention programs for resilience enhancement tailored for this understudied population infected with HIV living in rural China. 相似文献
In recent years, global collaboration has become a conventional strategy for new drug development. To accelerate the development process and shorten approval time, the design of multiregional clinical trials (MRCTs) incorporates subjects from many countries around the world under the same protocol. After showing the overall efficacy of a drug in all global regions, one can also simultaneously evaluate the possibility of applying the overall trial results to all regions and subsequently support drug registration in each of them. Several statistical methods have been proposed for the design and evaluation of MRCTs. Most of these approaches, however, assume a common variability of the primary endpoint across regions. In practice, this assumption may not be true, due to differences across regions (e.g., differences in ethnic factors and/or medical culture/practice). In this article, we use a random-effect model for modeling heterogeneous variability across regions for the design and evaluation of MRCTs. We also address consideration on the determination of the number of subjects in a specific region to establish the consistency of treatment effects between the specific region and the entire group. 相似文献
Tuberculosis (TB) of the hepatobiliary system is not uncommon, but as a cause of biliary strictures, it is very rare. It poses
difficulty in diagnosis and often requires surgical intervention to exclude underlying malignancy. To our knowledge, there
are fewer than 20 reported cases in the English literature. We report a 35-year-old Filipino woman who presented with a 3-day
history of obstructive jaundice, associated with significant weight loss and anorexia. Computed tomography (CT) revealed dilated
intrahepatic biliary system secondary to distal stricture at the confluence of the left and right bile ducts. Magnetic resonance
cholangiopancreatography characterised the lesion as an irregular stricturing at several sites in the common bile duct. Incidentally,
the scans also showed indeterminate pulmonary nodules in the right lower lobes. CT thorax confirmed bilateral involvement
of the lungs. She required percutaneous transhepatic drainage for biliary decompression. Tests on tissue from the lung lesions,
the blood, and the bile all confirmed the presence of TB. She was treated with anti-TB medication. This report emphasizes
the importance of considering TB as a possibile cause of biliary stricture, especially in South-East Asia. 相似文献
BACKGROUND: Pulmonary vein potentials recorded at the ostia of pulmonary veins (PV) are a useful guide for segmental isolation of the PV in patients with atrial fibrillation (AF). Even during coronary sinus pacing at 600 ms, atrial (A) and PV potentials can overlap in 50-60% of patients making the accurate identification of PV potentials very difficult. METHODS: Nineteen patients (M:F 15:4) with paroxysmal AF underwent segmental isolation of one or more PV. Coronary sinus (CS) pacing was performed at cycle lengths of 600/550/500/450/400/350/300 ms and bipolar electrograms were recorded from the 10 or 20 pole Lasso catheter placed at the atrial-PV junction in 27 pulmonary veins. Stimulus (S) to A, S-PVP and A-PVP intervals were measured during CS pacing at the different cycle lengths at sweep speed of 200 mm/sec. RESULTS: During CS pacing at 600 ms the A and PV potentials were significantly overlapped (A-PVP < or = 25 ms) in 15 of 27 (55%) veins. During pacing at 300 ms, the A and PV potentials were significantly separated (A-PVP > or = 25 ms) in 9 of the 15 veins where A and PV potentials overlapped and 21 of all 27 (78%) veins. In two patients pacing at 300 ms was associated with 2:1 conduction block from atrial to PV fascicle. CONCLUSIONS: Coronary sinus pacing at cycle length of 300 ms demonstrated better separation of A and PV potentials compared to pacing at 600 ms. This strategy is easier and less time consuming compared to extrastimuli testing. It also confirms that the electrophysiological properties of PV fascicles are different from that of the adjacent atrial musculature. 相似文献
Background: We conducted a meta-analysis to assess the overall risk of cardiac toxicity associated with trastuzumab treatment in elderly breast cancer patients.
Methods: We searched databases from PubMed, EMBASE and Cochrane Central Registry of Controlled Trials to identify relevant studies. Statistical analyses were conducted to calculate the incidence rate, overall hazard ratio (HR) and 95% CIs using a fixed effects model.
Results: A total of 116,342 and 360 elderly patients from five cohort studies and two randomized clinical trials (RCTs) were included for analysis. The pooled incidences of symptomatic congestive heart failure (CHF) and CHF/HF/CM were 6.4% (95% CI 4.1% – 9.4) and 16.4% (95% CI 16.19% – 16.61) in patients with median age of 67.5 years from two RCTs and in patients with median age of 67.5 (60 – 75), 71 (66 – 80+), 74.5 (65 – 89), 75 (66 – 81+) and 79.5 (60 – 99) from five cohort studies, respectively. Trastuzumab was significantly correlated with an increased risk of defined cardiac toxicities in five cohort studies (HR = 1.89, 95% CI 1.72 – 2.07, p < 0.00001) and two RCTs (HR = 3.00, 95% CI 1.71, 5.26, p < 0.00001). Sub-group analysis showed that the anthracycline-based chemotherapy increased the risk of CHF/HF and CM in patients among five cohort studies (HR = 2.16, 95% CI: 1.8 – 1.87, p < 0.00001).
Conclusion: Trastuzumab is likely associated with an increased risk of cardiac toxicity in elderly patients with HER-2-positive breast cancer. Carefully monitoring cardiac function in elderly patients receiving trastuzumab, particularly with concurrent use of anthracycline, is warranted. 相似文献
