The relationship between CD4-CD8-T cells in the peripheral blood of patients with pancreatic carcinoma and IL-4,IFN-γ levels

Objective To investigate the relationship between CD4-CD8-T cells ratio and IL-4, IFN-γ levels in the peripheral blood of patients with pancreatic carcinoma. Methods Floweytometer was used to analyze the CD4-CD8-T cells ratio in the peripheral blood of patients with pancreatic carcinoma and the IL-4 ,IFN-γ levels were detected by ELISA. Results The ratio of CD4-CD8-T cell in CD3+ T cell from 25 pancreatic carcinoma patients was(4.2 ± 1.7) %, the ratio of CD4-CD8-T cell in CD3+T cell from 45 healthy person was(6.3 ± 2.6) %, there was significant deviation between the two groups(P ＜ 0. 01). The IL-4 level of 25 pancreatic carcinoma patients was (86.3 ± 23.3) fg/L,the IL-4 level of 45 healthy person was (56.2 ± 9.2) fg/L,there was significant deviation between the two groups(P ＜0.01). The IFN-γ level of 25 pancreatic carcinoma patients was (16.4±4.8) fg/L before operation, the IFN-γ level of 45 healthy person was (27.4±3.8) fg/L,there was significant deviation between the two groups(P ＜0.01). The ratio of CD4-CD8-T cell in pancreatic carcinoma patient after operation was higher than before operation. It could be found negative correlation between CD4-CD8-T cells ratio and IL-4 level in pancreatic carcinoma patient,it could also be found positive correlation between CD4-CD8-T cells ratio and IFN-γ level in pancreatic carcinoma patient. In pancreatic carcinoma patient,the CD4-CD8-T cells ratio and IL-4 level was significant associated with clinical stage (P ＜ 0.05), but no relationship with hisological differentiation (P＞0.05), it could be found no relationship between IFN-γ level and clinical stage, hisoiogieal differentiation (P ＞ 0.05). Conclusion The CD4±CD8±T cells ratio in the peripheral blood of patients is decreased,it may be participate in the carcinogenesis and development of pancreatic carcinoma by influence the IFN-γ levels.     

不同敷料组合对烧伤患者供皮区创面微环境的影响

Objective To investigate the effect of dressing materials in various combinations on bum wound microenvironment and healing condition. Methods Two hundred donor sites with wounds of 0.3 mm in depth in 186 burn patients, who needed skingrafting and admitted to our ward were enrolled in study, and they were divided into A ( with dressing composed of aiginate + cotton pad for donor area), B ( with dressing composed of vaseline gauze + cotton pad for donor area) , C (with dressing composed of algi-nate+ foam dressing for donor area), D ( with dressing composed of vaseline gauze + foam dressing for donor area) groups according to random table method. Effect of dressings on wound evaporation and pH value were observed. Bacterial colonization, degree of pain complained by patients after dressing change, and wound healing time in each group were compared. Results One hundred and eighty-four patients complied with the study, while 2 patients were excluded due to untimely falling-off of the dressing. Wound evaporaton in respectively, among them B group showed optimal effect of keeping humidity (P<0.01). Wound pH value in A, B, C, D groups was 7.22±0.06, 7.41±0.03, 7.05±0.03, 7.34±0.06, respectively, among them it was highest in B group. The positive rate of bacteria in D group was highest (22.4%), and lowest in C group (4.0%). Pain was lightest in C group (score was 0.98±0.12), and most serious in B group ( score was 8.14±0.82). The shortest wound healing time was seen in C group (6.7±0.8 d) , and longest in D group (15.6±3.5 d). Conclusions Application of various dressings on similar wounds can pro-duce different wound microenvironment, which is closely related to wound healing time. Compared with pH value, humidity is the more important factor for wound healing.     

致伤物嵌入颅内致开放性颅脑损伤四例

致伤物嵌入颅内是一种特殊的开放性颅脑损伤，临床上相对少见，在诊治上具有特殊性。我院1999年1月-2006年6月共收治4例，效果满意。报告如下。     

灌酸盐敷料在烧伤供皮区创面的应用

关于烧伤手术供皮区创面的处理 ,传统方法是采用凡士林纱布贴敷 ,该法存在渗血多、患者疼痛、创面易感染和愈合迟缓等缺点。我科于１９９８年２月至２０００年２月使用藻酸盐敷料贴敷 ,具有止血佳、防感染、疼痛轻、创面愈合快等特点 ,报道如下。１．一般资料因烧伤取自体皮手术病人５７例 ,供皮区均为大腿。其中治疗组３５例 (男１９例 ,女１６例 ) ;年龄６～５２岁 ,平均年龄(２６．１±２２．０６)岁 ;供皮区面积 (指占人体总体表面积的百分比 ,下同 )１%～６% ,平均３．５２ %±２．７５%。对照组２２例 (男１３例 ,女９例 ) ;年龄５～…     

自固化磷酸钙根管封闭剂的临床疗效初步评价

目的:观察自固化磷酸钙根管封闭剂作为根管充填材料的临床疗效。方法:选择136例慢性根尖周炎患者,常规根管预备后,随机选择72例为实验组,采用牙胶尖和自固化磷酸钙根管封闭剂充填根管;对照组64例,以牙胶尖和碘仿糊剂充填根管。术后1周、3个月、6个月、1a复查,观察术后反应情况,评价临床疗效,对数据进行χ2检验和Wilcoxon等级秩和检验,应用SAS6.2统计软件包。结果:实验组的术后反应较小,3个月的临床成功率高于对照组;而随着观察时间的延长,两者间差异无显著性。结论:自固化磷酸钙根管封闭剂可作为一种根管充填材料使用,远期疗效有待进一步研究。

银屑病关节炎32例报告

银屑病关节炎是一种与银屑病(牛皮癣)有关的炎性关节病,病理改变主要为炎性滑膜炎,病程迁延,易复发,晚期可出现关节强直而致残.我国1984年的调查,银屑病的发病率为1.23%,银屑病关节炎仅占银屑病的0.69%,更为少见[1],常易误诊.我院1997年9月～1999年6月诊治的32例银屑病关节炎中28例曾误诊,误诊率87.5%.现就其临床表现及误诊情况报道如下.     

快速循环心境障碍患者的成因分析

目的探讨双相心境障碍患者快速循环发作的形成原因。方法对10例双相心境障碍快速循环发作患者的一般情况、治疗过程、药物种类、剂量、效果等进行统计分析。结果10例心境障碍快速循环发作患者中5例有家族史,4例个性内向,4例外向,以抑郁为首次发作者7例,平均使用药物4～11种,9例确诊时使用抗抑郁药物,5例联合使用过2种抗抑郁药物,5例明确诊断前联合使用心境稳定剂;控制快速循环时均使用心境稳定剂,其中使用2种者3例,单一使用丙戊酸钠2例;平均循环次数0.77±0.41次·mo-1。结论快速循环形成过程中使用抗抑郁药物可能是一个危险因素,未及早使用心境稳定剂也是可能因素之一。
